There is a debate on ongoing nitrous oxide (N2O) usage; in fact there is no specific indication for its use and N2O free anesthesia is easy to administer and ecologic. These all provide compelling reasons to question the continued use of nitrous oxide as a carrier gas in anesthesia. In this prospective randomized study, we aimed to improve low flow anesthesia practice and to compare the effects of different remifentanil doses in air/oxygen mixture with nitrous oxide/oxygen mixture on homodynamic, recovery times, volatile anesthetic consumption and costs in BIS controlled low flow sevoflurane anesthesia.Forty-five ASA I-II unpremedicated women scheduled for abdominal hysterectomy or myomectomy were randomly divided into three groups; Group N, Group RI and Group RII. 2 mg kg-1 propofol, 0.6 mg kg-1 rocuronium were administered at induction. In remifentanil groups following 0.5 µg kg-1 bolus remifentanil 0,2 µg kg-1 dk-1 for Group RI and 0,05 µg kg-1 dk-1 for Group RII were infused. After tracheal intubation, 50 % O2 with N2O (in Group N) or medical air (in Group RI, RII) in 4 L dk-1 fresh gas flow with 2 % sevoflurane volume was started. Following a five minute high flow period fresh gas flow was reduced to 1 L dk-1 and sevoflurane was increased to 2.5 % volume. Opioid infusions were constant as sevoflurane was adjusted to maintain BIS 50 ? 10. Hemodynamic parameters and SpO2, BİS, ETCO2, FiO2, FetO2, % sevoflurane volume, Fisevo, Fetsevo, MAC, for Group N FiN2O, FetN2O, time for extubation and eye opening, complications and treatments, recovery room findings were recorded. Consumption and costs for sevoflurane were calculated with Dion Formula represented as per person per minute.For all recording times, FiO2 was statistically greater in Group N. The difference between % O2 and FiO2 was the lowest in Group N, at the 90th minute of low flow period in Group RII this difference was the greatest compared with the other groups. In Group RI and RII heart rate and mean arterial pressure values were lower than Group N. There was no significant difference in sevoflurane consumption and costs per patient per minute between groups. Aldrete Recovery Scale was greatest in Group RII at postoperative 15th minute.In conclusion, in remifentanil groups with medical air-oxygen as carrier gas lower FiO2 values revealed that medical air is not safer than N2O when used in low fresh gas flows. Special regards with volatile anesthetic consumption and prevention of hypoxia no superiority was shown with N2O free low flow anesthesia. Monitoring FiO2 is essential in both air/oxygen or N2O/oxygen mixtures; both are safe to administer unless FiO2 is lower than %30. With appropriate remifentanil doses air/oxygen provided better hemodynamia without increasing sevoflurane consumption. Especially BIS controlled sevoflurane with its low solubility feature was successful in adapting quickly to variable anesthetic depth levels.

Azot protoksit kullanımına karşı çok sayıda tartışmanın varlığı, kullanımı için onaylanmış kesin bir endikasyonunun olmayışı ve azot protoksitsiz inhalasyon anestezisi uygulamanın kolay ve ekolojik oluşu nedeniyle bu gazın halen rutin taşıyıcı gaz olarak kullanımının sorgulanmasına yol açmıştır. Bu randomize prospektif çalışmada, düşük akımlı anestezi pratiğini arttırmak, sevofluran vaporizatör ayarlarının BIS ölçümleriyle sağlandığı farklı dozlarda remifentanil yanında hava/oksijen veya azot protoksit/oksijen ile düşük akımlı anestezi uygulamalarının hemodinamik parametreler, derlenme zamanları, volatil anestezik tüketimi üzerine etkileri ve maliyetlerinin karşılaştırılması amaçlanmaktadır.ASA I-II risk grubunda, abdominal histerektomi veya miyomektomi planlanan 45 olgu premedikasyon uygulanmadan azot protoksit (Grup N), yüksek doz remifentanil (Grup RI) ve düşük doz remifentanil (Grup RII) olmak üzere rastgele üç eşit gruba ayrıldı. Anestezi indüksiyonunda her üç gruba da 2 mg kg-1 propofol, 0.6 mg.kg-1 rokuronyum uygulandı. Remifentanil gruplarına 0.5 µg kg-1 remifentanil yüklemesini takiben Grup RI için 0.2 µg kg-1 dk-1 , Grup RII için 0.05 µg kg-1dk-1 infüzyona başlandı. Endotrakeal entübasyonun ardından, 4 L dk-1 % 50 O2/N2O (Grup N) veya hava (Grup RI-RII) karışımı içinde sevofluran % 2 volüm olarak başlandı. 5 dakikalık yüksek akımı takiben taze gaz akımı 1 L dk-1 düzeyine düşürülerek sevofluran % 2.5 volüm değerine yükseltildi. Remifentanil infüzyon hızı sabit tutulurken, sevofluran düzeyi BIS 50 ? 10 olacak şekilde ayarlandı. Hemodinamik parametreler ve SpO2 , BİS, etCO2, FiO2, FetO2, grup N2O için FiN2O, FetN2O, % sevofluran volümü, FiSevo, FetSevo, MAK değerleri, ekstübasyon ve spontan göz açma zamanları, oluşan komplikasyonlar ve tedavileri, derlenme odası bulguları kaydedildi. Maliyet hesaplaması Dion Formülü uygulanarak toplam ve hasta başına dakikada kullanılan sevofluran miktarı ve maliyeti hesaplandı.Tüm zamanlarda Grup N FiO2 değerleri diğer iki gruba göre istatistiksel olarak yüksek bulundu (p<0,05), % O2 ile FiO2 arasındaki fark en az azot protoksit grubunda saptanırken düşük akımın 90. dakikasında yüksek doz remifentanil grubunda diğer gruplara kıyasla fark anlamlı olarak fazlaydı. Grup RI ve RII'de KAH ve OAB değerlerinin Grup N'den daha düşük olduğu gözlendi. Hasta başına dakikada kullanılan sevofluran miktarı ve maliyeti hesaplandığında gruplar arasında istatistiksel fark bulunmadı. Aldrete Derlenme Skoru 15. dakikada Grup RII'de diğer gruplara göre anlamlı yüksek bulundu.Sonuç olarak; taşıyıcı gaz olarak hava-oksijen kullanılan remifentanil gruplarında FiO2'nin daha düşük seyretmesi, azot protoksit yerine hava kullanılmasının daha güvenli olmadığını gösterdi. Uygun dozda remifentanil ile hava oksijenin, azot protoksit grubuna göre daha iyi hemodinami sağladığı, volatil anestezik tüketimini arttırmadığı, her iki yöntemde de FiO2'nin dikkatle izlenmesi ve % 30 altına düşürülmemesi şartıyla güvenle kullanılabileceği, özellikle sevofluran gibi düşük çözünürlüğe sahip bir volatil anesteziğin titre edilerek BIS kontrolüyle uygulandığında değişen anestezi derinliklerine hızla uyum sağlayabileceği gösterilmiştir.
