SUMMARYObjective: Emergency department (ED) is one of the indispensable parts of palliative care (PC). Although there are many studies abroad for the evaluation and treatment of PC patients in ED, it is very limited in our country. With this comprehensive study, we aimed to contribute to the integration process of PC into ED by examining the needs and symptom severities with Speed, Edmonton and Karnofsky scales by using a new screening method of patients who applied to ED and who need PC. Materials and Methods: This prospective study, which was approved by the ethics committee, included patients aged 18 years and over who applied to Gaziosmanpaşa University Medical Faculty Hospital ED between 1 December 2015 and 1 February 2017, and the study form information was recorded. The study form contained the following variables; demographic information (age, gender) and diagnosis of the patients, the duration of diagnosis, the duration of ED follow-up, the need for consultation and the result of follow-up in the ED (discharge, clinical admission, Intensive care hospitalization, death). The patients were administered the Speed, Karnofsky Performance Scale and the Edmonton Symptom Identification Scale. The analysis of the study was prepared using IBM SPSS 20 and Excel 2010 version. Frequency (f) and percent (%) values of all variables were calculated. The Kolmogrov Smirnov Test was applied to test the normality assumptions of the scales. In comparative analyzes, Kruskal Wallis-H test, Mann Whitney U test and Pearson Chi square test were used. Mann Whitney U Test was used to find out from which groups the difference was originated in case of difference in Kruskal Wallis-H test result.Findings: The study was conducted with the participation of 150 patients. Of the patients participating in the study, 44% were female and 56% were male. 6,70% of the patients were in the age range of 18-45, 35,30% in the age of 46-65, and 58,00% were in the age of 65 years. The average score of the patients' Karnofsky performance score was calculated as 43.13. The most common symptoms in the patients were fatigue, pain, anorexia and nausea respectively. The Edmonton symptom definition scale and the Karnofsky performance scale were compared. Patients with dyspnea, changes in skin and nails and symptoms of numbness in the hands were found to have lower Karnofsky scores. As a result of age-related comparisons of Edmonton's symptoms, nausea, sadness and anxiety were found most frequently in the 18-45 age range. As a comparison of patients' health status with the Edmonton symptom information, it was found that those who did not receive home health care felt tired, sad, and anxious. There was no difference in the speed performance of the patients according to gender, age, time of diagnosis. However, the Speed performances of the patients who were observed to have health care at home were higher.Conclusions: This study was the first study to define the patient group who apply an ED and who should receive PC and to determine the current status and symptoms of this group of patients with scales. PC is a maintenance that should not be ignored in the current health system and whose benefits are clearly demonstrated. It is not possible for emergency services alone to undertake such care, but it is not possible to exclude emergency services from this care. Although PB systems are not established in our country; as ED, it is again our responsibility to provide the best care for this patient group. Any comprehensive work to be done in this regard will be of great value in terms of the PC structuring in our country.

