We have included 60 patients in our study with moderate COPD diagnosed according to the GOLD 2009 criteria, admited at the Istanbul University Cerrahpasa Medical Faculty Pulmonary Diseases Department between March 2009 and December 2009. Patients were stable during the last four weeks and were evaluated prospectively. Of the 60 patients, 30 (%50) had moderate, 20 (%33) had severe, and 10 (%16,7) had very severe COPD. Ten patients (16.7%) were currently using USOT and 5 patients (% 0,83) were using NIMV.Mean FEV1, pO2 and pCO2 values were 1,3±0,5 (%51±15), 71,3±9,7 and 39,8±4,9 respectively. Dyspne index was 2,2±1,07 according to MMRC, and was 2,68±2,51 according to Borg scala. For anxiety and depression evaluation HAD and BECK measurements were as follows 8,7±1, 7,3±4,5 and 17,3±9,8 respectively. HAD revealed %41,7 anxiety, and %46,72 depression of the patients while BECK revealed depression in %50 of the patients. SGRQ revealed a symptom score of 49,06±19,12, an activity scrore of 59,78±22,05, influence score of 34,99±20,90, and a total score of 44,79±18,02.There was no correlation between hospital admission, activity and total score. There was no correlation between disease severity and yearly execarbation rate. Activity score showed a negative correlation with FEV1, FVC, pO2 and SaO2. MMRC had a correlation with symptom, activity, and total score. There was significant correlartion between BORG and all scores. 6MWD showed correlation with all scores. BODE index correlated with symptom, activity and total score.Anxiety showed correlation COPD severity, FEV1 6MWD, MMRC BORG scala, BMI and BODE index.According to HAD, tehere was a correlation between depression and disease stage, length of hospital stay, FEV1, FVC, pO2, SaO2, USOT and NIMV. BECK depression scala showed correlation with FEV1, FVC, USOT. NIMV, 6MWD, MMRC, BORG scala, BODE index, and annual exacerbation.In COPD patients quality of life may decrease because of dyspnea, limitation of exercise capacity, and perception of well being. Anxiety and depression are the most frequent psychiatric disorders. Physical symptoms due to chronic disease along with social isolation causes depression. In return, depression decreases the strength of struggling with the disease. Quality of life detoriates with depression. The aim of treatment in COPD is to decrease the symtoms, to increase exercise capacity, and quality of life. Dyspnea may show great variation even in patients having the same level of pulmonary function. Consequently, it is thought that staging of COPD should be done according to the dyspne staging along with pulmonary function testing. Questionnare evaluating the quality of life are important because they reflect patient symptoms and their effects on patients? daily life. Clinicians should consider the presence of anxiety and depression in COPD patients before treating this symptom and prefer a multi diciplinary approach for dyspnea treatment.

Çalışmamıza GOLD 2009 rehberine göre en az orta şiddette KOAH tanısı almış olan ve son 4 haftadır stabil dönemdeki 60 hasta prospektif olarak çalışmaya dahil edilmiştir. 30 hastanın (%50) orta KOAH, 30 hastanın (%50) ağır KOAH olduğu, ortalama FEV1 değerlerinin ise 1,3±0,5 (%51±15) olduğu görülmüştür. Hastalara yapılan hastane anksiyete ve depresyon (HAD) ölçeğine göre hastaların %41,7'sinde anksiyete varlığı, %46,72'sinde depresyon varlığı, BECK ölçeğine göre ise %50 depresyon varlığı olduğu gösterildi. Yaşam kalitesini değerlendirmek için yapılan Saint George Respiratory Questionnare'nin (SGRQ) aktivite skorunun FEV1, FVC, pO2 ve SaO2 ile negatif korelasyon gösterdiği gösterilmiştir. Dispne skalalarından MMRC ile semptom, aktivite ve total skor arasında, BORG skalası ve egzersiz kapasitesini belirlemek amacıyla yapılan 6DYT mesafesiyle tüm skorlar arasında, BODE indeksiyle semptom, aktivite ve total skor arasında korelasyon olduğu belirlenmiştir. KOAH hastalarında anksiyetenin KOAH derecesi, FEV1, 6DYT, MMRC ve BORG skalasıyla, BMİ, BODE indeksiyle ilişkili olduğu, depresyonun ise hastalık evresi, hastanede yatış, FEV1, FVC, pO2, pCO2, USOT ve NIMV kullanımı, 6DYT, MMRC, BORG skalası, BMİ, BODE indeksi ile korelasyon gösterdiği saptanmıştır.KOAH'ta anksiyete ve depresyon en yaygın görülen ruhsal bozukluklardır. Kronik hastalığın yarattığı fiziksel semptomlar ve sosyal izolasyon depresif duygulanıma yol açarken, depresyonda hastalıkla mücadele etme gücünü azaltır ve yaşam kalitesi, hastalığa eşlik eden depresyonla daha da bozulur. KOAH evrelemesinin solunum fonksiyonlarının yanı sıra, dispne derecelendirmesine göre yapılmasının, hastanın ve hastalığın daha iyi değerlendirilmesine olanak sağlayacağı düşünülmektedir. Hastalardaki dispne yakınmasını hastalığa bağlamadan önce hastada KOAH'ta sık görülen anksiyete ve depresyon varlığıda dikkate alınmalı, hastalığın gidişini etkileyen bu durumlarla ilgili multidisipliner yaklaşım ihmal edilmemelidir.
