Introduction and Purpose: In the literature, we could not find enough data showing frequency and amount of ST segment deviation and T-wave changes on electrocardiography (ECG) and their relation to occlusion level, depending on isolated acute occlusion of circumflex coronary artery (Cx). The aim of this study was to examine the relationship among isolated acute occlusion of Cx and the changes of anterior, inferior, posterior ST segment and T-wave on ECG, and the occlusion level.Method: This study, retrospectively, was done with 175 (mean age: 58.9±10.8 years) patients that were chosen between the consecutive 21855 people applied selective left and right coronary angiography. This choosing was done between patients according to the inclusion and exclusion criteria. At selective coronary angiography, total occlusion is determined as ceasing course of coronary artery abruptly and being seen TIMI-0 or TIMI-1 flow. Cx is divided as central (proximal segment, obtuse marginal branch and intermediary branch) and peripheral (distal segment, poster-lateral branch and posterior descending branch) parts. The patients, depending on the occlusion's localization in Cx, were divided into two groups with central occlusion and peripheral occlusion, respectively.Results: The mean age of 175 patients that have isolated Cx total occlusion was 58.9 (between 32 and 83). 138 patients (78.9%) were male and 37 patients (21.1%) were female. 75 patients (42.9%) were ST-elevation myocardial infarction (STEMI), 77 patients (44%) were Non-ST-elevation myocardial infarction, 23 patients (13.1%) were unstable angina pectoris. In terms of demographic parameters of cases, significant differences between central Cx (n=58, 33.1%) and peripheral Cx (n=117, 66.9%) occlusion groups were not determined (p>0.05). When central and peripheral Cx occlusions were compared, inferior MI was observed more often at peripheral occlusion, whereas inferoposterolateral MI and anterolateral MI were observed more often at central Cx occlusion (46.5%, 40.6% and 12.5%, respectively and p=0.008). The rate and amount of ST segment depression at precordial V1 and V2 derivations were significantly more at the patients with central Cx occlusion (p<0.05). In case of patients with STEMI, compared to the patients with peripheral Cx occlusion, the rate and amount of ST segment depression at precordial V1-V3 derivations were significantly more at the patients with central Cx occlusion (p<0.05). In addition, the rate and amount of ST elevation at V6, V8 and V9 derivations were more at the patients with central Cx occlusion (p<0.05).Conclusion: There were no significant differences between central and peripheral Cx occlusions in terms of acute coronary syndrome (ACS) type. There were no significant differences between two groups in terms of ST segment and T-negativity changes in all other ECG derivations except for V1 and V2. While inferoposterolateral and anterolateral STEMI were observed more in patients with central occlusion than peripheral Cx occlusion, inferior STEMI was observed more at peripheral Cx occlusion. Moreover, in STEMI, the rate and amount of ST segment depression at precordial V1-V3 derivations and ST segment elevation at V6, V8 and V9 derivations were more at the patients with central Cx occlusion, when compared to the ones with peripheral Cx occlusion. Thus, in patients with ACS, depending on shape of ST segment changes and the amount of these changes on ECG records may help in determining acute isolated Cx occlusion and its localization.Key words: isolated circumflex acute occlusion, ST segment change, T-wave change.

