The aim of our study was to determine the differences in the metabolite ratios ofnormal appearing white matter between lung cancer patients who investigated at thebeginning and after 3 months of the diagnosis, without parenchymal metastatic lesions,and healthy normals with magnetic resonance spectroscopy (MRS).A total of 41consecutive patients (39 males, 2 females) having diagnosis of lungcancer were referred to deparment of radiology from department of chest medicine forimaging with computed tomography (CT) of lung and magnetic resonance imaging(MRI) of brain between September 2010 and June 2011. During study 6 patients weredied and other 6 were not participated to follow up scans. As a result, 29 patients (27males, 2 females, age range: 44-80 years, mean age: 65.8 ± 9.4) and 32 healthyvolunteers (29 males, 3 females, age range: 42-80 years, mean age: 62.8 ± 8.0) wereincluded in our study. MRI was performed on a 1.5 Tesla system (Gyroscan InteraMaster, Philips Medical Systems, Best, Netherlands). Axial T1 and T2, coronal FLAIRimages for brain were acquired. The patients having the diagnosis of lung cancerwithout cranial metastasis were imagined with single voxel MRS from the leftcerebellar and parietooccipital white matter. MRS was performed at the beginning andafter 3 months of the lung cancer diagnosis. Single voxel MRS was evaluated by using apoint-resolved spectroscopy sequence (PRESS) (TR: 2000/TE:136 ms) with 128averages; voxel sizes of 17x17x17 mm were used. Voxels were placed in the leftcerebellar and parietooccipital white matter. Peak area metabolite ratios of Nacetylaspartate(NAA)/ Creatinine (Cr), NAA/ Choline (Cho) and Cho/Cr werecalculated and compared between patients and healthy normalsThere were no significant differences between control group and patients at 3 months ofthe lung cancer and between patients at the beginning and after 3 months of thediagnosis in NAA/Cr, NAA/Cho and Cho/Cr ratios obtained from left cerebellar andparietooccipital white matters. There were also no significant differences betweenpatients at the beginning of the diagnosis and control groups in aferomentionedmetabolites obtained from mentioned white matters except the Cho/Cr ratio wassignificantly different obtained from cerebellar white matter (p=0.09).As a result, we have not found metabolic changes in the normal appearing whitematter of lung cancer patients during 3 months after diagnosis. These findings couldindicate no neuronal damage and affected metabolism of cell membrane.

Çalışmamızda, kontrol grubu olgularla konvansiyonel manyetik rezonansgörüntülemeyle (MRG) kraniyal metastaz saptanmayan akciğer kanserli olgularda ilktanı ve 3 ay sonrası dönemlerde manyetik rezonans spektroskopi (MRS) ile kraniyaldeğişikliklerin olup olmadığının saptanması amaçlandı.Çalışmamızda Eylül 2010- Haziran 2011 tarihleri arasında İnönü ÜniversitesiGöğüs Hastalıkları Anabilim Dalında akciger kanseri tanısı alan 41 hasta (39 erkek, 2kadın) yer aldı. Çalışma süresince, 41 hastanın 6'sı ex, 6'sı da ilk değerlendirme sonrasıtakip tetkiklerine gelmediğinden toplam 29 hasta ile çalışma tamamlandı. Yaşortalaması 65.7 ± 9.4 (44-80) dü. Çalışmamızda 29'u erkek, 3'ü kadın 32 sağlıklıbireyden oluşan kontrol grubu mevcuttu. Kontrol grubunun yaş ortalaması 62.9 ± 8 (42-80)di. İnönü Üniversitesi Tıp Fakültesi Radyoloji Anabilim Dalında, MRS öncesinderutin konvansiyonel MRG kesitleri alındı (Gyroscan Intera Master, Philips, Best,Hollanda). Aksiyel planda T1A (TR 450 msn, TE: 10 msn), aksiyel T2A (TR 4366 msn,TE: 120 msn), koronal FLAIR (TR 6000 msn, TE:110 msn, TI: 2000), sagital T2A (TR4366 msn, TE:120 msn) görüntüler elde edildi. Konvansiyonel MRG ile kraniyalmetastaz saptanmayan hastaların ilk tanı ve 3 ay sonrası normal görünümlü solpariyetooksipital ve sol serebellar beyaz cevherden PRESS (TR2000/TE136msn) ve 128ortalama ile tek voksel ROI (17x 17 x 17 mm) yerleştirilerek MRS ile elde edilen Nasetil aspartat (NAA)/ Kreatin (Cr), NAA/ Kolin (Cho) ve Cho/Cr oranları kontrolgrubu ile karşılaştırıldı.Kontrol grubu ile akciğer kanserli hastaların ilk tanı anında serebellum için yapılanMRS degerlerinde NAA/Cr için p degeri 0.70, NAA/Cho için p=0.14, Cho/Cr için p=0.03 pariyetooksipitalde NAA/Cr için p=0.68, NAA/Cho için p=0.28, Cho/Cr içinp=0.09 bulundu. Kontrol ve akciğer kanserli hastaların 3 ay sonraki MRSdeğerlendirilmesinde, serebellumda NAA/Cr için p=0.3, NAA/Cho için p=0.87, Cho/Criçin p=0.09, pariyetooksipitalde NAA/Cr için p=0.12, NAA/Cho için p=0.71, Cho/Criçin p=0.6'dir. İstatiksel olarak anlamlı farklılık, ilk tanı ile kontrol grubu serebellarbölge Cho/Cr değerinde saptanmıştır. Hasta grubu içerisinde ilk tanı ve 3 ay sonrasıserebellumda NAA/Cr için p=0.43, NAA/Cho için p=0.07, Cho/Cr için p=0.57,pariyetooksipitalde NAA/Cr için p=0.15, NAA/Cho için p=0.39, Cho/Cr için p =0.31olup hiçbirinde istatistiksel olarak anlamlı farklılık saptanmamıştır.Akciğer kanseri tanılı ve standard kraniyal MRG ile metastatik tutulumsaptanmayan olgularda, ilk tanı ve 3 ay sonrasında yapılan MRS ile elde edilenNAA/Cr, NAA/Cho ve Cho/Cr oranlarının istatistiksel olarak anlamlılık göstermemesi,her ne kadar NAA/Cr oranı kontrol grubundan 3 aylık dönem verilerine doğru rakamsalazalma ve Cho/Cr oranları ilk tanı ve 3 aylık dönemde rakamsal artış gösterse de,nöronal hasarın ve hücre zarı metabolizmasında etkilenimin olmadığını göstermektedir.
