Objective: It was aimed to compare different radiotherapy techniques, according to the thyroid gland doses (TGD). Conventional radiotherapy (CRT), three dimensional conformal radiotherapy (3DCRT) and intensity modulated radiotherapy (IMRT) were the techniques used for this comparison in nasopharyngeal and oropharynx cancer patients, whose thyroid gland was located in radiotherapy volume.Materials and Methods: The study group, including 10 patients with nasopharyngeal and oropharynx cancer, was found from the archive material of DEU Radiation Oncology Clinic. Computerized Tomography (CT) images were used to prepare four different treatment plans virtually with the techniques of Conventional Radiotherapy without midline block at anterior field (CRT I) and with midline block (CRT II), Three Dimensional Conformal Radiotherapy (3DCRT) and Intensity Modulated Radiotherapy (IMRT), by using Treatment Planning System (TPS - Nucletron, Oncentra MasterPlan version 3.3 SP3). The minimum, maximum and the average doses of target volumes and thyroid gland were calculated using Dose Volume Histogram (DVH), which was created for each plan. The Wilcoxon signed rank test was used for statistical analyses by using SPSS 15.0 statistical analysis program. The p value, less than 0.05, was accepted for statistical significance.Results : As a result of the binary comparison of planning techniques according to TGDmin values, the doses in CRT I were significantly higher than CRT II (p=0.005) and 3DCRT (p=0.007); and the doses in CRT II were significantly lower than 3DCRT (p=0.028) and IMRT (p=0.028). When the binary comparison was done according to the TGDmax values, the doses of IMRT techniques were found significantly higher than the doses of 3DCRT (p=0.007). When the planning techniques were compared according to the TGDavg, which were more critical about late effects; the significant difference was found between CRT I and CRT II (p=0.005), CRT I and 3DCRT (p=0.005). For all patients when the average values of TGDavg were analised according to planning techniques, the highest and the lowest values were found in CRT I and CRT II, respectively. It was observed that the average value of TGDavg of 3DCRT was lower than IMRT.Conclusion: More uniform dose distribution in target volumes and lower doses in thyroid gland were obtained with advanced techniques as 3DCRT and IMRT. If the CRT technique is used for treatment, using midline block to minimized the doses of thyroid gland, larynx and spinal cord should be preferred. Amaç: Bu çalışmada tiroid bezinin radyoterapi hacmi içerisinde yer aldığı orofarenks ve nazofarenks kanserli hastalarda Konvansiyonel Radyoterapi (KRT), 3 Boyutlu Konformal Radyoterapi (3BKRT) ve Yoğunluk Ayarlı Radyoterapi (YART) tekniklerinin tiroid bezi dozları (TBD) açısından karşılaştırılması amaçlanmıştır.Gereç ve Yöntem: Çalışma grubunu oluşturan orofarenks ve nazofarenks kanserli 10 hasta DEÜ Radyasyon Onkolojisi kliniği arşiv materyali taraması sonucunda retrospektif olarak bulundu. Hastaların var olan bilgisayarlı tomografi görüntülerinden tedavi planlama sistemi (TPS - Nucletron, Oncentra MasterPlan version 3.3 SP3) kullanılarak KRT ile ön alanda orta hat korumasız (KRT I) ve korumalı (KRT II) olmak üzere iki, 3BKRT (5-7 alan) ve YART teknikleriyle birer olmak üzere toplam dört farklı tedavi planı sanal olarak oluşturuldu. Hedef völümlerin ve tiroid bezinin minimum, maksimum ve ortalama dozları her bir plan için oluşturulan Doz Volüm Histogram (DVH) yardımıyla hesaplandı. Elde edilen veriler SPSS 15.0 istatistiksel analiz programında Wilcoxon signed rank test kullanılarak analiz edildi. İstatistiksel anlamlılık için p değerinin 0.05'ten küçük olması gerekliliği kabul edildi.Bulgular: Planlama yöntemlerinin TBDmin değerleri açısından ikili karşılaştırması sonucunda KRT I'deki dozlar KRT II (p=0.005) ve 3BKRT (p=0.007)'den anlamlı olarak daha yüksek; KRT II'deki TBDmin'ları ise 3BKRT (p=0.028) ve YART (p=0.028)'dan anlamlı daha düşük bulundu. TBDmax dozları açısından yapılan ikili karşılaştırmada YART yöntemindeki dozlar 3BKRT'ye göre anlamlı yüksek saptandı (p=0.007). Geç yan etkiler açısından daha önemli olan TBDavg dozları açısından planlama yöntemlerinin değerlendirilmesinde ise KRT I ile KRT II (p=0.005) ve KRT I ile 3BKRT (p=0.005) arasında anlamlı fark görüldü. Tüm olguların ortalama TBDavg değerleri planlama tekniklerine göre incelendiğinde ise en yüksek değer KRT I yönteminde bulunurken, en düşük değer KRT II yönteminde bulundu. 3BKRT' de ortalama TBDavg değerinin YART'ye göre daha düşük olduğu gözlendi.Sonuç: 3BKRT ve YART gibi gelişmiş radyoterapi teknikleri KRT tekniği ile kıyaslandığında hedef hacimlerde daha homojen doz dağılımı sağlanırken, tiroid bezlerinde KRT I tekniğine göre daha düşük dozlar elde edildi. KRT tekniği ile tedavi yapılacaksa başta tiroid bezi olmak üzere larenks ve spinal kord gibi yapıların dozlarının kontrol altına alınabilmesi için orta hat bloğu kullanılması daha uygundur. Hedef hacimlerde daha homojen doz dağılımının sağlandığı YART yönteminde tiroid bezinde görülen daha yüksek dozlar ise planlama sırasında TBD sınırları tanımlanarak düşürülebilir.Anahtar Sözcükler: Baş-boyun kanseri, konvansiyonel radyoterapi, üç boyutlu konformal radyoterapi, yoğunluk ayarlı radyoterapi, tiroid bezi dozu.
