Aim: The aim of this study was to develop the nanoparticles and ODT formulations containing KBZ and LEV combination (KBZ+LEV) for the treatment of epilepsy and to investigate the in vitro characteristics of these formulations. It is also to evaluate the antiepileptic effects of the formulations in vivo in a rat epilepsy model.Material and Method: HPLC-UV method was used for the determination of KBZ and LEV and also the analytical method was validated. KBZ+LEV-PLGA-NPs were prepared using nanoprecipitation method. Besides, B-PLGA-NPs formulation without active substance was also prepared. In vitro characterization studies (surface morphology, particle size and distribution, ZP, EE, DL and in vitro release properties) were performed for all prepared nanoparticle formulations. On the other hand, ODT formulations were prepared by lyophilization (L-ODT) and direct compression (DC-ODT) methods. The uniformity of mass, content uniformity, diameter, thickness, the resistance to crushing, friability, disintegration time, dispersion time, dissolution properties, wetting time and water absorption capacity of ODT formulations were examined. In addition, FT-IR and DSC analyzes were performed for all formulations. Subsequently, the antiepileptic effects of KBZ+LEV-PLGA-NPs and DC-ODT formulations were investigated PTZ-induced epilepsy model in rats.Results: It was determined that the prepared nanoparticles were approximately spherical shape. The particle size and ZP values of B-PLGA-NPs and KBZ+LEV-PLGA-NPs were found as 171.92±4.637 nm and 180.62±6.260 nm (p>0.05), -20.28±2.992 mV and -27.08±3.110 mV (p<0.05), respectively. The EE % and DL % values of the KBZ+LEV-PLGA-NPs formulation were %51.005.944 ve %3.890.429 for KBZ and %2.050.367 ve %0.160.026 for LEV, respectively. On the other hand, the uniformity of mass, content uniformity, friability and dissolution test results of the ODT formulations were determined within the pharmacopoeia limits. In addition, the disintegration times of L-ODT and DC-ODT formulations were determined as 1.550.344 sn ve 31.507.477 sn, respectively, and it was fulfilled a requirement of disintegration time of less than 3 min for ODTs according to the European Pharmacopoeia. The test results of dispersion time, wetting time and water absorption capacity for ODTs were found to be consistent with the literature. There was a significant decrease in the seizure scores for the KBZ+LEV group compared to the KBZ and LEV groups (p<0.05). KBZ+LEV-PLGA-NPs and DC-ODT formulations showed similar antiepileptic activity with KBZ+LEV group.Conclusion: Nanoparticles and ODT formulations containing KBZ+LEV were successfully prepared. The use of the formulations containing KBZ+LEV may be useful in the treatment of epilepsy.Key Words: Carbamazepine, epilepsy, levetiracetam, nanoparticle, orally disintegrating tablet.

Amaç: Bu çalışmanın amacı, epilepsi tedavisine yönelik KBZ ve LEV'i bir arada (KBZ+LEV) içeren nanopartikül ve ADT formülasyonlarını geliştirmek ve bu formülasyonların in vitro özelliklerini incelemektir. Ayrıca, epilepsi modeli oluşturulmuş sıçanlarda formülasyonların antiepileptik etkilerini in vivo olarak değerlendirmektir. Materyal ve Metot: KBZ ve LEV'in miktar tayini için HPLC-UV yöntemi kullanılmış ve analitik yöntemin geçerliliği gösterilmiştir. KBZ+LEV-PLGA-NPs nanopresipitasyon yöntemi kullanılarak hazırlanmıştır. Ayrıca, etkin madde içermeyen B-PLGA-NPs formülasyonu da hazırlanmıştır. Hazırlanan tüm nanopartikül formülasyonları için in vitro karakterizasyon çalışmaları (yüzey morfolojisi, partikül büyüklüğü ve dağılımı, ZP, EE, YK ve in vitro salım özellikleri) gerçekleştirilmiştir. Diğer taraftan, ADT formülasyonları liyofilizasyon (L-ADT) ve direkt basım (DB-ADT) yöntemleri ile hazırlanmıştır. ADT formülasyonlarının kütle tekdüzeliği, içerik tekdüzeliği, çapı, kalınlığı, kırılmaya karşı direnci, friyabilitesi, dağılma süresi, dispersiyon süresi, çözünme özellikleri, ıslanma süresi ve su absorplama kapasitesi incelenmiştir. Ayrıca, tüm formülasyonlar için FT-IR ve DSC analizleri de yapılmıştır. Daha sonra, PTZ ile sıçanlarda epilepsi modeli oluşturularak KBZ+LEV-PLGA-NPs ve DB-ADT formülasyonlarının antiepileptik etkileri incelenmiştir.Bulgular: Hazırlanan nanopartiküllerin küresele yakın bir şekle sahip olduğu saptanmıştır. B-PLGA-NPs ve KBZ+LEV-PLGA-NPs'nin partikül büyüklüğü ve ZP değerleri sırasıyla 171.92±4.637 nm ve 180.62±6.260 nm (p>0.05), -20.28±2.992 mV ve -27.08±3.110 mV (p<0.05) olarak saptanmıştır. KBZ+LEV-PLGA-NPs formülasyonunun % EE ve % YK değerleri sırasıyla KBZ için %51.005.944 ve %3.890.429; LEV için %2.050.367 ve %0.160.026'dır. Diğer taraftan, hazırlanan ADT formülasyonlarının ise kütle tekdüzeliği, içerik tekdüzeliği, friyabilite ve çözünme test sonuçları farmakope limitleri içerisinde tespit edilmiştir. Ayrıca, L-ADT ve DB-ADT formülasyonlarının dağılma süreleri sırasıyla 1.550.344 sn ve 31.507.477 sn olarak saptanmış ve Avrupa Farmakopesi'ne göre ADT'lerin 3 dk.'dan daha kısa sürede dağılması gerekliliği karşılanmıştır. ADT'ler için yapılan dispersiyon süresi, ıslanma süresi ve su absorplama kapasitesi tayini bulguları ise literatürlerle uyumlu bulunmuştur. KBZ ve LEV grupları ile karşılaştırıldığında KBZ+LEV grubu için nöbet skorlarında anlamlı bir düşüş saptanmıştır (p<0.05). KBZ+LEV-PLGA-NPs ve DB-ADT formülasyonları ise KBZ+LEV grubu ile benzer antiepileptik aktivite göstermiştir. Sonuç: KBZ+LEV içeren nanopartikül ve ADT formülasyonları başarılı bir şekilde hazırlanmıştır. KBZ+LEV içeren formülasyonların kullanılması epilepsi tedavisinde faydalı olabilir.
