Obesity is defined as a disease that can lead to systemic, hormonal, metabolic, aesthetic, psychological and social problems. Obesity is a chronic disease which adversely effects the quality and length of life and one of the most important health problems of the developing world. Adipose tissue is not only a storage that provides accumulates the lipids, but also it is an organ that endocrine, metabolic and inflammatory signals are associated with each other. Insulin resistance of adipokines as leptin and resistin and tumor necrosis factor-alpha (TNF-α) and proinflammatory cytokines as interleukin-6 (IL-6) and due to their effects on inflammation; are thought to play an important role skin physiology and pathophysiology. The data obtained from skin and skin adds of obese and overweight people comparing with the data obtained from body mass index (BMI) < 25 kg/m2 people and the impact of obesity on the health of a person's skin is to attract attention in Elazığ and neighboring provinces. In our study, over 18 years, 510 participants were enrolled. Participants were divided into three groups according to body mass index (BMI): <25,0 kg/m2 control group, 25,0 to 29,9 kg/m2, overweight group, ≥ 30,0 kg/m2 obese group were considered. Of the cases detailed dermatological examination was performed, the data were recorded. In all groups, the most common seven dermatoses were listed as respectively; stria distensae, plantar hyperkeratosis, dystrophic cellulite, acrochordon, acantosis nigricans, varicose veins and keratosis pilaris. Compared with obese and control group, between the BMI and; striae distanse, plantar hyperkeratosis, dystrophic cellulite, acrochordon, acanthosis nigricans, varicose veins, tinea pedis, lymphedema, hirsutism, onychomycosis, hidradenitis suppurativa, infective cellulite, androgenetic alopecia in women, androgenetic alopecia in men was found a positive correlation. In the study compared obese and control groups, keratosis pilaris, and psoriasis were no significant differences between the groups. Between acne and BMI was found negative correlation. Unlike previous studies, in our study; seborrheic keratosis, senile angioma, lentigines, congenital nevi, acquired nevi and nail findings were evaluated. When the obese and control groups are compared, in the great BMI group; seborrheic keratosis, senile angioma, lentigines was significantly higher. BMI and congenital nevi, acquired nevi and nail findings compared, between groups statistically significant relationship was not observed (p>0,005).In the study, acanthosis nigricans, varicose veins, hidradenitis suppurativa, furuncles, and intertrigo, when viewed in the absence of a significant difference between male and female gender; distanse striae, acne vulgaris and keratosis pilaris in women was statistically significantly higher.For the prevention of obesity-related dermatoses people refrain from excessive weight gain has concluded that significant.
Obezite; fizyolojik, sistemik, hormonal, metabolik, estetik, psikolojik ve sosyal sorunlara yol açabilen bir hastalık olarak tanımlanmaktadır. Obezite, yaşam kalitesini ve süresini olumsuz yönde etkileyen kronik bir hastalık olup gelişen dünyanın en önemli sağlık sorunlarından biridir. Yağ dokusu, sadece lipidlerin birikmesini sağlayan bir depo değil, aynı zamanda endokrin, metabolik ve inflamatuvar sinyallerin birbirleriyle bağlantısının olduğu bir organdır. Leptin ve resistin gibi adipokinlerin ve tümör nekroz faktör-alfa (TNF-α) ve interlökin-6 (IL-6) gibi proinflamatuvar sitokinlerin insülin direnci ve inflamasyon üzerindeki etkilerinden dolayı; deri fizyolojisi ve patofizyolojisinde önemli rol oynadıkları düşünülmektedir. Bu çalışmada amaç; Elazığ ve çevre illerdeki obez ve aşırı kilolu kişilerin deri ve deri eklerinden elde edilen verileri, VKİ 25 kg/m2 altında olan bireylerden elde edilen verilerle karşılaştırarak obezitenin kişinin deri sağlığında oluşturduğu etkilere dikkat çekmektir. Çalışmaya, dermatoloji polikliniğine başvuran ve çalışmaya gönüllü olarak katılmayı kabul eden 510 kişi alındı. Çalışmaya alınan hastalar vücut kitle indeksine (VKİ) göre üç gruba ayrıldı; kontrol grubu (<25,0 kg/m2), aşırı kilolu grup (25,0-29,9 kg/m2) ve obez grup (≥30,0 kg/m2). Olguların ayrıntılı dermatolojik muayeneleri yapılarak veriler kaydedildi.Tüm gruplarda en sık görülen yedi dermatoz sırasıyla; stria distensa, plantar hiperkeratoz, distrofik selülit, akrokordon, akantozis nigrikans, variköz ven ve keratozis pilaris idi. Obez ve kontrol grubu karşılaştırıldığında VKİ ile; stria distensa, plantar hiperkeratoz, distrofik selülit, akrokordon, akantozis nigrikans, variköz ven, tinea pedis, lenfödem, hirsutizm, onikomikoz, hidradenitis süpürativa, enfektif selülit, kadınlarda androgenetik alopesi, erkeklerde androgenetik alopesi arasında pozitif korelasyon vardı. Yapılan çalışmada, obez ve kontrol grubu karşılaştırıldığında keratozis pilaris ve psöriazis için gruplar arasında anlamlı fark yoktu. Akne vulgaris ile VKİ arasında ise negatif korelasyon tespit edildi.Önceki çalışmalardan farklı olarak çalışmamızda; seboreik keratoz, senil anjiom, lentigo, konjenital nevüs, akkiz nevüs ve tırnak bulgularına bakıldı. Obez ve kontrol grubu karşılaştırıldığında; VKİ büyük olan grupta seboreik keratoz, senil anjiom ve lentigo istatistiksel olarak anlamlı derecede daha fazlaydı. VKİ ile konjenital nevüs, akkiz nevüs ve tırnak bulguları kıyaslandığında tüm gruplar arasında istatistiki olarak anlamlı ilişki gözlenmedi (p>0.05).Yapılan çalışmada akantozis nigrikans, variköz ven, hidradenitis süpürativa, fronkül ve intertrigoya bakıldığında kadın ve erkek cinsiyetler arasında anlamlı fark yokken; stria distensa, keratozis pilaris ve akne vulgaris kadınlarda istatistiksel olarak anlamlı derecede fazlaydı. Obezite ile ilişkili dermatozların önlenmesi bakımından kişilerin aşırı kilo alımından sakınmasının önemli olacağı sonucuna varıldı.
