The aim of our study; was to determine the effects of Mulligan technique, passive joint mobilization, conventional physiotherapy used in osteoarthritis treatment on pain, Range of Motion (ROM), muscle strength, proprioception and functional status. 64 patients diagnosed with bilateral knee osteoarthritis were included in the study. The assesments were performed before and after treatment. Mulligan mobilization combined with exercise in Group-1; passive joint mobilization combined with exercise in Group-2; superficial heat or cold, Transcutaneous Electrical Nerve Stimulation (TENS), therapeutic ultrasound combined with exercise in Group-3; were applied for 12 sessions. All the groups were performed the same home exercise programme. The pain was assesed according to Visual Analog Scale (VAS) and algometer. ROM and position sense were assesed with digital goniometer. Muscle strength was evaluated by using Handheld dynamometry (Lafeyette Instrument®, Lafayette, IN). The functional status of the patients was evaluated by WOMAC (Western Ontario and McMaster Universities) index and Aggregated Locomotor Function test.Significant improvement was found after treatment on pain, ROM, muscle strength, proprioception and functional status in Group-1 and 2 (p<0.05). However, in Group-3 not all the outcome measurements were significantly improvement. When the groups compared by ANOVA it was found that outcome measures were not significantly different between Group-1 and Group-2 (p>0.05). However, the outcome measures were found significantly different in Group-1 and 2 compared to Group-3 (p<0.05). As a result of this study, the groups performed mobilization techniques combined with exercise were determined to be more effective than used electrotherapy treatment on pain, ROM, muscle strenght, position sense and functional status. Neither Mulligan techniques nor passive joint mobilization exercise were found to be superior for the treatment of osteoarthritis.

Çalışmamızın amacı; diz osteoartrit tedavisinde kullanılan Mulligan tekniğinin, pasif eklem mobilizasyonun ve geleneksel fizyoterapinin ağrı, eklem hareket açıklığı (EHA), kas kuvveti, propriosepsiyon ve fonksiyonel durum üzerine etkilerini belirlemektir. Bilateral diz osteoartrit tanısı alan 64 olgu çalışmaya dahil edildi. Değerlendirmeler tedavi öncesi ve tedavi sonrası yapıldı. Grup-1'e Mulligan mobilizasyonu ve egzersiz, Grup-2'ye pasif eklem mobilizasyonu ve egzersiz, Grup-3'e yüzeyel ısı veya soğuk, Transkutanöz Elektriksel Sinir Stimülasyonu (TENS), terapatik ultrason ve egzersiz 12 seans uygulandı. Her üç gruba da aynı ev egzersiz programı verildi. Ağrı Görsel Analog Skala (GAS) ve algometre ile, EHA ve pozisyon hissi dijital gonyometre ile, kas kuvveti `Hand-held` dinomometre (Lafeyette Instrument®, Lafayette,IN) ile, fonksiyonel durum WOMAC (Western Ontario ve McMaster Üniversiteleri) indeksi ve Birleştirilmiş Lokomotor Fonksiyon testi ile değerlendirildi.Grup içi değerlendirmede, Grup-1 ve Grup-2'de ağrı, EHA, kas kuvveti, propriosepsiyon ve fonksiyonel durumda iyileşme bulundu (p<0,05). Grup-3'te ise ağrı, EHA ve kas kuvvetinin bazı parametrelerinde ve fonksiyonel durumda iyileşme saptandı. Gruplar arası fark `ANOVA` ile değerlendirildiğinde tüm parametrelerde Grup-1 ile Grup-2 arasında istatistiksel olarak anlamlı farklılık görülmezken (p>0,05), ölçüm parametrelerinde Grup-1 ve Grup-2, Grup-3'e göre istatistiksel olarak anlamlı farklılık bulundu (p<0,05). Bu çalışmanın sonucunda, egzersiz tedavisine ek olarak uygulanan mobilizasyon tekniklerinin, kullanılan elektroterapi uygulamalarına göre ağrı, EHA, kas kuvveti, pozisyon hissi ve fonksiyonel durum üzerine daha etkin olduğu bulundu. Osteoartit tedavisinde Mulligan ve pasif eklem mobilizasyon tekniklerinin birbirine üstünlükleri saptanmadı.
