Renal transplantation (tx) is the best treatment option which is grown quality of living and survival for patient with end stage renal disease (ESRD). However, rejection of the allografts remains to be the most important barrier for a successful transplantation. It is important that is known about immunologic mechanism for designing the appropriate treatment strategies.The study population consisted of 50 renal transplant recipients and divided into two groups, 44 patients with stable graft function (SGF) and 6 patients with rejection (RHG). In this prospective study we followed renal transplant recipients along 6 month. Peripheral blood samples from patients were drawn on pre-tx day, in addition to the post-tx days 7, 28, 180 and rejection days for the analysis of the percentages IL-17 and IFN- cytokines by flow cytometry and ELISA. In the present study, we determined that the percentages of intracellular IFN were not significant in the group with RHG versus with SGF. Levels of intracellular IL-17 which are obtained at the 6th month post-tx were significantly higher in RHG than SGF. Therefore plasma levels of pre-tx IL- 17 were also higher in RHG, so it may be predictive biomarker of acute rejection for renal transplantation.Finally, the present study has an informative value about pre-tx and post-tx cytokines profile of ESRD patients. We suggest that cytokine analysis, might be a valuable to use of biomarker panel for preventing from rejection and find new treatment strategies for renal transplant patients.Key Words: Kidney Transplantation, Rejection, Cytokines, Th1 and Th17 cells, End stage renal failure.

Böbrek nakli, son dönem böbrek yetmezliği (SDBY) hastaları için hem yaşam kalitesini, hem de sağ kalımını artıran en iyi tedavi seçeneğidir. Rejeksiyon, allogreft kaybının en önemli sebebidir. Nakil öncesi ve sonrası yapılan immün yanıt değerlendirmeleri greftin takibi açısından önemlidir.Çalışmaya, böbrek nakli olan 50 hasta dahil edildi ve hastalar 6 ay takip edildi. Bu hastalardan 6 tanesi rejeksiyonlu hasta grubu (RHG), 44 tanesi stabil greft fonksiyonluydu (SGF). Böbrek nakli olan hastaların takiplerinde nakil öncesi, nakil sonrası 7. gün, 1.ay, 6.ay ve rejeksiyon geçirdiği gün (anti-rejeksiyon tedavisi almadan) olmak üzere periferal kandan lenfosit ve plazma elde edildi. Tüm hastalardan elde edilen lenfositlerden Th17'den salınan IL-17 ve Th1'den salınan IFN-seviyeleri için hücre içi sitokin tayini kiti kullanılarak akan hücre ölçer cihazıyla değerlendirildi. Ayrıca IL-17, IFN- sitokinlerinin plazmada seviyeleri ELİSA yöntemiyle belirlendi. Tüm hasta grubunda, IFN- hücre içi sitokin seviyelerine göre 1. ay ve 6. ay takiplerinde SGF ve RHG karşılaştırıldığında anlamlı farklılıklar tespit edilmedi. Hücre içi IL-17 seviyeleri ise 6. aylarında, RHG'da SGF'ye göre daha yüksek bulundu.. Plazma sitokin seviyelerinde ise IFN- anlamlı değişim göstermezken, RHG'da nakil öncesi IL-17 sitokin seviyesi SGF'den daha yüksek bulundu. RHG'da nakil öncesinde yüksek çıkan IL-17 seviyesi, rejeksiyon biyobelirteci olarak kullanılabilirliği desteklemektedir.Çalışmada elde edilen veriler, ülkemiz SDBY hastalarının nakil öncesi ve sonrası dönem hakkında bilgi verici düzeydedir. Hasta sayısı artırılarak transplantasyon sonrası sitokin profilinin değerlendirilmesinin bu immün cevabı anlamaya ve rejeksiyonu engellemek adına yeni tedaviler bulmaya ışık tutabileceğini düşünmekteyiz.Anahtar Kelimeler: Böbrek Transplantasyonu, Rejeksiyon, Sitokinler, Th1 ve Th17 hücreleri, Son Dönem Böbrek Yetmezliği.
