Spontaneous bacterial peritonitis (SBP) is a frequently encountered and important complication of decompensated liver cirrhosis. In the patients with cirrhosis, the defectiveness in the both humoral and cellular immunity, decrease in filtration function of liver and the decrease in antimicrobial capacity of ascite fluid, all of them increase the risk of spontaneous bacterial peritonitis. Delay in SBP diagnosis causes serious problem. Procalcitonin is a protein, a calcitonin precursor, which is produced by cells of thyroid gland in healthy individuals. A half-life of procalcitonin in serum is 20-24 hours which makes it suitable for daily monitoring and enables to control a course of treatment and to distinguish bacterial infection from other types of inflammations. Procalcitonin levels rise in bacterial, parasite, and yeast infections. The aim of this study is investigate whether there is a difference in serum levels of procalcitonin in group decompensated cirrhotic patients with SBP, group decompensated cirrhotic patients with sterile ascites and control group and to investigate the predictor value of peripheral blood procalcitonin (PCT) levels in the evaluation of prognosis of decompensated cirrhotic patients.A hundred individuals included in the study, are divided into three groups: Group 1 (n:50): Decompansated cirrhotic patients with spontaneous bacterial peritonitis ( polymorphonuclear cells ? 250/mm3 in asidic fluid and/or positive ascite fluid culture) Group 2 (n:25): Decompansated cirrhotic patients with sterile ascitic fluid ( ? 250/ mm3 in asidic fluid and negative ascite fluid culture) Group 3 (n:25): Healthy individuals in the control group included some functionally gis disorders such as functionally dyspepsia or irratable bowel syndrome. At admission, serum and ascites samples were taken thereafter white blood cells and polymorphonuclera cells were calculated in ascitic fluid. Microbiological evaluation of ascitic fluid was performed and the level of procalcitonin was measured in serum fluid.There were no statistically significant difference between three groups in terms of level of serum procalcitonin. White bloods cell counts of ascitic fluid were statistically significantly higher in spontaneous bacterial peritonitis group as compared in sterile ascitic fluid group. Polymorphonuclera cell counts of ascitic fluid were statistically significantly higher in spontaneous bacterial peritonitis group as compared in sterile ascitic fluid group. When we compared the level of serum procalcitonin according to the severity of liver disease, Child A, B and Child C groups showed similar results. there was no statistically significant difference.In conclusion whether there is no difference the level of serum procalcitonin in spontan bacteriyal peritonitis group, sterile ascitic fluid group and in control group. Our study showed that the predictor value of peripheral blood procalcitonin (PCT) levels in the evaluation of prognosis of decompensated cirrhotic patients is not important role.Key words: Liver cirrhosis, spontaneous bacterial peritonitis, procalcitonin.

Spontan bakteriyel peritonit (SBP) dekompanse karaciğer sirozunun sık ve önemli komplikasyonlarından biridir. Karaciğer sirozlu hastalarda hem humoral ve hem de hücresel immünitedeki bozukluk, karaciğer filtrasyon fonksiyonundaki azalma ve assit sıvısının antimikrobiyal kapasitesinin azalması spontan bakteriyel peritonit riskini artırmaktır. SBP tanısının gecikmesi ciddi bir problemdir. SBP tanısına parasentez ile alınan assit mayideki nötrofil sayısı ve kültür sonuçları ile karar verilir. Prokalsitonin (PCT) normal sağlıklı bireylerde tiroid bezinden salgılanan kalsitonin prekürsörü olan bir proteindir. PCT'nin yarılanma ömrünün 20 ile 24 saat arasında olması onun günlük takibini sağlar. Böylece tedavinin izleniminde, kontrolünde ve bakteriyel infeksiyonun diğer inflamasyon sebeplerinden ayrılmasında yardımcı olur. Serum prokalsitonin düzeyi bakteriyel, parazit ve mantar infeksiyonlarında artar.Bu çalışmanın amacı; dekompanse karaciğer siroz tanısı ile takip edilen SBP'li hastalar, steril assitli hastalar ve kontrol grupları arasında serum prokalsitonin düzeyleri arasında bir fark olup olmadığını ve serum prokalsitonin düzeyinin dekompanse karaciğer sirozlu hastaların prognozunda prediktif değerinin olup olmadığını araştırmaktır.Çalışma kapsamına 100 birey alındı, bireyler 3 gruba ayrıldı.1.grup (n:50): Spontan bakteriyal peritonit gelişen dekompanse karaciğer sirozlu hastalardan (assit mayide PMNL sayısı 250/ mm3 ve yukarısı olan ve/veya assidik mayide üremesi olanlar) oluşturuldu2.grup (n:25): Steril assitli dekompanse karaciğer sirozlu hastalardan (assidik mayide PMNL sayısı 250/ mm3'den daha az ve assit mayide üremesi olmayanlar) oluşturuldu.3.grup (n:25): Kontrol grubu olup bilinen organik ve sistemik bir hastalığı olmayan, fonksiyonel dispepsi, irratable barsak sendromu gibi bazı fonksiyonel gastrointestinal rahatsızlıkları nedeniyle iç hastalıkları polikliniğine başvuran bireylerden oluşturuldu.Hastalardan başvuru anında assit mayi ve serum örnekleri alındı. Daha sonra assit sıvısında total lökosit ve PMNL sayıları hesaplandı, assit sıvı kültürü değerlendirildi ve serum prokalsitonin düzeyi ölçüldü.Her üç gruba ait serum PCT değeri karşılaştırıldığında gruplar arasında istatistiksel olarak anlamlı fark bulunamadı.SBP'li grup ile steril assitli grup arasında assit mayi total lökosit ortalama değerleri karşılaştırıldığında istatiksel olarak anlamlı fark bulundu. SBP'li grup ile steril assitli grup arasında assit mayi ortalama Polimorf nükleer lökosit değerleri karşılaştırıldığında istatistiksel olarak anlamlı fark bulundu.Çalışmamızda, karaciğer hastalığının şiddeti açısından prokalsitonin düzeyi karşılaştırıldığında, Child A, B ve C grupları, birbirine benzer sonuçlar gösterdi. Her üç grup arasında istatistiksel olarak anlamlı bir fark bulunmadı.Sonuç olarak çalışmamızda; SBP'li grup, steril assitli grup ve kontrol grupları arasında serum prokalsitonin düzeyleri arasında bir farkın olmadığını tespit edildi. Aynı zamanda, bu çalışmada serum prokalsitonin düzeyinin dekompanse karaciğer sirozlu hastalarda, hastalığın klinik evresi ile bir uygunluk göstermediği sonucuna varıldı.Anahtar Kelimeler: Karaciğer sirozu, spontan bakteriyel peritonit, prokalsitonin.
