Aim: In this study, it was aimed to investigate the effect of using `Aperture Shape Controller`(ASC) feature in the optimization with `Photon Optimizer` (PO) algorithm in `VolumetricModulated Arc Therapy` (VMAT) RT plans of head and neck, lung, and prostate cancer patientsin terms of treatment plan quality (TPQ), treatment delivery efficiency (TDE), and agreementbetween calculated and delivered dose distribution (QA).Hypothesis: The use of ASC feature in optimization will be superior in terms of TPQ, TDE, andQA.Material-Method: A total of 36 patients (each 12 with head and neck, lung, and prostate cancer)with a definitive RT plan were selected as the study group. Treatment plans were reprepared withfull VMAT technique and optimized with the PO algorithm. In each case, optimization wasperformed by selecting 3 different levels of ASC feature (1. off, 2. medium, 3. very high). Inorder to test the dosimetric acceptability of each RT plan prepared, patient-specific QualityAssurance (QA) was evaluated with `ArcCheck`. Comparisons of different ASC levels wereperformed using `Wilcoxon Signed-Rank` test (two related samples test) in terms of TPQ(Homogeneity Index (HI), OAR dose-volume parameters (DVP)), TDE (Average Open Aperture(AA), Modulation Complexity Score (MCS), Total Monitor Unit (MU)), and QA.Results: TPQ: The most homogeneous dose distribution in terms of HI for all PTVs in head andneck and prostate cancer is provided when the ASC level was off. OAR: As the ASC level waschanged, significant differences were observed in the DVPs of brainstem, right cochlea, totalparotid, right parotid, left and right optic nerve, lung, intestine, left femoral head and bladder.TDE: There was a significant difference only in MCS in head and neck cancer while in AA, MCSand total MU values in lung and prostate cancers.4Conclusion: In the VMAT planning of patients with head and neck, lung and prostate cancer, theuse of very high level of ASC is more favorable compared to the closed + medium level in termsof TDE and QA.

Amaç: Çalışmamızda baş-boyun, akciğer ve prostat kanserli hastaların Volümetrik Modüleedilmiş Ark Tedavisi (VMAT) RT planlarında, `Photon Optimizer` (PO) algoritması ileoptimizasyonda `Aperture Shape Controller` (ASC) özelliğinin kullanılmasının tedavi plankalitesi (TPK), tedavi verme etkinliği (TVE) ve hesaplanan ve verilen doz dağılımı arasındakiuyum (QA) üzerine etkisinin incelenmesi amaçlandı.Hipotez: ASC kullanımının kullanılmamasına göre TPK, TVE ve QA açısından daha üstünolacağıdır.Gereç-Yöntem: Definitif RT planı yapılmış olan toplam 36 (akciğer, baş-boyun ve prostatkanserli 12'şer) hasta çalışma grubu olarak seçildi. Tamamı VMAT tekniğiyle yeniden tedaviplanları hazırlanıp, PO algoritması ile optimize edildi. Her bir olguda, ASC özelliğinin 3 farklıdüzeyi (1. kapalı, 2. orta, 3. çok yüksek) seçilerek optimizasyon yapıldı. `ArcCheck` ile herhastada her bir plana özgü Kalite Güvenirliği (QA) değerlendirilmesi yapıldı. TPK (Homojeniteİndeksi(HI), OAR doz-volüm parametreleri(DVP)), TVE (Ortalama Açıklık Aralığı(AA),Modülasyon Karmaşıklık Skoru(MCS), toplam Monitor Unit(MU)) ve QA yönünden ASCdüzeylerinin ikili karşılaştırmaları `Wilcoxon Signed-Rank` testiyle yapıldı.Bulgular: TPK: Baş-boyun ve prostat kanserinde tüm PTV'ler için HI yönünden en homojen dozdağılımı ASC Kapalı olduğunda sağlanmaktadır. OAR: Beyin sapı, sağ koklea, toplam parotis,sağ parotis, sol ve sağ optik sinir, akciğer, barsak, sol femur başı ve mesane DVP'nde ASCdüzeyi değiştiğinde anlamlı farklar ortaya çıkmıştır. TVE: Baş-boyun kanserinde sadece MCS,akciğer ve prostat kanserinde ise AA, MCS ve toplam MU değerlerinde ASC düzeyi değiştiğindeanlamlı fark bulunmuştur. QA: Tüm olgularda ASC düzeyi değişimiyle anlamlı fark gözlenmiştir.2Sonuç: Baş-boyun, akciğer ve prostat kanserli hastaların VMAT planlanmasında, ASCkullanımında çok yüksek düzey ile kapalı+orta düzeye kıyasla TVE ve QA açısından olumlusonuçlar elde edilmektedir.
