Aim: In this study, it is aimed to determine the knowledge of breast cancer symptoms and breast cancer risk levels of women, mammography, clinical breast examination and self-knowledge about breast examination.Materials and Methods: The study was carried out between June 2017 and May 2019 in Selçuk University Medical Faculty Hospital Family Medicine Polyclinic. 1000 women aged 20-69 years were included in the study. The questionnaire which consisted of 62 questions questioning sociodemographic characteristics, knowledge, attitudes and behaviors of breast cancer screening methods and breast cancer risk levels of the participants was applied with face to face interview technique. After completing the questionnaire of the participants, BSE training was given. Smokers who want to quit the participants were given counseling on this issue. All data were evaluated with SPPS 21.0 statistical program. Mean, percentage, number, median and standard deviation, Chi-square analysis, Kruskal Wallis-H test, Whitney U test and Spearman's rho correlation analysis were used for data analysis. Significance level was taken as p <0.05.Results: The mean age of the participants was 40.32 ± 13.47, 62.4% were married, 28.6% were single, 9.0% were widowed or divorced. 43.1% of the participants were housewives, 45.1% were employed, 9.3% were students and 2.5% were retired. While 39.4% of the participants were university graduates, 19.2% were high school graduates, 6.1% were secondary school graduates, 29.7% were primary school graduates, 5.6% were illiterate. While 3.7% of the participants had first-degree relatives, 6.2% had second-degree relatives with breast cancer, the frequency of personal breast cancer was 0.5%. When the risk levels of the participants were examined according to the breast cancer risk assessment form, 93.4% were low; 5.9% were in the middle, 0.2% were high and 0.5% were in the highest risk group. Breast cancer risk score averages; It was found to be statistically higher in patients older than 60 years, with familial and personal breast cancer history, in those who had first childbearing age 30 and after, menstruating age 11 and under, and those who were overweight (p <0.05). The risk levels of those aged 40 and over, widowed or divorced, illiterate, retired, abortion were significantly higher (p <0.05). There was a positive correlation between the breast cancer risk score of the participants and the number of cigarettes smoked per day and the duration of smoking. 48.5% of the participants answered the age of BSE, 29.6% answered that BSE should be performed at the end of menstruation and 42.2% answered the frequency of BSE correctly. 6.2% of the participants knew the incidence of CMM and 13.9% knew the age and frequency of onset of mammography correctly. The frequency of regular BSE was 10.6%. The incidence of at least one BSE was higher in patients aged between 30-40 and university graduates (p <0.001). Participants learned how to perform BSE most frequently from health care workers. The incidence of having at least one CMM was 36.2%, and the frequency of having a mammography at least once more than 40 years of age was 57.4.Conclusions: Although breast cancer risk assessment in women gives an idea about the level of risk, it does not provide precise information about the possibility of having breast cancer. Therefore, screening for women according to age, which is the most important independent risk factor, should be performed. Smoking increases the risk of breast cancer. Women who smoke should be encouraged to quit smoking. Women lacked knowledge about screening methods and breast cancer symptoms. The rates of using breast cancer screening methods were low. Participants did not use screening methods because they did not have the most common breast complaints. Most of the participants had information from health workers. For this reason, health professionals, especially family physicians, should feel the necessary responsibility for women to have cancer screening properly and regularly and inform their patients about this.Key words: Breast cancer risk, breast cancer screening, breast cancer awareness.
