Aim : In this research, the efficiency aiming at protecting children aged 1-5 years old from environmental tobacco smoke effect (ETS) and influence of a comprehensive education programme approach (three home visits and two telephone follow-ups) for mothers were evaluated. To that end, an effiency in terms of reducing the ETS exposure of children 1) prior to education and 2) a restricted education approach (two home visits) were compared.Materials and Method : This randomized and controlled intervention research was performed in Cengizhan settlement in Bayraklı/İzmir. 182 mothers having 1-5 year old children who smoke or have smoking husbands were graded based on the number of smoking parents and 80 of these mothers were chosen by the random sampling method. Mothers who were visited at their homes were conducted a questionnaire, trained, left brochures and taken urine samples from their children. Following that 40 mothers determined by graded randomization method in SAS programme were divided into comprehensive and restricted education groups. The follow-up of two mothers in the comprehensive education programme group could not be completed. The comprehensive education group was visited again one month later and their urinary cotinine level was informed and they were carried out a follow-up questionnaire. After that they were called twice every two weeks. The urinary cotinine level of other group, however, was reported by post. In the last visit, both groups were conducted questionnaires and urinary samples were taken. The measurements of cotinine were carried out by enzyme immunoassay kit. In this research, the preventive behaviours of mothers from ETS effects were assessed based upon the concepts of Theory of Planned Behaviour forming attitude, control perception, social norm and intention.Findings: The levels of cotinine in the comprehensive (t=6,123; p=0,000) and restricted education (t=5,218; p=0,000) groups in the final follow-up were significantly low compared to the levels in the first follow-up. The rate of mothers expressing a precise smoking ban at home in the last follow-up was found out to be higher in the comprehensive education group than restricted education group (?2=3,903; p=0,048). There was no significant difference in attitude (t=-0.078; p=0.938 ) and social scores (t=-1.559; p=0.123) between the first follow-up and the last follow-up after the intervention in both group mothers. Control (t=-4.721; p=0.000) and intention scores (t=-3.420; p=0.001), however, were identified to increase in the last follow-up.Results : A reduction was established in the rate of children in both groups who were exposed to ETS in view of prior to education period and the intervention performed to comprehensive education group was determined to be more influential. Following the intervention, while there was an increase in the scores of control perception and intention aimed at smoking ban conducted by mothers at home, there was no considerable difference in the social norm scores of them.Key words : Environmental Tobacco Smoke, Child Health, Home Visit, Randomize Controlled Test, Cotinine.

Amaç: Bu araştırmada 1-5 yaş arası çocukları ÇTD'den korumayı hedefleyen ve annelere yönelik olan kapsamlı bir eğitim girişiminin (üç ev ziyareti ve iki telefon izlemi) etkinliği değerlendirilmiştir. Bu amaçla bu girişiminin çocukların ÇTD maruziyetini azaltma açısından etkinliği 1) eğitim öncesi durum ile 2) sınırlı bir eğitim girişiminin (iki ev ziyareti) etkinliği ile karşılaştırılmıştır.Gereç ve Yöntem: Bu randomize kontrollü girişim araştırması İzmir-Bayraklı ilçesi Cengizhan Mahallesi'nde uygulanmıştır. 1-5 yaş arası çocuğu olan, eşi ve/veya kendisi sigara içen 182 anne sigara içen ebeveyn sayısına göre tabakalandırılmış, rasgele örnekleme yöntemiyle 80 anne seçilmiştir. Evlerinde ziyaret edilen annelere anket uygulanmış, eğitim verilmiş, broşür bırakılmış ve çocuklarından idrar örnekleri alınmıştır. Ardından SAS programında tabakalı randomizasyon yöntemiyle 40'ar anne kapsamlı ve sınırlı eğitim gruplarına ayrılmıştır. Kapsamlı eğitim grubundaki iki annenin izlemleri tamamlanamamıştır. Kapsamlı eğitim grubu bir ay sonra tekrar ziyaret edilerek kotinin düzeyi bildirilmiş ve izlem anketi uygulanmıştır. Ardından iki hafta arayla iki kez telefon edilmiştir. Diğer gruba ise sadece postayla kotinin düzeyi bildirilmiştir. Son ziyarette iki gruba da anket uygulanmış ve idrar örneği alınmıştır. Kotinin ölçümleri enzim immunassay kiti ile yapılmıştır. Araştırmada, annelerin çocukları ÇTD etkileniminden koruma davranışları Planlı Davranış Kuramı'nı oluşturan tutum, kontrol algısı, sosyal norm ve niyet kavramları temelinde değerlendirilmiştir.Bulgular: Kapsamlı (t=6,123; p=0,000) ve sınırlı (t=5,218; p=0,000) eğitim gruplarında son izlemdeki kotinin düzeyleri ilk izleme göre anlamlı düzeyde düşüktür. Son izlemde evde tam bir sigara yasağı uygulandığını belirten anne yüzdesi kapsamlı eğitim grubunda, sınırlı eğitim grubuna göre daha yüksek bulunmuştur (?2=3,903; p=0,048). Girişim sonrası her iki gruptaki annelerin ilk izlem ile son izlemdeki tutum (t=-0.078; p=0.938 ) ve sosyal puanları arasında (t=-1.559; p=0.123) anlamlı fark yoktur. Kontrol (t=-4.721; p=0.000) ve niyet (t=-3.420; p=0.001) puanlarının ise son izlemde arttığı belirlenmiştir.Sonuç: Her iki grupta da çocukların ÇTD maruziyetinde eğitim öncesine göre azalma saptanmış olup, kapsamlı eğitim grubuna uygulanan girişimin daha etkili olduğu belirlenmiştir.
