Giriş ve Amaç : DEHB tanısı konan çocuk ve ergenlerde sigara, alkol ve madde kullanım sıklığının ve şiddetinin belirlenmesi ve DEHB tanısı konan çocuk ve gençlerde sigara alkol ve madde kullanımında bozukluğa, çocuğa ve psikososyal çevreye ilişkin etkileyici değişkenlerin araştırılması amaçlanmıştır. Yöntem: Çalışma örneklemi, Ankara Üniversitesi Tıp Fakültesi Çocuk ve Ergen Ruh Sağlığı ve Hastalıkları Anabilim Dalı?nda Dikkat Eksikliği Hiperaktivite Bozukluğu tanısıyla tedavi ve takibine devam edilmekte olan 11-16 yaş aralığındaki toplam 45 çocuk ve ergen ile 36 sağlıklı gönüllü çocuk ve ergenden oluşmaktadır. DEHB grubu 11-16 yaş arasında, DSM-IV-TR kriterlerine göre DEHB tanısı konmuş, klinik olarak normal zeka düzeyinde çocuk ve ergenler arasından seçilmiştir. Kontrol grubu, DEHB grubu ile demografik özellikler bakımından eşleştirilmiş 11-16 yaş arasında, klinik olarak normal zeka düzeyinde, DSM-IV-TR ölçütlerine göre DEHB tanısı konmayan çocuk ve ergenler arasından seçilmiştir. Araştırmaya katılan çocuklar, anne babaları ve öğretmen tarafından doldurulmak üzere Piers-Harris Çocuklar İçin Öz Kavramı Ölçeği, Güçler ve Güçlükler Anketi ebeveyn, öğretmen ve ergen formu, Yenilenmiş Conners Ebeveynve Öğretmen Derecelendirme Ölçeği, Conners-Wells Ergen Özbildirim Ölçeği, Belirti Tarama Listesi (SCL-90R) verilmiştir. Çocuklara araştırmacı tarafından, Çocuk ve Ergenlerde Sigara Alkol ve Madde Kullanımı Değerlendirme Anketi ile Okul Çağı Çocukları için Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi- Şimdi ve Yaşam Boyu Versiyonu (K-SADS-PL) uygulanmıştır.Sonuçlar: Çalışmamızda, DEHB tanısı konan çocuklar, DEHB tanısı konmayan çocuklar arasında düzenli sigara, alkol ve madde kullanımı olan çocuk ve ergen saptanmamıştır. DEHB grubunda 13 çocuğun, kontrol grubunda 10 çocuğun sigara denemesi oldu saptanmıştır.Her iki grup sigara alkol ve madde kullanım 126örüntüleri açısından karşılaştırıldıklarında da aralarında anlamlı bir farklılık bulunmamıştır.DEHB grubunda 28 olguda ,kontrol grubunda ise 12 olguda ek tanı saptanmıştır. Gruplar arasında ek tanı varlığı açısından (p=0,024) istatistiksel olarak anlamlı bir farklılık olduğu görülmüştür. DEHB grubunda en sık saptanan ek tanı ÖÖG(%39,2), kontrol grubunda ise Anksiyete Bozukluğu?dur (%41,6). DEHB grubunda ÖÖG? den sonra saptanan ek tanılar sıklık sırasıyla Anksiyete B. (%35,7), KOKG (%17,8), DB( %3,5) ve Depresyon?dur (%3,5). Kontrol grunda ise Anksiyete B.?dan sonra saptanan ek tanılar sıklık sırasıyla Tik Bozukluğu (%25), Depresyon (%8,3), KOKG(%8,3), Enürezis (%8,3), Enkoprezis(%8,3) `tir. DEHB?li çocuk ve gençlerde sigara deneyiminde yaş,baba eğitimi, haftalık harcana para miktarı, alkol deneyimi ve tedavi uyumunun etkili olduğu saptanmış. DEHB?li çocuk ve gençlerde alkol deneyiminde yaş, anne ve baba eğitimi, anne mesleği, aile alkol kullanımı ve çocuğun sigara deneyiminin etkili olduğu saptanmış. DEHB?li olmayan çocuk ve gençlerde sigara deneyiminde anne, baba eğitimi, babanın mesleği, aylık gelir,ailede sigara kullanımı ve ailede alkol kullanımının etkili olduğu, DEHB?li olmayan çocuk ve gençlerde alkol deneyiminde ise yaş, aileye göre ders başarı durumu ve ailede alkol kullanımın etkileyici olduğu saptanmıştır.Tartışma : Çalışmamızda DEHB?li çocukların sigara, alkol ve madde kullanım özellikleri kontrol grubundaki çocuklarla benzer saptanmıştır. Bu alan yazınında ki DEHB ve sigara, AMKK arasında ilişki saptamayan araştırma sonuçlarıyla uyumludur. Bu durum DEHB?de sigara, alkol ve madde kullanım bozukluklarında esas belirleyici faktörün KOKG ve Davranım Bozukluğu gibi Yıkıcı Davranım Bozuklukları komorbiditesi olduğu hipotez ve araştırma bulgularını destekler niteliktedir çünkü bizim araştırma örneklemimizde Yıkıcı Davranım Bozuklukları komorbiditesi azdı. DEHB?li çocuk ve gençlerde tedavi uyumunun kötü olması sigara deneyimde yordayıcı bir faktör olduğu görüldü.Sonuç olarak; DEHB?li çocuk ve gençlerde tedavi uyumu kötü olan olgularda tedavi uyumunu zorlaştıran faktörlerin iyi ele alınması ve direncin çözülmesi sigara alkol ve madde kullanım sorunlarıyla baş etmede önemli olacaktır. Bununla birlikte diğer yıkıcı davranım bozuklukları ve DEHB?li çocukların tedavi sürecinde bu 127sorunlara yönelik etkin ve erken müdahalelerin yapılması sigara alkol ve maddeye yönelim riskini azaltıcak ve sağaltımı etkinliği artıracaktır. Introduction And Purpose: The aim of our study was to determine the frequency and severity of tobacco, alcohol and substance abuse in children and adolescents with the diagnosis of Attention Deficit Hyperactivity disorder and to investigate