SUMMARYThe aim of this study was to investigate the efficiency on apical debris and smear layer removal of the final irrigation solution QMix activated by different activation systems.For this purpose, 180 single-rooted human premolar teeth were used, the crowns of the teeth were removed and the roots were shaped up to size X4 using the ProTaper Next rotary instrument file system. Firstly, the roots were randomly divided into 6 main groups according to be used different activation protocols (n=30): Manuel activation with 30G NaviTip needle (GIA), manual dynamic activation with gutta-percha cone (GPA), passive ultrasonic activation (PUA), EndoActivator (EA), XP-Endo Finisher (XP) and Photon Induced Photoacoustic Streaming (PIPS). Each main group was then divided into 3 subgroups according to be used the final irrigation solution (distilled water, 17% EDTA and QMix) (n=10). While debris was examined in the randomly selected five of the 10 roots in each subgroup, the smear layer was examined in the other five roots.The roots to be examined the debris removal activity were divided into longitudinal sections by means of a spatula placed on pre-opened notches at buccal and lingual surfaces and a standardized groove was prepared in the apical part of each segment. Then, artificial dentin debris was placed into the groove and the root halves were reassembled. Each specimen was irrigated with according to the identified activation protocol. After irrigation the roots were split again and the remaining amount of the debris in the grooves was examined by stereomicroscope under the x25 magnification and scored according to 4-grade scoring system.In order to examine the smear layer removal activity, the roots were irrigated with the defined activation protocols and then separated into the longitudinally. SEM analysis was performed on a randomly selected a part of the root half. SEM images were taken with x1000 magnification at the apical thirds of the root canals and scoring was performed with 5-grade scoring system.Scoring results were analyzed statistically. According to the results of statistical analysis, it was found that in the apical debris removal, XP and PIPS activation techniques were more effective than other techniques for QMix (p<0.05). For EDTA, there was no statistically significant difference between activation techniques (p>0.05). In the apical smear layer removal, EDTA was equally effective in use with all activation techniques (p>0.05). QMix was found to be more effective when used with GIA, PUA, EA and XP activation methods than GPA and PIPS (p<0.05). As results; EDTA may be recommended as a solution for use with all activation systems for apical debris removal, while it is advisable to use QMix with XP and PIPS activation methods. In addition, while EDTA can be recommended for use with any activation techniques for cleaning the smear layer in the apical third of root, it could be said that QMix will not be the right choice for use in activation with GPA and PIPS. ÖZETBu çalışmanın amacı farklı aktivasyon sistemleri ile aktive edilen final irrigasyon solüsyonu QMix'in apikal debris ve smear tabaka çıkarma etkinliklerini incelemektir.Bu amaçla, 180 adet tek köklü insan premolar dişleri kullanıldı, dişlerin kuronları uzaklaştırıldı ve kökler ProTaper Next döner alet eğe sistemi kullanılarak X4'e kadar şekillendirildi. Daha sonra öncelikle kökler kullanılacak farklı aktivasyon sistemlerine göre 6 ana gruba ayrıldı (n=30): 30G geleneksel endodontik irrigasyon iğnesi ile yapılan elle aktivasyon (GİA), güta-perka kon ile yapılan dinamik aktivasyon (GPA), pasif ultrasonik aktivasyon (PUA), EndoActivator (EA), XP-Endo Finisher (XP) ve PIPS tekniği ile aktivasyon. Daha sonra her bir ana grup kullanılan final irrigasyon solüsyonuna göre (distile su, %17'lik EDTA ve QMix) 3 alt gruba ayrıldı (n=10). Her bir alt gruptaki 10 kökün rastgele seçilen 5 tanesinde debris, diğer 5 tanesinde ise smear tabakası incelendi.Debris çıkarılma etkinliği incelenecek köklerin bukkal ve lingual yüzeylerinde önceden açılan çentiklere yerleştirilen bir spatül yardımı ile kökler longitudinal olarak ikiye ayrıldı ve rastgele seçilen bir segmentinin apikal kısmında standart yapay oluk açıldı. Açılan oluğa yapay debris yerleştirildi ve kökler diğer yarısyla tekrar birleştirilerek, belirlenen irrigasyon ve aktivasyon protokollerine tâbi tutuldu. Daha sonra kökler tekrar ayrıldı ve oluklara yerleştirilen debris miktarındaki değişim x25 büyütme altında stereomikroskop ile incelendi ve ardından kalan debris miktarı 4'lü skor sistemine skorlandı.Smear tabakası incelenecek kökler belirlenen irrigasyon ve aktivasyon işlemlerine tâbi tutuldu ve daha sonra longitudinal olarak ikiye ayrıldı. İkiye ayrılan köklerden rastgele bir parçaya SEM analizi yapıldı. x1000 büyütme ile alınan SEM görüntüleri üzerinde 5'li skor sistemi ile skorlama yapıldı.Skorlama sonuçları istatistiksel olarak analiz edildi. İstatistiksel analiz sonuçlarına göre; kök kanal sisteminin apikalinde debris temizlemede QMix solüsyonu için XP ve PIPS aktivasyon yöntemlerinin diğerlerine göre daha etkili olduğu (p>0.05); EDTA için ise aktivasyon yöntemleri arasında önemli bir fark olmadığı belirlendi (p>0.05). EDTA'nın kök kanalının apikal üçlüsünde smear tabaka temizleme yönünden araştırmada kullanılan tüm aktivasyon yöntemlerinde eşit şekilde etkili olduğu (p>0.05); QMix'in ise GİA, PUA, EA ve XP aktivasyon yöntemleriyle kullanımda GPA ve PIPS'le kullanımına göre daha etkili olduğu belirlendi (p<0.05). Sonuç olarak, apikalden debris temizleme açısından tüm aktivasyon sistemleriyle kullanımda solüsyon olarak EDTA tavsiye edilebilirken, QMix'in özellikle XP ve PIPS aktivasyon yöntemleriyle kullanımı tavsiye edilebilir. Apikal üçlüde etkili smear tabaka temizliği amacıyla herhangi bir aktivasyon yöntemiyle kullanım için EDTA tavsiye edilebilecek bir solüsyon iken, QMix 'in GPA ve PIPS'le aktivasyonda kullanım için doğru bir seçim olmayacağı söylenebilir.
