ABSTRACTInvestigation of the Antifungal Activity of Henna (Lawsonia inermis) on Dermatophytes Isolated From Patients in Erzurum and Region Dermatophytes are the most important fungus species that may lead superficial mycotic infections on the skin tissue. Determination of the regional flora and dermatophytosis agents is important in terms of effective treatment, prevention methods and epidemiology. Extensive studies about finding the novel antifungal substance was carried out, because of some problems encountered the treatment of dermatophytes and the high tendency to chronicity. It is known that henna (Lawsonia inermis) is used among the people against fungal diseases. In this study, we aimed to investigate the antifungal activity of L. inermis on the causative dermatophytes and determine the dermatophytosis agents of Erzurum region.Direct microscopic examinations were performed on the skin and nail scrapings and hair samples that were treated with KOH and obtained from the patients referred to Ataturk University, Medical Faculty, Department of Dermatology. Native preparation positive samples were cultured with mycobiotic agar, Saboraud dextrose agar (SDA) and potato dextrose agar (PDA) mediums. The identification of dermatophytes was performed on the species level by observing the microscopic and macroscopic structures of colonies in culture media. The antifungal activity of L. inermis against dermatophytes was investigated by agar diffusion method after the mixture of 10 gr L. inermis with 30 ml steril saline was filled into the wells prepared into the SDA plate, sterilized by autoclave. The inhibition zone of L. inermis against dermatophytes was measured and noted in millimeters.Dermatophytes were isolated from 54 (%39.1) of 138 samples which are native preparation positive. The most frequently diagnosed dermatophyte was Trichophyton rubrum The detection rates of other species such as Trichophyton mentagrophytes, Trichophyton tonsurans, Trichophyton violaceum, Microsporum audounii, Microsporum canis and Epidermophyton floccosum were %9.2, %3.7, %1.9, %1.9, %1.9 and %1.9 respectively.The percentages of Tinea pedis, Tinea unguium, Tinea corporis, Tinea inguinalis Tinea capitis and Tinea manum were %60.1, %28.2, %4.3, %2.9, %2.1 and %2.1 respectively by the prevalence of our patient?s clinical diagnosis. The most common one that was obtained clinically was T. pedis.The anti-dermatophytic activity of L. inermis was evaluated against dermatophytes. The average inhibition zone diameters of L. inermis were 34.7 mm for 43 T. rubrum, 27.6 mm for 5 T. mentagrophytes and 30 mm for 2 T. tonsurans. The inhibition zone diameters of L. inermis against other species such as 30 mm for M. audouinii, 23 mm for M. canis and 42 mm for E. floccosum were detected.It was proved that our findings are close to other findings obtained from the other regions of our country and detected that there were any apparent regional differences by the way of dermatophytes prevalence. We also detected that L. inermis has an anti-dermatophytic activity and can create an effective inhibition zone diameters against dermatophytes under in-vitro conditions. Finally, we concluded that L. inermis may be an alternative to antifungal drugs by further research.Key words: dermatophytes, dermatophytosis, Lawsonia inermis, henna.

ÖZETErzurum ve Çevresindeki Hastalardan İzole Edilen Dermatofitler Üzerine Kınanın (Lawsonia inermis) Antifungal Aktivitesinin AraştırılmasıDeride yüzeyel mikotik enfeksiyon yapan en önemli mantar türü dermatofitlerdir. Dermatofitoz etkenlerinin ve bölgesel floranın saptanması epidemiyoloji, korunma yöntemleri ve etkin tedavi açısından önemlidir. Dermatofitlerin, kronikleşme eğiliminin yüksek olması ve tedavisinde karşılaşılan çeşitli problemlerden dolayı yeni antifungal maddelerin bulunması konusunda yoğun çalışmalar yapılmaktadır. Kınanın (Lawsonia inermis) halk arasında mantar hastalıklarında kullanıldığı bilinmektedir. Biz de çalışmamızda Erzurum bölgesinin dermatofitozis etkenlerinin belirlenmesi ve L. inermis?in bu etken dermatofitler üzerine antifungal aktivitesini araştırmayı amaçladık. Atatürk Üniversitesi Tıp Fakültesi Dermatoloji Anabilim Dalı Polikliniği?ne başvuran, dermatofitoz ön tanılı hastalardan alınan deri ve tırnak kazıntısı, saç ve kıl örneklerinin KOH ile hazırlanan preparatlarında direk mikroskobik incelemeleri yapıldı. Nativ preparatı pozitif olan örnekler kültür için mycobiotic agar, Saboraud dextroz agar (SDA) ve patates dekstroz agara (PDA) ekildi. Besiyerlerinde üreyen kolonilerin makroskobik ve mikroskobik yapıları incelenerek dermatofitlerin tür düzeyinde identifikasyonu yapıldı. L. inermis?in 10 gramı ile 30 ml steril serum fizyolojikten oluşan karışım otoklavda steril edildikten sonra, SDA plağında hazırlanan kuyucuklara doldurularak agar difüzyon yöntemiyle dermatofitlere karşı antifungal aktivitesi araştırıldı. L. inermis?in dermatofitlere karşı oluşturduğu inhibisyon zonunun çapı ölçülerek mm olarak kaydedildi.Nativ preparatı pozitif 138 örneğin 54?ünden (%39.1) dermatofit izole edildi. Trichophyton rubrum %79.6 oranı ile dermatofitler arasında en sık tespit edilendi. Diğer türler sırasıyla Trichophyton mentagrophytes %9.2, Trichophyton tonsurans %3.7, Trichophyton violaceum %1.9, Microsporum audounii %1.9, Microsporum canis %1.9, Epidermophyton floccosum %1.9 oranıyla tespit edildi.Hastalarımızın klinik ön tanılarına göre dağılımı, Tinea pedis %60.1, Tinea ungium %28.2, Tinea corporis %4.3, Tinea inguinalis %2.9, Tinea capitis %2.1 ve Tinea manum %2.1 oranlarında idi. Klinik olarak en fazla T. pedis gözlendi. Üreyen dermatofitlere karşı L. inermis?in antidermatofitik aktivitesi değerlendirildi. L. inermis?in inhibisyon zon çapları ortalama 43 T. rubrum için 34.7 mm, 5 T. mentagrophytes için 27.6 mm, 2 T. tonsurans için 30 mm olarak belirlendi. L. inermis?in diğer türlere karşı oluşturduğu inhibisyon zon çapları ise M. audouinii için 30 mm, M. canis için 23 mm, E. floccosum için 42 mm olarak ölçüldü.Elde ettiğimiz bulgularımızın dermatofitlerin etyolojisi, epidemiyolojisi ve demografisi açısından ülkemizdeki diğer bölgelerle yakın olduğu görüldü ve dermatofit dağılımında bariz bir bölgesel farklılık olmadığı tespit edildi. L. inermis?in dermatofitlere karşı in-vitro koşullarda efektif bir inhibisyon zon çapı oluşturduğunu ve antidermatofitik aktivitesinin olduğu saptandı.