ÖZETAmaç: Acil servis (AS), palyatif bakımın (PB) vazgeçilmez parçalarından biridir. PB hastalarının AS'te değerlendirme ve tedavisinin eksiksiz yapılabilmesi adına yurt dışında birçok çalışma mevcut olmakla beraber ülkemizde bu alanda yapılmış olan çalışmalar oldukça sınırlıdır. Biz bu kapsamlı çalışma ile AS'e başvuran ve PB ihtiyacı olan hastaları yeni bir tarama metodu ile belirleyip, ihtiyaçlarını ve semptom şiddetlerini Speed, Edmonton ve Karnofsky skalaları ile inceleyerek PB'ın AS'e entegrasyonu sürecine katkıda bulunmayı amaçladık.Materyal- metod: Etik kurul izni alnmış olan bu prospektif çalışmamıza 1 Aralık 2015 ile 1 Şubat 2017 tarihleri arasında Gaziosmanpaşa Üniversitesi Tıp Fakültesi Hastanesi AS'ine başvuran, 18 yaş ve üzeri hastalar dahil edildi ve çalışma formu bilgileri kaydedildi. Çalışma formu belirtilen şu değişkenleri içeriyordu; hastaların demografik bilgileri (yaşı, cinsiyeti), tanı süresi, PB takibi olup olmadığı, evde bakım hizmeti alıp almadığı, hastalığı sebebiyle maddi devlet desteği alıp almadığı, AS takip süresi, konsültasyon istenip istenmediği ve AS'teki takibinin sonuçlanma şekli (taburculuk, klinik yatış, yoğun bakım yatış, vefat). Hastalara Speed, Karnofsky Performans Skalası ve Edmonton Semptom Tanımlama Ölçeği uygulandı. Çalışmanın analizleri IBM SPSS 20 ve Excel 2010 versiyonu kullanılarak hazırlandı. Tüm değişkenlerin frekans (f) ve yüzde (%) değerleri hesaplandı. Ölçeklerin normallik varsayımlarının test edilebilmesi için Kolmogrov Smirnov Testi uygulandı. Karşılaştırmalı analizlerde Kruskal Wallis-H Testi, Mann Whitney U Testi ve Pearson Chi Square Testi kullanıldı. Kruskal Wallis-H Testi sonucunda farklılık gözlenmesi durumunda farklılığın hangi gruplardan kaynaklandığının bulunması için Mann Whitney U Testi uygulandı.Bulgular: Çalışma 150 hastanın katılımı ile gerçekleştirildi. Çalışmaya katılan hastaların %44'ü kadın, %56'sı erkekti. Hastaların % 6,70'i 18-45, %35,30'u 46-65 yaş aralığında, % 58,00 'ı ise 65 yaş üzerindeydi. Hastaların Karnofsky performans skalasından aldıkları puanların ortalaması 43,13 olarak hesaplandı. Hastalarda en sık karşılaşılan semptomların sırasıyla yorgunluk, ağrı, iştahsızlık ve bulantı olduğu görüldü. Edmonton semptom tanımlama ölçeği ile Karnofsky performans skalası kıyaslandı. Dispne, ciltte ve tırnaklarda değişiklik ve ellerde uyuşma semptomları olan hastaların Karnofsky puanları daha düşük bulundu. Edmonton semptom bilgilerinin yaşlara göre karşılaştırılması sonucunda hastaların bulantı, üzüntü ve endişe hissinin en sık 18-45 yaş aralığında görüldüğü saptandı. Hastaların Edmonton semptom bilgilerinin evde sağlık hizmeti alıp almamalarına göre karşılaştırılması sonucunda, evde sağlık hizmeti almayanların kendilerini daha yorgun, üzgün, ve endişeli hissettikleri belirlendi. Hastaların Speed performanslarının cinsiyet, yaş, tanı alınan zaman açısından karşılaştırılmasına göre herhangi bir farklılık bulunmadı. Ancak, evde sağlık hizmeti aldığı gözlemlenen hastaların Speed performanslarının daha yüksek bulundu.Sonuçlar: Çalışmamız AS'e başvuran ve PB alması gereken hasta grubunun tanımlanması ve bu hasta grubunun mevcut durum ve semptomlarının ölçeklerle belirlenip değerlendirilmesinin yapıldığı ilk çalışmadır. PB günümüz sağlık sistemi içerisinde göz ardı edilmemesi gereken ve faydaları açıkça ortaya konan bir bakımdır. Tek başına AS'lerin bu bakımı üstlenmesi mümkün olmamakla beraber AS'leri bu bakımın dışında tutmak da mümkün değildir. Ülkemizde PB sistemleri yerleşmiş olmasa da; AS'ler olarak bu hasta grubuna en iyi bakımı sağlamak yine bizim sorumluluğumuzdadır. Bu konuda yapılacak kapsamlı her çalışma PB yapılanmasının ülkemizde yerleşmesi bakımından oldukça değerli olacaktır.
