Respiratory distress syndrome and associated diseases are the most important causes of morbidity and mortality in preterm newborns. Surfactant given as prophylactically and CPAP use have been shown to be benefical in babies with or at risk of developing RDS. When administered immediately after delivery, surfactant mixes with the fetal lung fluid and reaches the alveoli before the onset of lung injury potentially created by the first applied positive pressure ventilation at delivery room. The purpose of this study was to determine the clinical effects of very early prophylactic surfactant administration with first breath.The study was designed as a prospective, randomized controlled trial in infants born before 28 weeks? gestation and infants born at 29 to 30 weeks? gestation who did not receive or complete antenatal steroid dose. Babies were randomized before delivery to receive either immediate bolus (group 1) or surfactant at 15 minutes after birth (group 2). During resuscitation and surfactant administration infants were ventilated with a T-piece device. After surfactant treatment, infants were extubated to NCPAP. The primary outcome was the need for MV within the first 5 days of life, the secondary outcomes were neonatal morbidities, duration of hospitalization and mortality.A total of 52 newborns were enrolled and 28 were randomized to group 1 and 24 to group 2. The prenatal features, mean gestational weeks and birth weights of the groups were similar. Five (18%) infants in group 1 and 7 (29%) infants in group 2 needed MV during the first 5 days (p>0.05). Median hours of both MV and NCPAP of groups were similar. The patients in group 2 received more further doses of surfactant (p<0,05). There were no significant differences between the groups with a respect to the other secondary outcomes. Mortality rates and mean duration of hospitalization were similar.Our study did not demonstrate a superiority of the immediate bolus of surfactant prophylaxis with first breath to the administration of surfactant at 15 minutes.

Respiratuvar distres sendromu ve ilişkili hastalıklar, prematüre yenidoğanlarda en önemli morbidite ve mortalite nedenidir. RDS'li veya gelişme riski yüksek olan bebeklerde profilaktik sürfaktan ve CPAP uygulamasının faydalı olduğu gösterilmiştir. Sürfaktan doğumdan hemen sonra uygulandığında, fetal akciğer sıvısına karışarak, doğum salonunda ilk PBV ile oluşabilecek akciğer hasarı başlamadan alveollere ulaşabilir. Bu çalışmanın amacı, ilk nefesle çok erken profilaktik sürfaktan uygulamasının klinik etkinliğinin değerlendirilmesidir.Bu çalışma, prospektif, randomize kontrollü olarak 28 hafta altında doğan tüm bebekler ve 29-30 hafta arası ANS almamış veya kür tamamlanmadan doğmuş bebeklerde yapılmıştır. Bebekler doğmadan önce ilk nefesle sürfaktan grubu (Grup 1) ve 15. dakikada sürfaktan uygulanan grup olarak randomize edildiler. Çalışmaya alınan tüm bebeklere resüsitasyon ve sürfaktan uygulaması T-parçalı alet aracılığıyla yapıldı, sürfaktan uygulaması sonrası tüm bebekler NCPAP'a bağlandı. Hastalar birincil olarak yaşamın ilk 5 gününde MV gereksinimi, ikincil olarak neonatal morbiditeler, hastanede yatış süresi ve mortalite açısından değerlendirildi.Toplam 52 hastanın alındığı çalışmada grup 1'e 28, grup 2'ye 24 hasta randomize edildi. Grupların doğum öncesi özellikleri, ortalama gebelik haftaları ve doğum ağırlıkları benzerdi. Grup 1'den 5 (%18) ve grup 2'den 7 (%29) hastanın yaşamın ilk 5 gününde MV gereksinimi oldu (p>0,05). Gruplardaki hem MV süreleri, hem NCPAP süreleri benzerdi. Grup 2'deki hastalarda ek sürfaktan dozu kullanımı fazlaydı (p<0,05). Her iki grup arasında ikincil sonuçlar açısından fark yoktu, hastanede yatış süreleri ve mortalite oranları benzerdi.Çalışmamızda, ilk nefesle çok erken profilaktik sürfaktan uygulamasının, 15. dakikada sürfaktan uygulamasına üstünlüğünün olmadığı görülmüştür.