impressive variables related with the disorder, child and psychosocial environment.Method: Study sample consist of 45 children and adolescents between the ages of 11-16 applied for the first time and continued to follow up in Ankara University Faculty of Medicine, Department of Child and Adolescent Psychiatry and with the diagnosis of ADHD using the DSM-IV-TR diagnostic criteria ; and 36 healthy children and adolescents. ADHD group is selected from children and adolescents between the ages of 11-16 , with the level of clinically normal intelligence and diagnosed with ADHD according to DSM-IV-TR criteria. Control group is selected from children and adolescents between the ages of 11-16 and matched with ADHD group for demographic characteristics, with the level of clinically normal intelligence and not diagnosed with ADHD according to DSM-IV-TR criteria. PiersHarris Children's Self-Concept Scale, Strengths and Difficulties Questionnaire parent, teacher and adolescent form, newly revised versions of Conners' Parent and Teacher Rating Scale, Conners-Wells Adolescent self-report Scale, the Symptom Check List (SCL-90R) are given to children , parents and teachers participating in the study. ?Assessment Questionnaire for tobacco, alcohol and substance abuse in children and adolescents? and ?Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version (K-SADSPL)? are administered to children participating in the study by the researcher. Results: In our study,there was no regular tobacco, alcohol, and substance use in children and adolescents with or without ADHD. 13 children of ADHD group 129and 10 children of the control group had been experience of smoking. Both groups were compared in terms of patterns of tobacco , alcohol and substance use , there was no statistically significant difference. Comorbidity was found in 28 cases of ADHD group and 12 cases of control group. There was a statistically significant difference(p = 0.024) between the two groups for the presence of comorbidity. The most common comorbidity in children with ADHD was specific learning disability(SLD)(%39,2). The most common comorbidity in control group was anxiety disorder (%41,6). In ADHD group , in order of frequency , comorbid diagnoses after SLD are anxiety disorder (%35,7), oppositional defiant disorder (%17,8), conduct disorder( %3,5) , and depression (%3,5). In control group, in order of frequency , comorbid diagnoses after anxiety disorder are tic disorder (%25), depression (%8,3), oppositional defiant Disorder (%8,3), enuresis (%8,3),encopresis (%8,3). Smoking experience in children and adolescents with ADHD was found to be associated with age, education level of father , the amount of money spent per week, alcohol experience, and treatment compliance. Alcohol experience in children and adolescents with ADHD was found to be associated with age, maternal and paternal education, maternal occupation, alcohol use of family, and smoking experience of the child. Smoking experience in children and adolescents without ADHD was found to be associated with paternal education, father's occupation, monthly income, family history of cigarette smoking and alcohol use in the family. Alcohol experience in children and adolescents without ADHD was found to be associated with age, success in school perceived by family , alcohol use in the family.Discussion : In our study, characteristics of tobacco, alcohol and substance use in children with ADHD were similar in control group . This finding are consistent with literature that were no relationship between ADHD and tobacco ,alcohol ,substance abuse. This supports the hypothesis that the decisive factor in tobacco, alcohol and substance use in children with ADHD is comorbidity of disruptive behavior disorders such as oppositional defiant disorder and conduct disorder. Because in our study sample , comorbidity of disruptive behavior disorders were fewer. Poor compliance to treatment in children with ADHD was found a predictive factor for smoking experience.130As a result, handling with difficulties affecting treatment compliance and resolviNg the resistance will be important in dealing with tobacco, alcohol and substance use problems in children with ADHD. However, in the treatment course of children with ADHD and other disruptive behavior disorders, early and effective interventions to these problems will reduce the risk of tobacco, alcohol and substance use and increase the effectiveness of treatment.Key Words: Attention deficit hyperactivity disorder, smoking, Alcohol and Substance Use Disorders.
