Abstract

The purpose of the study was to evaluate the influence of the PRF (Platelet Rich Fibrin), CGF (Concentrated Growth Factors) and A-PRF (Advanced Platelet Rich Fibrin) with or without autogenous bone(AB) on bone healing in surgically created critical-size defects (CSD) in the rat calvaria, by histological and stereological analysis. Eighty animals of 8 groups were used for surgical procedure, 15 rats used for cardiac puncture. A semilunar incision was made in the scalp on the anterior region of the calvarium. A 5-mm diameter CSD was created with a trephine bur. Defects of control group was left empty. Other groups were filled with autogenous bone, PRF, A-PRF, CGF, PRF with autogenous bone, A-PRF with autogenous bone and CGF with autogenous bone. The animals were sacrificed at 4 weeks. New bone formation was assessed by stereological and radiological analysis. The total percentage of newly formed bone was lowest in group empty and highest in group A-PRF with autogenous bone. Mean newly formed bone volumes between the A-PRF with autogenous bone group and autogenous bone group was statistically significant. Besides that, the difference between the A-PRF+AB group and PRF+AB group wasn't statistically significant in terms of the newly formed bone volume. Addition of A-PRF, PRF, and CGF have had significantly increased the bone formation at the 4th week. The effect of A-PRF, PRF, and CGF was similar and may be useful in the future to increase the success rate of bone grafting.

Çalışmanın amacı, TZF (Trombositten Zengin Fibrin), KBF (Konsantre Büyüme Faktörü) ve G-TZF'nin (Geliştirilmiş Trombositten Zengin Fibrin) tek başına ya da otojen kemikle birlikte kullanımının, rat kalvaryumunda oluşturulan kritik boyutlardaki defektlerde yeni kemik oluşumu üzerindeki etkisinin radyolojik ve stereolojik olarak değerlendirilmesidir. Cerrahi prosedür için 8 grup oluşturulan 80 rat, kan alımı için ise 15 rat kullanılmıştır. Ratların kalvaryumunun ön tarafını ortaya çıkarmak için semilunar insizyonla flep kaldırılmıştır. Trefan frez yardımıyla 5 mm'lik kritik boyutlarda defektler oluşturulmuştur. Kontrol grubundaki defektler boş bırakılarak kapatılmıştır. Diğer gruplar sırayla otojen kemik, TZF, KBF, G-TZF, TZF ve otojen kemik, KBF ve otojen kemik, G-TZF ve otojen kemik ile doldurularak kapatılmıştır. Hayvanlar 4 hafta sonra sakrifiye edilmiştir. Yeni kemik miktarları stereolojik ve radyolojik olarak incelenmiştir. Yeni kemik dokusu oluşumu en az miktarda boş kontrol grubunda en yüksek oranda ise G-TZF ve otojen kemiğin birlikte yerleştirildiği grubunda gözlenmiştir. Bununla birlikte otojen kemik grubuyla G-TZF ve otojen kemiğin birlikte yerleştirildiği grup arasında istatistiksel açıdan anlamlı fark bulunmuştur. Ayrıca yeni kemik hacmi açısından G-TZF ve otojen kemik grubu ile TZF ve otojen kemik grubu arasında istatistiksel olarak anlamlı bir fark bulunmamıştır. Ayrıca defektlere G-TZF, KBF ve TZF'nin tek başına yerleştirilmesi 4. haftada belirgin bir şekilde kemik oluşumunu arttırdı. Sonuç olarak gelecekte G-TZF, KBF ve TZF, otojen kemik üzerindeki benzer olumlu etkileri sayesinde kemik greftleme işlemlerinin başarısını arttırmak için kullanımı yaygınlaşabilir.
