In view of statistic data, migraine is the most common neurologic reason among headache disorders seeking medical advice. Significant developments have been seen in the area of headache and especially migraine genetics research. In recent years migraine is accepted to be a complex and multigenic disease caused by the interaction of diverse genetic and environmental factors. There is some evidence that genetic factors play a more important role in migraine with aura compared to migraine without aura. GABA is a major central neurotransmitter that controls neuronal excitability and network interactions in the cerebral cortex. A study by Plummer et al showed a significant down regulation of GABA genes among patients with migraine. This finding led to the thought that the course of migraine headache could be affected by the use of GABA receptor agonists and by drug linking GABA A receptor. With the working hypothesis that AED resistance is a polygenic disorder in which several genetic variants with a modest effect interact with each other Kim et all used multifactor dimensionality reduction-MDR, a new statistical method for genetic association and aimed to find pharmacogenetic evidence of epistatic interactions that underlies AED. That study with the MDR analysis method indicated that the rs12658835 and rs35166395 SNPs in the gene GABRA 1 were statistically significant in AED.This study has a more cautious approach towards the positive relation between SPN and the GABRA 1 gene, especially with patients who had migraine with aura. This study therefore aimed at exploring an association between selected gen polymorphisms and the disease. Examining rs12658835 among 108 study subjects with migraine with aura and 107non-headache healthy control subjects %7 homozygous mutant type genotype and %93 homozygous-wild type genotype were observed among the subjects with migraine with aura. All the subjects in the control group had wild type homozygous genotype. From the comparison between both groups the p value of 0.007 was found significant among the patients with migraine with aura. %60 wild type genotype %16 mutant type genotype %24 heterozygous type genotype were observed in examining the patients for (p<0.05) rs35166395. Among the control group %65 wild type genotype %13homozygous mutant type genotype %22 heterozygous type genotype were observed. The comparison between the two study groups showed no statistically significance (p>0.05). As a result the rs12658835 of the SNP's associated with GABRA 1 have a tendency towards migraine with aura. On the other hand, rs35166395 was observed to be widespread in local societies but it can be claimed that it is not associated with migraine with aura.

Migren, istatistiki veriler göz önüne alındığında tüm baş ağrısı hastalıkları içinde doktora en fazla başvuru nedeni olan nörolojik hastalıktır. Günümüzde baş ağrısı, özellikle de migren genetiği konusunda önemli ilerlemeler olmuştur. Son yıllarda migren; çevresel faktörlerle genetik faktörlerin etkileşim gösterdiği kompleks ve çok genli bir hastalık olarak kabul edilmektedir. Aurasız migrenle kıyaslandığında, auralı migrende genetik faktörlerin daha fazla rolü olduğu konusunda çeşitli bulgular vardır. GABA; beynin serebral korteksindeki ağ etkileşimleri ve nöron uyarısını kontrol eden majör bir santral sinir sitemi nörotransmiteridir. GABA genleri üzerine yapılan bir çalışmada migrenli hastalarda GABA genlerinde down regülasyon gösterilmiştir. Bu durum migren tedavisinde GABA reseptör agonistlerinin kullanılması ve ilgili ilaçların GABA A reseptörüne bağlanmasıyla migren sürecininin etkilenebileceği fikrini ortaya çıkarmıştır. Antiepileptik ilaç direncinin, herbirinin bir diğeri üzerine düşük seviyede etkileşimi olan birkaç genetik çeşitlilikteki çok genli düzensizlik olduğu hipotezinin çalışıldığı başka bir çalışmada ise, yeni bir istatistik yöntem olan çok faktörlü boyut indirgeme analizi (Multifactor Dimensionality Reduction-MDR) kullanılmıştır. Bu çalışmada antiepileptik ilaç direncinin altında yatan epistatik etkileşimlerin farmakogenetik kanıtının bulunması amaçlanmıştır. Bu çalışmanın sonucu olarak GABRA 1 genindeki rs12658835 ve rs35166395 SNP?leri MDR analizi yöntemiyle antiepileptik ilaç direncinde yüksek düzeyde anlamlı bulunmuştur. Bizim yaptığımız çalışmada, auralı migrenli hastalarda GABRA 1 geni ilişkisi açısından anlamlı çıkan SNP?ler de aynı şekilde şüpheli bulunarak seçilmiş,gen polimorfizminin araştırılması ve bulunacak olan sonuçlarla hastalık arasındaki ilişkinin incelenmesi amaçlanmıştır. Toplam 108 auralı migren ve 107 sağlıklı kontrol grubunun ele alındığı çalışmamızda rs12658835 incelemesinde hasta gurubunun %7?sinde homozigot mutant tip genotip, %93?ünde homozigot wild tip genotip tespit edildi. Kontrol gurubunda ise tüm bireyler wild tip homozigot genotipe sahipti. Hasta ve kontrol gurubu kıyaslandığında p değeri 0,007 çıkarak hasta grubu tarafına anlamlı bulundu.(p<0,05) rs35166395 için yapılan incelemede hasta grubunun %60'ında wild tip genotip,%16'sında homozigot mutant tip genotip, %24'ünde heterozigot tip genotip tesbit edildi. Kontrol gruplarında ise %65 wild tip genotip %13 homozigot mutant tip genotip,%22 heterozigot tip genotip tesbit edildi. Hasta ve kontrol gurubu kıyaslandığında istatistiksel bir fark bulunamadı. (p>0.05) Sonuç olarak GABRA 1 geni ile ilgili SNP'lerden rs12658835'in auralı migrene yatkınlık oluşturduğu, rs35166395'in ise toplumumuzda yaygın olduğu fakat auralı migren ile ilişkilendirilemeyeceği söylenebilir.
