Purpose: The aim of this study was to define a sutureless peripheral nerve repair technique using a vein graft and compare it to epineural repair with or without wrapped vein. The vein grafts were also used as a reservoir for bone marrow-derived stem cells (BMSC) and their effects on primary nerve repair was investigated.Methods: Thirty Wistar Albino rats were randomly divided into five groups. The sciatic nerve was transected. In the first group (K), epineural repair was performed. In the second group (SV), an epineural repair was wrapped with a vein graft. The third group (MSV) were treated like SV group and BMSC was injected into the vein. In the fourth group (V), sutureless repair with vein graft was performed. The fifth group (MV) was treated like V group and BMSC was injected. Functional recovery was evaluated with pinprick, toe spread tests and sciatic nerve index (SFI). Nerve regeneration was also assessed by gastrocnemius index and immunohistochemical analysis.Results: There was no difference in pinprick and toe spread tests between the groups. SFI revealed no difference at 4th and 8th weeks, however, at the 12th week all other groups showed better improvement compared to epineural repair where the MSV revealed the best (p<0.05). (K: -76.5±3.7, SV: -65.2±11.7, MSV: -46.2±19.4, V: -68.8±9.8, MV: -56±8.8) There was no difference in gastrocnemius index between the groups. Better immunoreaction in GAP-43 and NF-H staining was observed in MSV, MV, SV, V groups, respectively (p<0.05). MSV group showed less fibrosis and better alignment compared to other groups.Conclusions: Although sutureless repair with a vein graft reveals better functional results compared to epineural repair, epineural suture repair before vein wrapping improves functional and immunohistochemical results and decreases fibrosis. BMSCs also improves regeneration in primary peripheral nerve repair.
Amaç: Bu çalışmanın amacı ven grefti kullanılarak sütürsüz periferik sinir tamir tekniği tanımlayarak bu tekniği epinöral sinir tamirleriyle kıyaslamaktı. Ayrıca ven grefti kemik iliği kaynaklı mezenkimal kök hücreler (BMSC) için rezervuar olarak kullanılarak primer sinir tamirlerindeki etkilerinin incelenmesi amaçlandı.Metod: Otuz Wistar Albino sıçan rastgele beş gruba ayrıldı. Siyatik sinir kesildi. İlk grupta (K) epinöral tamir uygulandı. İkinci grupta (SV) epinöral tamir üzerine ven grefti sarıldı. Üçüncü grup (MSV), SV grubu gibi tamir edilerek ven içine BMSC enjekte edildi. Dördüncü grupta (V) ven grefti ile sütürsüz tamir uygulandı. Beşinci grup (MV), dördüncü grup gibi tamir edilerek ven içine BMSC enjekte edildi. Fonksiyonel iyileşme pinprick, toespread testleri ve siyatik fonksiyon indeksi (SFI) ile değerlendirildi. Sinir rejenerasyonu aynı zamanda gastroknemius kas indeksi ve immunohistokimyasal analizle değerlendirildi.Bulgular: Pinprick ve toespread testleri açısından gruplar arasında fark izlenmedi. SFI 4 ve 8. haftalarda fark göstermezken, 12. haftada tüm diğer gruplardaepinöral tamire göre daha iyi sonuçlar izlendi ve en iyi sonuç MSV grubunda görüldü (p<0.05). (SFI, K: -76.5±3.7, SV: -65.2±11.7, MSV: -46.2±19.4, V: -68.8±9.8, MV: -56±8.8) Gruplar arasında gastroknemius kas indeksinde bir fark yoktu. GAP-43 ve NF-H boyamalarında en iyi immunreaktivite sırasıyla MSV, MV , SV, V gruplarında izlendi (p<0.05). MSV grubu diğer gruplara göre daha az fibrozis ve daha iyi hizalanma gösterdi.Sonuçlar: Ven grefti ile sütürsüz teknik, epinöral tamir ile kıyaslandığında daha iyi fonksiyonel sonuçlar göstermesine rağmen, ven grefti ile sarılmadan önce yapılan epinöral sütürle tamirin fonksiyonel ve immunohistokimyasal sonuçları iyileştirip fibrozisi azalttığı izlendi. BMSC uygulamasının da primer sinir tamirlerinde rejenerasyonu artırdığı saptandı.
