Upper gastrointestinal system (UGS) bleedings cover any bleeding into the lumen between the upper part of the esophagus and the ligament of Treitz. UGS bleeding is a common major health problem that is still an important cause of morbidity and mortality all over the world despite the diagnostic and therapeutic innovations.In this study, we aimed to determine the characteristics of the patients with non-variceal UGS bleeding. For this purpose, a total of 195 patients at the ages of 19-88 years with the diagnosis of non-variceal upper gastrointestinal bleeding were evaluated prospectively. They either admitted or were already inpatients in gastroenterology clinics of Faculty of Medicine, Ataturk University Research Hospital between January 2010 - January 2011.Among non-variceal upper gastrointestinal bleeding patients; 35.9% were females and 64.1% were males and male/female ratio was 1.7. The mean age of patients was 58.6 and 52.8% of these patients were over 60 years of age. Drug use which may be predisposing to UGS bleeding was found as 52.8%. The drugs used by the patients were evaluated and 20.5% were using nonsteroidal anti-inflammatory drug (NSAID) and 17.4% were using acetylsalicylic acid (ASA). Cigarette smoking habit was found in 34.9% and cigarette plus alcohol usage consumption was found in 5.1% of cases. The most common complaint was melena (45.1%) and melena and hematemesis (37.4%). The mean systolic and diastolic blood pressures, mean heart rate and mean hemoglobin level on admission to hospital were 108.6 mm Hg, 68.7 mm Hg, 92.8 / min, and 9.8 g / dL respectively. In 58.5% of the patients, co-morbid diseases were detected. The most common co-morbid diseases were hypertension (32.8%), diabetes mellitus (16.4%) and coronary artery disease (14.4%). Either diagnostic or therapeutic endoscopy was performed within the first 24 hours in 95.9% of the patients. The most common cause of bleeding was duodenal ulcer (34.2%), the second was erosive gastritis (29.4%) and third was gastric ulcer (24%). Peptic ulcer disease was detected in 55% of the patients. The most common Forrest Classification in patients with peptic ulcer was, Forrest 3 (57.3%). Helicobacter Pylori was detected in 82.2% of the patients with gastric ulcer and in 90.6% of the patients with duodenal ulcer. The mean hospital stay was 6.7 days. 63.1% of the patients received blood transfusion during hospital stay (mean 1.9 units). 3.6% of the patients consulted and transferred to the department of general surgery. In 6.7% of cases, re-bleeding was observed. Mortality was 3.6%. All of the patients who died were over 60 years of age. Statistically significant relationship was found between age and mortality (p<0.05).As a result, we have determined that non-variceal UGS bleeding is still an important issue and important cause of the mortality and morbidity, that the rate of drug use which may cause non-variceal upper gastrointestinal bleeding and occurrence of co-morbid diseases are high and that presence of co-morbid disease and mortality increased with age. Therefore, elderly patients and patients with co-morbid diseases should be treated more carefully and be monitored under intensive care as needed.

Üst gastrointestinal sistem (GİS) kanamaları, özofagusun üst kısmı ile Treitz ligamanı arası herhangi bir yerden lümen içine olan kanamaları kapsar. Üst GİS kanamaları teşhis ve tedavideki yeniliklere rağmen hala tüm dünyada önemli bir morbidite ve mortalite sebebi olan, sık görülen önemli bir sağlık problemidir.Çalışmamızda varis dışı üst GİS kanamalı hastaların özelliklerini belirlemek amaçlandı. Bu amaçla Ocak 2010 - Ocak 2011 tarihleri arasında Atatürk Üniversitesi Tıp Fakültesi Araştırma Hastanesi Gastroenteroloji Kliniği'ne varis dışı üst GİS kanaması sebebi ile yatırılan veya yatarken varis dışı üst GİS kanaması geçiren 19-88 yaş arası toplam 195 hasta prospektif olarak değerlendirildi.Bu çalışmada varis dışı üst GİS kanamalı hastaların %35,9'u kadın, %64,1'i erkek, erkek/kadın oranı 1,7 olarak bulundu. Hastaların yaş ortalamasının 58,6 olduğu ve bu hastaların %52,8'inin 60 yaşın üzerinde olduğu görüldü. Üst GİS kanamaya predispozan olabilecek ilaç kullanım oranı %52,8 olarak bulundu. Hastaların kullandığı ilaçlar sorgulandığında, %20,5'inde nonsteroid antiinflamatuar ilaç (NSAİİ), %17,4'ünde asetilsalisilik asit (ASA) kullanımı tespit edildi. Hastaların alışkanlıkları, sigara kullanımı %34,9, sigara ve alkolün birlikte kullanımı %5,1 idi. Üst GİS kanamalı hastalarda en sık başvuru şikayeti %45,1 ile melena, %37,4 ile melena ve hematemez bulundu. Hastaların hastaneye başvuru anındaki ortalama sistolik kan basıncı 108,6 mmHg, ortalama diastolik kan basıncı 68,7 mmHg, ortalama nabız sayısı 92,8 /dk, ortalama hemoglobin değeri 9,8 gr/dl olarak bulundu. Hastaların %58,5'inde ek hastalık tespit edildi. En sık rastlanan ek hastalık %32,8 ile hipertansiyon, %16,4 ile diabetes mellitus ve %14,4 ile koroner arter hastalığı olarak bulundu. Hastaların %95,9'una tanı ve tedavi amacıyla ilk 24 saat içinde endoskopi yapıldı. Hastalarda kanama sebebi olarak en sık %34,2 ile duodenal ülser, ikinci sırada %29,4 ile eroziv gastrit, üçüncü sırada % 24 ile gastrik ülser tespit edildi. Peptik ülser %55 oranında bulundu. Peptik ülserli hastalar Forrest Sınıflamasına göre incelendiğinde en sık Forrest 3, %57,3 oranında bulundu. Gastrik ülserli hastaların %82,2 sinde, duodenal ülserli hastaların %90,6'sında Helikobakter Pilori pozitifliği mevcuttu. Ortalama hastanede yatış süresi 6,7 gün olarak saptandı. Hastaların %63,1'ine kan transfüzyonu yapıldı (ortalama 1,9 ünite). Hastaların %3,6'sı genel cerrahi kliniğine devredildi. Yeniden kanama %6,7'sinde görüldü. Mortalite %3,6 olarak bulundu. Ölen hastaların hepsi 60 yaşın üzerinde idi. Yaş ile mortalite arasında istatistiksel olarak anlamlı ilişki tespit edildi (p<0,05).Sonuç olarak yapmış olduğumuz bu çalışmada varis dışı üst GİS kanamalarının halen önemli bir mortalite ve morbidite sorunu olduğunu saptadık. Varis dışı üst GİS kanamaya sebep olabilecek ilaçların kullanım oranının, ek hastalıkların birlikteliğinin yüksek olduğunu, yaşlılarda sık görüldüğünü ve yaşla birlikte mortalitenin arttığını tespit ettik. Bu yüzden yaşlılarda ve ek hastalığı olanlarda daha dikkatli davranılmalı, hastalar gerektiğinde yoğun bakım koşullarında takip edilmelidir.
