Abstract

Introduction and Aim: Rib fractures are the most common injuries in blunt thoracic trauma. The use of computed tomography, which is considered as a reference in the diagnosis, is questioned for accessibility, radiation exposure and high cost. At this point, the ultrasound comes to mind in emergency conditions with fast, cheaper and radiation free nature. İn this study we aim to determine the value of sonography in terms of rib fracture eveluation and to compare it with computed tomography.Material and Methods: In this study, the diagnostic value of ultrasound which performed by emergency physician for rib fractue in patient with symptomatic thorax trauma who applied to the Marmara Üniversitesi Tıp Fakültesi Pendik Training and Research Hospital for a certain period was evaluated in the thorax CT reference. Results and Conclusion: A total of 484 ribs were examined sonographically in 122 patients in our study. The sensitivity of ultrasonography was 87,4% and specificity 86.27% in CT reference. With these results it can be said that emergency ultrasound is almost as competent, valuable and successful as CT in the case of rib fractures diagnosis.

Giriş ve Amaç: Kot kırıkları künt toraks travmalarında en sık rastlanan yaralanmalardır. Tanıda referans kabul edilen biligisayarlı tomografi kullanımı, ulaşılabilirlik, radyasyon maruziyeti ve yüksek maliyet sebebiyle sorgulanmaktadır. Bu noktada hızlı, ekonomik ve radyasyonsuz oluşuyla acil servislerde yerini alan ultrason akla gelmektedir. Çalışmamızda sonografinin kot kırığı tanısında değerliliğini saptamayı ve bilgisayarlı tomografi ile karşılaştırmayı amaçlamaktayız.Gereç ve Yöntem: Bu çalışmada belirli bir sürede Marmara Üniversitesi Tıp Fakültesi Pendik Eğitim ve Araştırma hastanesine başvuran semptomatik toraks travmalı hastalarda kot kırığı açısıdan acil tıp hekimince yapılan ultrasonun tanısal değerliliği toraks BT referansıında değerlendirilmiştir.Bulgular ve Sonuç: Çalışmamızda 122 hastada toplamda 484 kot sonografik olarak incelenmiştir. BT referansında ultrasonun duyarlılığı %87,4, özgüllüğü %86,27 saptanmıştır. Bu sonuçlar ışında acil serviste kot kırığı tanısında ultrasınun neredeyse BT kadar yetkin, değerli ve başarılı olduğu söylenebilir.
