OBJECTIVE: The present study aimed at investigating tendency to eyelid laxity, problems of ocular surface and tear film and thinning in nerve fiber layer of the retina and changes in findings of ocular surface and tear film in following nasal CPAP treatment in the patients with severe obstructive sleep apnea syndrome (OSAS).MATERIAL AND METHOD: The study group consisted of 40 patients diagnosed as having severe OSAS by polysomnography and 21 control cases diagnosed as simple snoring. The patients group underwent detailed ophthalmological examination and measurement of intraocular pressure, ocular surface staining with fluoresceine, Schirmer 1 test, TBUT test, measurement of eyelid distraction distance, topography, OCT, and conjunctival impression cytology. The control group also received the same procedures. These examinations and procedures were performed twice; once prior to initiation of nasal CPAP treatment and at month 4 after CPAP treatment. In statistical comparisons with the control group, the baseline examination findings of the patients with OSAS were considered.FINDINGS: Eyelid laxity was found in 9 of 40 patients with severe OSAS. Frequency of eyelid laxity was found as 22.5%. increased frequency of eyelid laxity was found in these patients. The fact that frequency of dysfunction of meibom gland, rate of punctuate epitheliopathy and evaporative dry eye was 42.5% was statistically significant compared to the control group (p < 0.05). The finding of dry eye was significantly more on the left eye. Following CPAP treatment decrease in BUT, increase in Schirmer test, increase in conjunctival squamous metaplasia, increase in ocular irritation symptoms in the right eye and increase in thickness of central cornea in both eyes were all significant compared to the control group (P < 0.05). history of smoking, frequency of morbid obesity and hypoxia was significantly more in the cases with eyelid laxity compared to those without it.CONCLUSION: The present study showed tendency of the patients with OSAS to dry eye. Furthermore, the fact that signs of dry eye in these patients following CPAP also showed that these patients should be carefully evaluated by ophthalmologists and specialists of sleep disorders. Increased rate of history of smoking and morbid obesity as well being hypoxic of these patients suggest that these factor may play role in etiopatogenesis of eyelid laxity. Further studies are needed to explore these associations.

Amaç: Bu çalışmada Ağır Obstruktif Uyku Apne(OUAS) hastalarında göz kapağı gevşekliği, oküler yüzey ve göz yaşı film problemleri ile retina sinir lifi tabakası incelmesine yatkınlık, ve bu hastalarda nazal CPAP tedavisi sonrası oküler yüzey ve gözyaşı bulgularındaki değişimi araştırmayı amaçladık.Materyal ve Metod: Polisomnografi ile Ağır OUAS tanısı almış 40 hasta ve Basit horlama tanısı almış 21 kontrol olgusu çalışma grubunu oluşturdu. Hasta grubuna detaylı oftalmolojik muayene ve GİB ölçümü, floressein ile oküler yüzey boyanması, schirmer 1 testi, TBUT testi, gözkapağı distraksiyon mesafesi ölçümü, topografi, OCT, konjonktival impresyon sitolojisi tetkikleri yapıldı. Kontrol grubuna da aynı muayeneler yapıldı. Bu muayene ve tetkikler Ağır OUAS hastaları nazal sürekli pozitif havayolu(CPAP) tedavisine başlamadan önce ve nazal CPAP tedavisi sonrası 4. ayda olmak üzere iki kez yapıldı. Kontrol grubu ile yapılan istatistiksel karşılaştırmada OUAS'lı hastaların ilk muayene bulguları dikkate alındı.Bulgular: Ağır OUAS' lı 40 hastadan 9'unda gevşek gözkapağı saptandı. Gevşek göz kapağı sıklığı %22.5 olarak bulundu. Bu hastalarda artmış göz kapağı laksitesi saptandı. Bu hastalarda meibom gland disfonksiyonu, punktat epitelyopati görülme sıklığı ile Evoporatif kuru göz sıklığının %42.5 olması kontrol grubuna göre istatistiksel olarak anlamlıydı(p<0.05). Kuru göz bulguları anlamlı şekilde sol gözde daha fazlaydı. CPAP tedavisi sonrası sağ gözde BUT'da azalma, schirmerde artış, konjunktival skuamöz metaplazide artış, oküler irritasyon semptomlarında artış ve her iki gözde santral kornea kalınlığındaki artma kontrol grubuna kıyasla anlamlıydı(p<0.05). Gevşek gözkapaklı olgularda sigara içimi, morbid obezite sıklığı ve hipoksi gevşek göz kapağı olmayanlara göre anlamlı şekilde fazlaydı.Sonuç : Bu çalışma OUAS'lı hastaların evoporatif göz kuruluğuna yatkın olduğunu gösterdi. Ayrıca nazal CPAP tedavisi sonrası göz kuruluğu bulgularında artma olması bu hastaların oftalmologlar ve uyku hastalıkları uzmanı tarafından dikkatli değerlendirilmesi gerektiğini gösterdi. Ayrıca gevşek gözkapaklı olgularda sigara içimi, morbid obezite birlikteliğinin sık olması ve bu hastaların daha hipoksik olmaları gevşek gözkapağı etyopatogenezinde bu faktörlerin rol oynayabileceğini düşündürmektedir. Bu ilişkilerin aydınlatılabilmesi için daha ileri çalışmalara ihtiyaç vardır.
