ABSTRACTRole of Presepsin in the Diagnosis of the Patients With Ventilator-Associated PneumoniaIntroduction: Mechanical ventilation is vital for treatment of acute respiratory failure in the intensive care units(ICU). Ventilator associated pneumonia (VAP), one of the dreaded complications of mechanical ventilation, is an important risk factor for morbidity and mortality besides increasing costs and length of intensive care unit stays. This necessitates the early diagnosis and identification of high-risk patients with VAP requiring aggressive treatment. Aim: Aim of this study is to investigate whether the presepsin is a diagnostic biomarker for VAP and it has an effect on mortality ICU patients. Materials and Method: The present study was prospectively conducted in the Intensive Care Clinic of Department of Anesthesiology and Reanimation and Intensive Care Internal Medicine Department from November 2015 to July 2016 upon the approval of the Ethics Committee of Çukurova University School of Medicine.60 patients who have been in the intensive care for more than 120 hours and who are over 18 years old and diagnosed with VAP have been included in our study.Exclusion criteria were HIV infection, active tuberculosis, immunosuppressive therapy, postsplenektomi and neutropenia in the study. On the first day of diagnosis of VAP, APACHE II score was calculated and recorded by using the worst values of 17 variables of APACHE II score. On the 1st, 3rd and 7th days, blood samples were collected from the patients included in the study in order to detect PRESEPSİN, WBC, CRP, PCT and Lactate values. Patients included in the study were monitored until 28th dayand the results were recorded as recovered, monitoring or exitus.Conclusion: It was concluded that Presepsin has no diagnostic impact in the patients diagnosed with VAP while APACHE II scores have prognostic impact for these patients and their increased results are associated with mortality. Giriş: Yoğun bakım ünitelerinde, akut solunum yetmezliği tedavisinde mekanik ventilasyon önemli bir yere sahiptir. Mekanik ventilasyonun en korkulan komplikasyonlarından birisi olan ventilatör ilişkili pnömoni (VİP) yatış sürelerini ve maliyeti arttırması yanısıra, morbidite ve mortalite için de önemli bir risk faktörüdür. Bu durum agresif tedavi gerektiren yüksek riskli VİP hastalarının erken tanımlanması ihtiyacını doğurur.Amaç: Bu çalışmanın amacı, yoğun bakımdaki VİP tanısı almış hastaların APACHE II skoru hesaplanarak, yeni bir biyomarkır olan Presepsin ile birlikte rutinde kritik hastaların takibinde bakılan WBC, CRP, PCT ve Laktat değerlerinin, hastaların erken tanısı ve mortalitelerine olan etkilerini değerlendirerek hastalığın prognozu hakkında öngörüde bulunmaktır.Materyal ve Metod: Bu çalışma prospektif olarak Çukurova Üniversitesi Tıp Fakültesi etik kurul izni alındıktan sonra Kasım 2015-Temmuz 2016 tarihleri arasında Anesteziyoloji ve Reanimasyon Anabilim Dalı Yoğun Bakım Kliniğinde ve Dahiliye Anabilim Dalı Yoğun Bakım Kliniğinde yapıldı.Çalışmamıza yoğun bakım ünitesinde 120 saatten fazla yatan 18 yaş ve üzeri VİP tanısı almış 60 hasta dahil edildi HIV enfeksiyonu olan, aktif tüberkülozu olan, immün supresif tedavi alan, postsplenektomili hastalar ve nötropenik hastalar ise çalışmaya dahil edilmedi. VİP tanısı konulan ilk gün içerisinde APACHE II skorunu oluşturan değişkenlerin en kötü değerleri alınarak APACHE II değeri hesaplandı ve kaydedildi. Çalışmaya dahil edilen hastaların 1. 3.ve 7. gününde Presepsin, WBC, CRP, PCT ve Laktat değerlerini bulmak için kan örnekleri alındı. Çalışmaya dahil edilen hastalar 28. Güne kadar takip edildi ve şifa, takibe devam veya exitus olarak kayıt edildi. Sonuç: VİP tanısı almış hastalarda Presepsinin diagnostik bir özelliğinin olmadığı, APACHE II değerlerinin ise bu hastalarda prognostik özelliklere sahip olduğunu ve artmış sonuçlarının mortalite ile ilişkili olduğu kanısına vardık.Anahtar Kelimeler: APACHE II, Presepsin, Ventilatör İlişkili Pnömoni, WBC, CRP, PCT, Laktat.
