In infectious diseases, the presciption of appropriate antibiotic is needed due to not only for therapeutic purpose but also reduce the risk of developing resistance. Until recently, effective antibiotics were available for he treatment of resistant microorganisms, whereas today, antibiotherapy is often ineffective because of rapidly developing resistance. In population and hospital-acquired infections, primarily, it is extremely important that making true decision to whether or not antibiotic use is mandatory and if so, which one is the most accurate antibiotic for using.In our study, we aimed to investigate the distribution of gram-negative bacilli in population and hospital acquired infections and their resistance status to quinolones. For this purpose, we isolated gram negative bacilli from the specimens coming from in-patient and out-patient departments to microbiology laboratory, and defined by various methods. Antibiotic sensitivity testing according to CLSI criterions by Kirby-Bauer disc diffusion system was applied to 196 samples defined as genus or species for testing quinolone sensitivity. For this purpose ciprofloxacin, levofloxacin, ofloxacin, norfloxacin nalidicsic acid, pefloxacin and moksifloksasin our used.In 196 samples 122 were obtained from hospitalised patients whereas 74 were obtained from out-paitents. One hundred and sixt of those were found as population, 90 were hospital based microorganisms. One hundred and twenty specimens were taken from the children under sixteen. One hundred and sixteen samples were isolated from urine which most frequently coming to laboratory and mostly infectious agents isolated specimen. Seventy nine of samples included in the study were defined as E. Coli, 39 were Enterobacter, 36 were Pseudomonas, 15 were Salmonella, 13 were Citrobacter, 11 were Proteus, 5 were Acinetobacter, 5 were Stenotropomonas and 2 were Shigella.With regard to quinolone resistance, the most resistant microorganism was pseudomonas whereas the most sensitive ones were Enterobacter, Proteus, Citrobacter, Salmonella and Shigella. From the quinolones, the most effective quinolone against to population based microorganism was moxifloxacine whereas the most ineffective one was nalidixic acid. On the other hand, in hospital acquired microorganisms, the most effective quinolones were levofloxacine, ofloxacine and perfloxacine which were equally efective whereas the most ineffective one was also nalidixic acid.As a conclusion, although quinolones are effective and reliable antibiotics in case of gram negative enteric bacterial infections whereas they are not effective against non fermentative microorganisms.

İnfeksiyonlarda sadece tedavi amaçlı değil, antibiyotiğin bakteri direncini artırabileceği düşünülerek uygun olan antibiyotiği reçetelemek gerekmektedir. Kısa zaman öncesine kadar dirençli mikroorganizmaları tedavi edecek antibiyotikler olmasına rağmen, günümüzde bakteri direnci hızla geliştiği için artık antibiyotik tedavisi yetersiz kalmaktadır. Toplum ve hastane kökenli infeksiyonlarda öncelikli olarak antibiyotik kullanımının gerekliliğine, eğer gerekli ise hangi antibiyotiğin kullanılacağına doğru karar vermek gerekir.Biz çalışmamızda toplum ve hastane kökenli infeksiyonlarda gram olumsuz çomakların dağılımını ve kinolonlara direncini araştırmayı amaçladık. Bunun için hastanemiz Mikrobiyoloji laboratuvarına ayaktan ve yatan hastalardan gelen materyallerden gram olumsuz çomakları çeşitli yöntemlerle izole ettik ve isimlendirdik. Cins veya tür seviyesinde tanımladığımız 196 örneği CLSI kriterleri doğrultusunda Kirby-Bauer disk difüzyon yöntemi ile kinolonlara duyarlılıklarını araştırmak amacıyla antibiyotik duyarlılık testine aldık. Bunun için siprofloksasin, levofloksasin, ofloksasin, norfloksasin, nalidiksik asit, pefloksasin ve moksifloksasin antibiyotiklerini kullandık.Elde ettiğimiz 205 örneğin 122'si yatan hastalardan 74'ü poliklinik hastalarından gelmiş olup, bunların 106'sı toplum, 90'ı hastane kökenli bulunmuştur. Ayrıca materyalin 120'si 16 yaş altı çocuklara aitti. 115 örnek laboratuvara en fazla gelen ve infeksiyonların en sık izole edildiği idrardan elde edildi. Çalışmaya aldığımız bu örneklerin 79'u E.coli, 39'u Enterobacter spp, 36'sı Pseudomonas aeruginosa, 8'i Salmonella spp, 13'ü Citrobacter freundii, 11'i Proteus spp, 5'i Acinetobacter spp, 3'ü Stenotrophomonas maltophilia ve 2'si de Shigella flexneri olarak tanımlandı.Kinolonlara direnç durumlarına göre ise en dirençli Pseudomonas, en duyarlı olan mikroorganizmalar ise Enterobacter, Proteus, Citrobacter, Salmonella ve Shigella'dır. Kinolonlardan toplum kökenli mikrorganizmalara en etkili antibiyotik moxifloksasin, en etkisiz olanı ise nalidiksik asittir. Hastane kökenli olanlarda ise en etkili olan antibiyotik etki dereceleri aynı bulunan levofloksasin, ofloksasin ve pefloksasin, en etkisiz olanı ise yine nalidiksik asit olmuştur.Sonuç olarak gram negatif enterik bakteriler söz konusu olduğunda kinolonlar güvenilir bir seçenek olmasına rağmen, non fermentatifler için kinolonlar etkili antibiyotikler değildi.
