An accurate impression and definitive cast are fundamental to a successful outcome in any prosthodontic rehabilitation. Impression techniques for implant-supported prostheses, have been directly adapted from traditional prosthodontics. The most crucial step in the fabrication process is the accurate three-dimensional (3D) capture and transfer of the implant position from the mouth to the definitive cast via an impression. The resultant prostheses misfit can lead to potential biomechanical complications due to excessive stress within the prosthesis and bone-implant-prosthesis interface. Osseointegrated implants have a very limited capasity to compansate for discrepancies in prosthetic framework fit, because of their limited movement allowance compared to natural teeth, It is therefore critical for an implant impression and resultant definitive cast to be as accurate as possible in order to fabricate a successful implant-supported prostheses The advent of digital tecnology gives clinicians the option to use intraoral scanners instead of conventional impression tecniques. It is claimed that the use of digital impressions eliminates the materials used in conventional impressions and potentially contributed to patient comfort as well as reducing misfit rate. Several in-vitro studies have compared traditional impression procedures with digital impression approaches, yet there remains a lack of consensus regarding the accuracy and clinical acceptibility of digital techniques. The aim of this study is the comparative evaluation of digitally and conventionally fabricated implant models in terms of accuracy and clinical acceptability. In our study two upper jaw models with three posterior teeth missing were prepared from self curing acrylic resin. Two implants were placed parallel to each other and the teeth axis in premolar and molar regions to first model and two implants were placed with the angle of 15° tilted towards to each other in premolar and molar regions to second model, completely with the aid of surgical guides. 12 impressions from each group were taken in total of 72 impressions using convenitonally open tray, snap-on methods and digitally intraoral scanner. With reference of the master models, impression models were scanned on a 3-dimensional scanner and the positional and angular measurements were made. Data were statistically analyzed using Kruskal-Wallis and Mann-Whitney U tests (p=0,05).According to the results, it can be cocluded that none of implants in master models could be transferred in 3-D to impression models. In both parallel and 15 ° angulated implants, the most precise and accurate results are the digital impression method. Then comes the open-tray method and snap-on method respectively in precision and accuration (p<0,05).Key Words: Implant impression; Impression accuracy; Digital impression; Conventional impression.

Herhangi bir protez rehabilitasyonunda başarılı bir sonuç için doğru bir ölçü ve belirgin model esastır. İmplant destekli protezlerde ölçü teknikleri, geleneksel protezlerden temel alınarak geliştirilmiştir. İmplant üstü protezlerin üretim aşamasındaki en önemli basamak ağız içindeki implant konumunu, ölçü ile üç boyutlu olarak modele aktarabilmektir. Yanlış ölçü sonrasında elde edilen uyumsuz protezler aşırı stres sonucu kemik-implant-protez arayüzlerinde biomekanik komplikasyonlara neden olabilmektedir. Osseoentegre implantların kemik içerisinde hareket etme kapasitesinin dişlere göre çok daha az olmasından dolayı implant destekli sabit bölümlü protezler diş destekli sabit bölümlü protezlere oranla protez altyapısındaki uyumsuzlukları kompanse etmede daha zayıftır. Bu yüzden implant destekli sabit bölümlü protezlerin ölçüsünün doğruluğu ve kesinliği diş destekli sabit bölümlü protezlerin ölçüsüne göre daha kritik öneme sahiptir. Dijital teknolojinin gelişmesiyle klinisyenlere geleneksel yöntemlere opsiyon olarak ağız içi tarayıcılar aracılığıyla ölçü yöntemleri sunulmuştur. Dijital ölçülerin kullanımı geleneksel ölçüde kullanılan materyalleri elimine ettiği, bunun da potansiyel olarak hem hasta konforuna katkıda bulunduğu, hem de hata payını azalttığı iddia edilmektedir. Geleneksel ölçü yöntemleriyle dijital yaklaşımları karşılaştıran birbirinden farklı in-vitro çalışmalar olmasına rağmen, dijital ölçü yöntemlerinin doğruluğu ve klinik kabul edilebilirliği konusunda bir konsensus oluşmamıştır. Bu çalışmanın gerekçesi dijital ve konvansiyonel yollarla elde edilen implant modellerinin kesinlik ve klinik kabul edilebilirlik açısından karşılaştırmalı değerlendirmesidir. Çalışmamızda posterior bölge üç diş eksikliği bulunan iki üst çene modeli otopolimerizan akrilik rezinden hazırlanmıştır. Modellerden birinde premolar ve molar bölgeye birbirine ve komşu dişlere paralel olacak şekilde iki implant, diğerinde yine premolar ve molar bölgeye birbirine doğru 15° açıyla eğimlendirilmiş şekilde iki implant tamamen cerrahi rehber plaklar kullanılarak yerleştirilmiştir. Elde edilen modellerden konvansiyonel açık kaşık ve snap-on ölçü yöntemi ile dijital ağız içi tarayıcı kullanarak her bir gruptan 12 adet toplamda 72 adet ölçü alınmıştır. Ana modeller referans alınarak elde edilen ölçü modelleri 3-boyutlu tarayıcıda taranmış, konumsal ve açısal ölçümler yapılmıştır. Elde edilen veriler Kruskal-Wallis ve Mann-Whitney U testleri ile istatistiksel olarak analiz edilmiştir (p=0,05). Elde edilen sonuçlara göre ana modellerdeki hiçbir implant ölçü modellerine 3 boyutlu olarak transfer edilememiştir. Hem paralel hem de 15° açılı implantlarda en hassas ve doğru sonuçları dijital ölçü yöntemi vermiştir.
