We aimed to investigate the effects of preoperative anxiety and the anesthetic agents on the awareness of the 90 pregnant women undergoing elective caesarean operation with general anesthesia. All patients were implemented Beck Anxiety Inventory Test to assess the anxiety attitude in the preoperative period. Patients were randomized and divided into three groups. Routine hemodynamic and Bispectral index (BIS) monitorization were performed. Pneumatic tourniquet to the arm which has no vascular access was applied for isolated forearm (IFA) test. Anesthesia induction was provided using in Group P with propofol 2-2.5mg/kg, in Group T with thiopental 5mg/kg and in Group K with ketamine 1mg/kg. After loss of consciousness, the tourniquet was filled and succinylcholine bolus was implemented. Anesthetic maintenance in all groups was provided using 2% sevoflurane. After intubation, BIS and hemodynamic parameters and end-tidal sevoflurane (Etsevo) concentrations and IFA test response were recorded intermittently. Patient's Numeric Rating Scale (NRS) at the postoperative period and first analgesic requirement time were recorded. The questions in the Modified Brice Scala (MBS) were asked for postoperative awareness in the postoperative 1sth, 3rd h, 1st and 3rd day. Etsevo values reached %1.2, seven minutes after intubation for all study groups. Although BIS values for Group K were measured higher than other groups, the results of IFA test response were lower. However, we observed that according to the answers of the Group K patients to the MBS, they had much more dreaming and hearing voices rates comparing other groups. Mean arterial pressure levels in Group T and also 0.hour NRS values were founded higher than other groups, in addition to that the time of first analgesic application time was shorter. The responses for IFA test in Group P were less than Group T. Preoperative anxiety levels of all groups were similar and at low levels so we assumed that anxiety had no influence on awareness. Regarding all these factors, assessments of hemodynamic parameters, IFA test, the use of BIS, measuring of Etsevo concentrations, MBS may not sufficient to decide which anesthetic agent could be better, as describing intraoperative awareness and we assumed that interviewing patients in late postoperative period could be more convenient to detect awareness and application of propofol for induction could cause less awareness.

Preoperatif anksiyetenin ve anestezik ajanların genel anestezi ile elektif sezaryen operasyonu geçirecek 90 gebe kadında farkındalık üzerine etkilerinin araştırılmasını amaçladık. Tüm hastalara preoperatif anksiyete durumunu değerlendirmek için Beck Anksiyete Değerlendirme Ölçeği uygulandı. Hastalar rastgele 3 eşit gruba ayrıldı. Rutin hemodinamik monitörizasyon, Bispektral indeks (BİS) mönitorizasyonu uygulandı. Damar yolu olmayan kola izole ön kol (İÖK) testi amacıyla pnömotik turnike yerleştirildi. Anestezi indüksiyonu Grup P'de propofol 2-2,5 mg/kg, Grup T'de tiyopental 5 mg/kg ve Grup K'da ketamin 1mg/kg ile sağlandı. Bilinç kaybı sonrası pnömotik turnike şişirildi ve süksinilkolin bolus uygulandı. Anestezi idamesi tüm gruplarda %2 sevofluran ile sağlandı. Entübasyon sonrası BİS ve hemodinamik veriler, end-tidal sevofluran (Etsevo) değerleri ve İÖK test yanıtları aralıklı olarak kaydedildi. Postoperatif Sayısal Ağrı Skalası (NRS) değerleri ve ilk analjezik yapılma zamanı kaydedildi. Farkındalığı anlamaya yönelik postoperatif 1. s, 3. s, 1. ve 3. gün Modifiye Brice Skalası'ndaki (MBS) sorular soruldu. Etsevo değerleri tüm gruplarda entübasyondan 7 dk sonra %1.2 değerlerine ulaştı. Grup K'da BİS değerleri diğer iki gruptan yüksek seyrederken İÖK testine verilen yanıtlar daha azdı. Ancak Grup K'da MBS sorularına verilen yanıtlarda diğer gruplara oranla daha fazla rüya görüldüğü ve ses duyulduğu belirlendi.Grup T'de ortalama arter basıncı seviyeleri ve 0. saat NRS değerleri diğer gruplardan yüksek seyretti, ilk analjezik yapılma zamanı da daha kısa bulundu.Grup P'de İÖK testine verilen yanıtlar Grup T'den daha azdı.Grupların preoperatif anksiyete seviyeleri benzer ve düşük bulundu. Bu nedenle anksiyetenin farkındalık üzerine etkisinin olmadığı kanısına varıldı.Tüm bu etkenler göz önüne alındığında sezaryen operasyonlarında anestezik ajanların farkındalık üzerine etkilerinin araştırılmasında intraoperatif farkındalığı tanımlamada tek başına hemodinamik parametreler, İÖK testi, BİS kullanımı, Etsevo konsantrasyonlarının ölçümü, MBS değerlendirmelerinin hangi anestezik ajanın daha iyi olduğunu belirlemede yeterli olamayabileceği, farkındalık tespiti için postoperatif daha geç dönemlerde de görüşme yapılmasının uygun olacağı, indüksiyonda propofol uygulamasının daha az farkındalığa neden olabileceği kanısına varılmıştır.