Amaç: Bu çalışmada, kadınların meme kanseri belirtilerini bilme durumlarının ve meme kanseri risk düzeylerinin belirlenmesi, mamografi çektirme, klinik meme muayenesi ve kendi kendine meme muayenesi konusundaki bilgi tutum ve davranışlarının incelenmesi amaçlandı.Gereç ve Yöntem: Çalışmaya Haziran 2017-Mayıs 2019 tarihleri arasında Selçuk Üniversitesi Tıp Fakültesi Hastanesi Aile Hekimliği Polikliniğine yürütüldü. 20-69 yaş arasında 1000 kadın çalışmaya alındı. Katılımcıların sosyodemografik özelliklerini, meme kanseri tarama yöntemleri hakkında bilgi, tutum ve davranışlarını ve meme kanseri risk düzeylerini sorgulayan 62 sorudan oluşan anket formu yüz yüze görüşme tekniği ile uygulandı. Katılımcıların anket formunun doldurulması sonrasında KKMM eğitimi verildi. Sigara içen katılımcılardan bırakmak isteyenlere, bu konuda danışmanlık hizmeti verildi. SPPS 21.0 istatistik programı ile tüm veriler değerlendirildi. Verilerin incelenmesinde ortalama, yüzde, sayı, median ve standart sapma, Ki Kare analizi, Kruskal Wallis-H testi, Whitney U testi ve Spearman's rho korelasyon analizi kullanıldı. Önemlilik düzeyi olarak p<0,05 alındı.Bulgular: Katılımcıların yaş ortalaması 40,32±13,47, %62,4'ü evli, %28,6'sı bekar, %9,0'u dul veya boşanmıştı. Katılımcıların %43,1'i ev hanımı, %45,1'i çalışan, %9,3'ü öğrenci ve %2,5'i emekliydi. Katılımcılardan %39,4'ü üniversite, %19,2'si lise, %6,1'i ortaokul, %29,7'si ilkokul mezunu iken, %5,6'sı ise okuryazar/okuryazar değil idi. Katılımcıların %3,7'sinde 1.derece akrabalarında, %6,2'sinde 2.derece akrabalarında meme kanseri öyküsü varken kişisel meme kanseri sıklığı ise %0,5 idi. Katılımcıların meme kanseri risk değerlendirme formuna göre risk düzeylerine bakıldığında ise %93,4'ü düşük; %5,9'u orta, %0,2'si yüksek ve %0,5'i en yüksek risk grubundaydı. Meme kanseri risk puanı ortalamaları; 60 yaşından büyük olanlarda, ailesel ve kişisel meme kanseri öyküsü olanlarda, ilk çocuk doğurma yaşı 30 yaş ve sonrası olanlarda, 11 yaş ve altında adet görenlerde ve beden yapısı kilolu olanlarda istatistiksel açıdan anlamlı olarak yüksek bulundu (p<0,05). 40 yaş ve üzeri olanların, medeni durumu dul veya boşanmış olanların, eğitim durumu okuryazar olanların, emeklilerin, kürtaj olanların risk düzeyleri anlamlı şekilde yüksekti (p<0,05). Katılımcıların meme kanseri risk puanı ile halen sigara içen katılımcıların günlük içtikleri sigara adedi ve sigara kullanım süreleri arasında pozitif yönde korelasyon vardı. Katılımcıların %48,5'i KKMM yaşını, %29,6'sı KKMM'nin adet sonunda yapılması gerektiğini, %42,2'si ise KKMM sıklığını doğru cevapladı. Katılımcıların %6,2'si KMM sıklığını, %13,9'u mamografinin başlama yaşı ve sıklığını doğru bilmekteydi. Düzenli KKMM yapanların sıklığı %10,6 idi. En az bir kez KKMM yapma sıklığı 30-40 yaş arasında olanlarda ve üniversite mezunlarında fazla idi (p<0,001). Katılımcılar KKMM yapmayı en sık sağlık çalışanlarından öğrenmişlerdi. En az bir kez KMM yaptıranların sıklığı %36,2, 40 yaş üstü en az bir kez mamografi çektirenlerin sıklığı ise 57,4 idi. Sonuçlar: Kadınlarda meme kanseri risk değerlendirmesi, risk düzeyi hakkında fikir vermekle beraber meme kanseri olma olasılığı için kesin bilgi vermemektedir. Bu nedenle en önemli bağımsız risk faktörü olan yaşa göre kadınlara uygun taramaların yapılması gerekmektedir. Sigara kullanımı meme kanseri riskini artırmaktadır. Sigara içen kadınlar sigarayı bırakmaları konusunda teşvik edilmelidir. Kadınların tarama yöntemlerini ve meme kanseri belirtilerini bilme açısından eksikleri mevcuttu. Meme kanseri tarama yöntemlerini kullanma oranları düşük bulundu. Katılımcılar en sık meme ile ilgili şikayetleri olmadığı için tarama yöntemlerini kullanmamaktaydı. Katılımcıları en sık sağlık çalışanlarından bilgi edinmişlerdi. Bu sebeple sağlık çalışanları özelliklede aile hekimleri, kadınların kanser taramalarını doğru ve düzenli şekilde yaptırmaları konusunda gerekli sorumluluğu hissetmeli ve hastalarını bu konuda bilgilendirmelidirler.Anahtar kelimeler: Meme kanseri riski, meme kanseri taraması, meme kanseri farkındalığı.
