Central venous catheters have been increasingly used in critical patients. These widespread uses have also led to the occurrence of some life-threatening complications.Patients who were hospitalized in Gazi University School of Medicine Pediatric Intensive Care Unit between January 2019 and September 2019 were included in our study. The obstruction of the inserted catheters was examined and the factors that could cause obstruction were investigated.188 patients were hospitalized in the intensive care unit and 73 catheters were inserted during the study. 37% of the patients were female and 63% were male. 52 were intubated and 21 were extubated. 57 of them were central venous catheters and 16 of them were hemodialysis catheters. The most common indication for catheter insertion was the difficulty of peripheral vascular access (46,6%). 18 of the 73 catheters have obstruction. There were more obstruction in catheters in intubated patients, in femoral regions, in 4 Fr and 5 Fr catheters, catceters inserted without USG; and central venous catheters rather than hemodialysis catheters. These differences were without statistical difference (p> 0,05). There was no significant relationship between age, catheter lumen number and length, catheter insertion time, vascular trial number, practitioner experience and obstruction. Experienced practitioners were shown to insert catheters with USG rate of 85,2% under 3 min (p<0,05). The use of USG has been shown to reduce complications and shorten the insertion time.As a result; obstruction of central venous catheters can lead to catheter dysfunction and serious problems. Regular maintenance of catheters and elimination of identified risk factors can prevent obstructions. In addition, complication rates have been shown to be low in USG guided catheters. Therefore, the use of USG should be made widespread.
Sağladığı hasta konforu ve tıbbi uygulamaları kolaylaştırması nedeniyle gittikçe kullanımları artan santral venöz kateterler; yoğun bakım ve yenidoğan üniteleri gibi kritik hastaların olduğu yerlerde sıklıkla kullanılmaktadır. Bu yaygın kullanımlar aynı zamanda hayati olabilen bazı komplikasyonların oluşmasına da neden olmuştur.Çalışmamıza Ocak 2019 ile Eylül 2019 arasında Gazi Üniversitesi Tıp Fakültesi Çocuk Yoğun Bakım Ünitesinde yatırılarak kateter takılan hastalar dahil edilmiştir. Takılan kateterlerin tıkanma durumu incelenip, tıkanmaya neden olabilecek faktörler araştırılmıştır.Çalışma süresince; yoğun bakıma 188 hasta yatırılıp, 73'üne kateter takılmıştır. Hastaların %37'si kız, %63'ü erkekti. 52'si entübe, 21'i ekstübe izlenmiştir. Takılan kateterlerin 57'si santral venöz kateter, 16'sı hemodiyaliz kateteriydi. En sık kateter takılma endikasyonu (%46,6) periferik damar yolu açılmasının zor oluşuydu. 73 kateterin 18'inde tıkanma olup, entübe hastalarda, femoral bölge yerleşimli, 4 Fr ve 5 Fr kalınlıktakiler de, USG kullanılmadan takılanlarda ve santral venöz kateterlerin hemodiyalizlere oranla daha fazla tıkandığı görülmüştür, ancak istatistiksel anlamlı fark yoktur (p>0,05). Yaş, kateter lümen sayısı ve boyu, kateter takılma süresi, damar deneme sayısı, uygulayıcı tecrübesi ile tıkanıklık arasında anlamlı ilişki bulunmamıştır. Tecrübelilerin sıklıkla 3 dk altında ve %85,2 oranında USG ile kateter taktıkları gösterilmiştir (p<0,05). USG kullanımının komplikasyonları azaltıp takılma süresini kısalltığı gösterilmiştir.Sonuç olarak; yeterli ve güvenli damar erişimi sağlayan santral venöz kateterlerde olan tıkanma, kateter disfonksiyonuna ve hastalarda ciddi problemlere yol açabilmektedir. Kateterlerin düzenli bakımı ve belirlenen risk faktörlerleri ortadan kaldırılarak tıkanıklıklar önlenebilir. Ayrıca USG eşliğinde takılan kateterlerde komplikasyon oranlarının düşük olduğu gösterilmiştir. Bu sebeple USG kullanımı yaygın hale getirilmelidir.
