Pneumonia caused by a soil saprophyte Rhodococcus equi is considered asone of the most important infections with high mortality rate in foals worldwide.The lack of effective treatment, control and prevention measures results insignificant economical losses, and therefore urgent action is needed to overcomethis infection.In this study, nasal and fecal swab samples were collected from foalsreared at the state and private horse farms located in Malatya, Şanlıurfa, Bursa,Eskişehir and İzmit provinces where racehorse breeding is common. In addition,soil and water samples were taken from these enterprises. This study was carriedout to investigate the presence and frequency of R. equi by bacteriological andmolecular methods, and to determine genetical relationship between the fieldisolates by Pulsed Field Gel Electrophoresis (PFGE). Also, possible risk factors ofthe infection and in vitro sensitivity of the isolates to various antibiotics includingampicillin, vancomycin, erythromycin, rifampicin, clarithromycin, tetracycline,streptomycin, gentamicin were determined.Following bacteriological and molecular analyses, 10.04% (43/428) ofnasal swab samples, 22.9% (98/428) of fecal swab samples, 29.4% (6/17) of soilsamples and 5.8% (1/17) of water samples were confirmed as R.equi. Of the total147 isolates confirmed R.equi, 46.2% (68/147) were identified as virulent by the PCR using vapA gene specific primers. When the antibiotic susceptibility profilesof the isolates were examined, it was found that 43.5% (64/147) of the strainswere sensitive to all tested antibiotics while 2.72% (4/147) showed multipleresistance to three or more antibiotics. PFGE analyses of 65 virulent R.equiisolates produced 29 different genotype patterns within 12 clusters. In thestatistical analyses of farm parametres, a positive correlation was found betweenthe frequency of mechanical cleansing of paddocks and the absence of deaths dueto R. equi infection. While virulent R. equi isolates were detected in farms withprevious history of infection, the isolates obtained from farms without previousinfection history were determined to be avirulent.In conclusion, the present study revealed PFGE profiles of R. equi fieldisolates for the first time in Turkey. As a remarkable finding, the detection ofdifferent genotypes in farms where virulent R. equi isolates were obtainednecessitates comprehensive investigation of transmission sources and risk factorsof the infection in addition to elucidation of genotypic features of resistant strains.It is also required to develop immunization techniques due to the fact that immunesystems of foals are less improved when compared to horses. Bir toprak saprofiti olan Rhodococcus equi‟den kaynaklanan pnömoniler;dünya genelinde özellikle taylar için mortalite oranının yüksek olması, tedavi,korunma ve kontrol metotlarının yetersizliği ve buna bağlı olarak ekonomikdüzeyde ciddi kayıplara yol açması sebebi ile baş edilmesi gereken önemlienfeksiyonlar arasındadır.Çalışmamızda yarış atı yetiştiriciliğinin yaygın olarak yapıldığı Malatya,Şanlıurfa, Bursa, Eskişehir ve İzmit şehirlerinde bulunan kurumsal işletmelerebağlı haralarda ve özel çiftliklerdeki taylardan alınan nazal ve fekal svaplar ile buçiftliklerden toplanan toprak ve su örnekleri kullanıldı. Örneklerde R.equi‟ninvarlığının bakteriyolojik ve moleküler yöntemlerle araştırılması ve hastalığınmuhtemel risk faktörlerinin belirlenmesinin yanı sıra, elde edilen izolatlararasındaki genetik ilişkinin Pulsed Field Gel Electrophoresis (PFGE) yöntemiyleortaya konması ve izolatların çeşitli antibiyotiklere (ampisilin, vankomisin,eritromisin, rifampisin, klaritromisin, tetrasiklin, streptomisin ve gentamisin)duyarlılıklarının belirlenmesi amaçlandı.Yapılan bakteriyolojik ve moleküler analizler neticesinde nazal svapörneklerinde %10,04 (43/428), fekal svap örneklerinde %22,9 (98/428), toprakörneklerinde %29,4 (5/17), su örneklerinde %5,8 (1/17) oranlarında R.equiizolasyonu ve identifikasyonu gerçekleştirildi. R.equi olarak doğrulanan toplam147 izolatın vapA genine spesifik primerlerin kullanıldığı Polimeraz ZincirReaksiyonu (PCR) ile incelenmesi sonucunda %46,2‟si (68/147) virülent olaraktanımlandı. Saha izolatlarının antibiyotik duyarlılık profilleri incelendiğinde iseizolatların %43,5 (64/147)‟inin test edilen tüm antibiyotiklere karşı duyarlıolduğu, buna karşılık %2,72 (4/147) ‟sinin üç veya daha fazla antibiyotiğe karşıçoklu direnç gösterdiği tespit edildi. PFGE uygulanan toplam 65 adet virülentR.equi izolatında 29 farklı genotip belirlendi ve bu genotipleri içinde bulunduran12 küme saptandı. Çalışmamızda çiftlik bilgileri ile ilgili yapılan istatistikseldeğerlendirme sonucunda padokların mekanik temizliğinin yapılma sıklığı ileR.equi enfeksiyonlarından kaynaklı ölümlerin görülmemesi arasında pozitifkorelasyon bulundu. Önceki yıllarda enfeksiyonun bildirildiği çiftliklerde virülentR.equi tespit edilirken, enfeksiyon bildirilmeyen çiftliklerden elde edilenizolatların avirülent olduğu saptandı.Sonuç olarak bu çalışma ile Türkiye‟de ilk kez R.equi izolatlarının PFGEprofilleri ortaya konuldu. Özellikle virülent R.equi suşlarının izole edildiğiçiftliklerde farklı genotiplerin saptanması nedeniyle bulaş kaynakları ve riskfaktörlerinin daha kapsamlı bir şekilde araştırılmasına, dirençli suşların genotipikkarakterlerinin ayrıntılı olarak ortaya konmasına ve taylarda bağışıklık sistemininyetişkin atlara göre daha az gelişmiş olması nedeniyle immunizasyontekniklerinin geliştirilmesine ihtiyaç duyulmaktadır. Bu minvalde elde edilecekverilerin R. equi enfeksiyonlarına karşı daha etkin kontrol ve korunmastratejilerinin geliştirilmesine olanak sağlayacağı düşünülmektedir.Anahtar Kelimeler: Pnömoni, Rhodococcus equi, vapA, PFGE, PCR,antimikrobiyal duyarlılık.