Giriş ve Amaç: Literatürde, sirkumfleks (Cx) koroner arterin izole akut oklüzyonuna bağlı olarak elektrokardiyografide (EKG) ST segment ve T dalga değişikliklerinin sıklığını, miktarını ve oklüzyon düzeyi ile ilişkisini gösteren yeterli veri bulunmamaktadır. Bu çalışmanın amacı izole Cx akut oklüzyonu ile EKG'de anteriyor, inferiyor, posteriyor ST segment ve T dalga değişiklikleri ve oklüzyon düzeyi arasındaki ilişkiyi incelemek idi.Metod: Bu çalışma retrospektif olarak, selektif sol ve sağ koroner angiografileri yapılan ardışık 21855 hasta arasından çalışmaya dahil edilme ve dışlanma kriterlerine uygun toplam 175 (ortalama yaş: 58.9±10.8 yıl) hasta alınarak yapıldı. Selektif koroner angiografide total oklüzyon, koroner arter seyrinin aniden kesildiği ve TIMI-0 ya da TIMI-1 akımın görülmesi olarak tanımlandı. Cx santral (proksimal segment, obtus marginal dal ve intermediyer dal) ve periferal (distal segment, posterolateral dal ve posteriyor desending dal) olarak 2 kısma ayrıldı. Hastalar Cx'de oklüzyonun bulunduğu yere göre santral oklüzyonlu ve periferal oklüzyonlu olanlar şeklinde 2 gruba ayrıldı.Bulgular: İzole Cx total oklüzyonu olan 175 hastanın yaş ortalaması 58.9 yıl (32 ile 83 arası) idi. Hastaların 138 (%78.9)'i erkek, 37 (%21.1)'si kadın cinsiyete sahipti. Hastaların 75 (%42,9)'i ST segment yükselmeli AMİ (STEMİ), 77 (%44)'si ST segment yükselmesiz AMİ, 23 (%13.1)'ü unstabil angina pektoris idi. Santral Cx (n=58, %33.1) ve periferal Cx (n=117, %66.9) oklüzyon grupları arasında olguların demografik verileri açısından anlamlı fark bulunmadı (p>0.05). Santral ve periferal Cx oklüzyonları karşılaştırıldığında inferiyor Mİ periferal Cx oklüzyonunda, inferoposterolateral Mİ ve anterolateral Mİ ise santral Cx oklüzyonunda daha sık görülmekteydi (sırasıyla %46.5, %40.6, %12.5, p=0.008). Prekordiyal V1 ve V2 derivasyonlarındaki ST segment depresyon miktarı ve görülme oranı santral oklüzyonlu hastalarda anlamlı ölçüde daha fazlaydı (p<0.05). STEMİ'li hastalarda prekordiyal V1-V3 derivasyonlarda ST segment depresyon görülme oranı ve miktarı periferal Cx oklüzyonlu hastalara kıyasla santral Cx oklüzyonlu hastalarda anlamlı ölçüde daha fazlaydı (p<0.05). Yine, V6,V8 ve V9 derivasyonlarında ST elevasyon görülme oranı ve miktarı santral Cx oklüzyonlu hastalarda daha fazlaydı (p<0.05).Sonuç:. Santral ve periferal Cx oklüzyon grupları arasında akut koroner sendrom (AKS) tipi açısından anlamlı fark bulunmamaktadır. Her iki grup arasında V1 ve V2 hariç diğer tüm EKG derivasyonlarındaki ST segment değişiklikleri ile T negatiflik değişiklikleri açısından da anlamlı fark yoktur. İnferoposterolateral STEMİ ve anterolateral STEMİ santral Cx oklüzyonlu hastalarda periferal Cx oklüzyonlu hastalara kıyasla daha fazla görülüyorken, inferiyor STEMİ periferal Cx oklüzyonlu hastalarda daha fazla görülmektedir. Yine STEMİ'nde prekordiyal V1-V3 derivasyonlarında ST segment depresyonu ve V6, V8 ve V9 derivasyonlarında ST segment elevasyonu görülme oranı ve miktarı santral Cx oklüzyonlu hastalarda periferal Cx oklüzyona göre daha fazla olmaktadır. Böylece, AKS tanısı ile başvuran hastalarda çekilen EKG kayıtlarındaki ST segment değişikliklerinin şeklinin ve değişme miktarının belirlenmesi akut izole Cx oklüzyonu ve oklüzyonun yerinin tahmin edilmesinde yardımcı olabilir.Anahtar Kelimeler: izole Cx akut oklüzyonu, ST segment değişikliği, T dalga değişikliği.
