One lung ventilation is a widely used method in order to facilitate surgical interventions in thoracic surgery. The pathophysiological events may cause lung injury in one lung ventilation and then during the transition period to double lung ventilation. The aim of this study was to investigate the protective effect of pentoxifylline on lung injury after one lung ventilation in rats. 20 Sprague-Dawley rats were randomized into two groups in equal numbers (n=10). In control group we performed one lung ventilation via tracheostomi entubation for 60 minutes, following 30 minutes double lung ventilation. In pentoxifylline group, same ventilation procedure was performed, additionally 50 mg/kg intraperitoneal pentoxifylline was administered 10 minutes before the experiment. In control and pentoxifylline group, tissue samples of lung were taken for biochemical analysis and histopathological evaluation at the end 60th and 90th minutes. Superoxide dismutase (SOD), malondialdehyde (MDA) and Tumor necrosis factor alpha (TNF-α) levels were determined by using ELISA method. Tissue samples were stained with hematoxylin-eosin for histopathological evaluation and were scored acording to the alveolar congestion, interstitial edema, polymorphonuclear leukocytes infiltration, lymphocyte infiltration and intraalveolar hemorrhage amount. There was significant decrease in the levels of MDA and TNF- α in the group given pentoxifylline after double lung ventilation (90th minutes). The levels of SOD significantly increased in the same group. In histopathological evaluation, tissue injury in pentoxifylline group was observed lesser than control group. It seems that pentoxifylline has protective effects on lung injury especially in reexpansion period after one lung ventilation.

Tek akciğer ventilasyonu (TAV), cerrahi girişimi kolaylaştırmak amacıyla göğüs cerrahisinde sık olarak uygulanan bir yöntemdir. TAV ve daha sonra çift akciğer ventilasyonuna (ÇAV) geçiş ile birlikte gelişen patofizyolojik olaylar akciğer hasarına neden olabilir. Bu çalışmanın amacı, sıçanlarda TAV' a bağlı oluşan akciğer hasarı sonrası pentoksifilinin koruyucu rolünü araştırmaktır. 20 adet Sprague-¬Dawley cinsi sıçan randomize olarak, eşit sayıda (n=10) iki gruba ayrıldı. Kontrol grubuna trakeostomi açılmasını takiben 60 dakikalık TAV sonrası 30 dakikalık ÇAV uygulandı. Pentoksifilin grubunda, aynı ventilasyon protokolü uygulandı ve çalışmanın 10 dakika öncesinde 50 mg/kg pentoksifilin intraperitoneal olarak verildi. Kontrol ve pentoksifilin grubunda 60 dakikalık TAV sonrası ve 30 dakikalık ÇAV sonrası biyokimyasal analiz ve histopatolojik inceleme için doku örnekleri alındı. Malondialdehit (MDA), tümör nekroz faktör alfa (TNF-α) ve süperoksit dismutaz (SOD) düzeyleri ELISA methodu kullanılarak ölçüldü. Histopatolojik incelemede dokular hemotoksilen eosin ile boyandı ve akciğerlerde oluşan hasar alveoler konjesyon, interstisyel ödem, intraalveoler kanama, polimorf nüveli lökosit (PMNL) ve lenfosit infiltrasyonu varlığı ve miktarına göre skorlandı. Pentoksifilin verilen grupta ÇAV sonrası ölçülen MDA ve TNF-α ölçümlerinde istatistiksel olarak da anlamlı düşüş görüldü. SOD düzeyinde istatistiksel olarak anlamlı yükselme görüldü. Histopatolojik incelemede pentoksifilin grubunda, oluşan doku hasarının kontrol grubuna göre daha az olduğu gözlendi. Pentoksifilinin akciğer hasarı üzerinde özellikle TAV sonrası reekspansiyon döneminde koruyucu etkisi olduğunu düşünüyoruz.
