Pain after total knee arthroplasty (TKA) is severe and can produce discomfort. Since it may change the efficacy of the surgical treatment itself and overall mortality and morbidity in scope of the complications it may lead; it is important to ensure postoperative pain control. The challange is to choose an ideal approach which is efficient, safe, practical, with minimal cost and side effect; providing patient comfort. Facilitating early physiotheraphy without motor blockage and muscle weakness is also an important issue. Correspondingly, after evaluating early literature related; we have chosen single shot preemptive femoral nerve blockage to study for its efficacy on postoperative pain control after TKA. Forthy patients; ASA I-III undergoing TKA were randomized into two groups. Group A received preemptive femoral nerve blockage and after a time -at least fifteen minutes- standart general anesthesia applied and IV PCA was given postoperatively. Group B received standart general anesthesia and IV PCA was also given postoperatively. Peroperative morfine requirement, peroperative hemodynamic parameters were recorded. Quality of postoperative analgesia was evaluated by NAS and opioid consumption by IV PCA values; all of which followed up within twenty four hours with 4 hours interval. Side effects and complications were also noted. In conclusion; we determined that preemptive femoral nerve blockage is an effective way to control postoperative pain after TKA, leading to lower opioid consumption through 24 hours. For this reason we recommend preemptive femoral nerve blokage as an efficient way for pain control after TKA.

TDA sonrası ağrı şiddetlidir ve uygulanan cerrahi tedavinin etkinliğini, sebep olabileceği komplikasyonlar ile mortalite ve morbiditeyi olumsuz etkilemesi açısından postoperatif ağrı kontrolü önemlidir. TDA sonrası postoperatif ağrı kontrolünde etkili, güvenli, uygulamada pratik, minimal yan etki ve maliyeti olan; hasta konforunu sağlayıp motor blok ya da kas güçsüzlüğü olmadan erken fizyoterapiye olanak tanıyan ideal bir yaklaşım seçmek gerekmektedir. Biz de daha önce yapılmış çalışmalardaki sonuçları değerlendirerek bu özelliklere uyan tek doz femoral blok uygulamasının TDA sonrası ağrı kontrolündeki etkinliğini araştırdık.Çalışmaya tek taraflı TDA yapılacak ASA I-III sınıfı 40 hasta alınarak, hastalar randomize 20 şer kişilik 2 gruba ayrıldı. Grup A hastalarına preoperatif femoral blok uygulandıktan minimum 15 dakika sonra standart genel anestezi uygulandı ve ameliyat sonrası derlenme ünitesinde İV HKA cihazı verildi. Grup B ya standart genel anestezi uygulandı ve aynı şekilde İV HKA cihazı verildi. Hastaların peroperatif morfin ihtiyacı ve peroperatif hemodinamik verileri kayıt edildi. Hastaların ağrıları derlenme ünitesinden başlayarak postoperatif 24 saat boyunca 4 saatlik aralıklarla NAS değerleri ile; opioid tüketimi ise eşzamanlı HKA cihazı değerleri ile incelendi. Yan etki ve komplikasyonlar not edildi. Çalışmamızın sonucunda postoperatif ağrı kontrolünde preemptif femoral bloğun TDA sonrası postoperatif ağrı kontrolünde 24 saat süre ile etkin olduğunu ve böylece daha az opioid tüketimi sağladığını belirledik. Bu nedenle TDA sonrası ağrı kontrolünde preemptif femoral blok uygulamasının rutin olarak kullanılabilecek etkili bir yöntem olduğunu düşünmekteyiz.
