The relationship between SCUBE-2 levels and coronary artery disease severity and coronary collateral vessel development in patients with acute coronary syndrome in emergency department.Objective: SCUBE-2 [signal peptide-CUB (complement C1r / C1s, UEGF, and Bmp1) -EGF (epidermal growth factor) -like domain-containing protein-2] is a cell surface protein of the EGF superfamily and is involved in development, organogenesis and branching morphogenesis. There are studies stating that it plays a role in the development and progression of atherosclerosis in human coronary arteries and is also effective in the process of angiogenesis. There is no study in the literature on the relationship between coronary artery disease (CAD) and coronary collateral circulation (CCC) and SCUBE-2. Since SCUBE-2 plays a role in atherosclerosis and angiogenesis process; the aim of this study was to investigate the relationship between SCUBE-2 levels and the severity of CAD and CCC existence.Methods: Our study was conducted in the Emergency Medicine Clinic between 01.11.2018 and 01.05.2019 with 67 patients who were above 18 years of age undergoing coronary angiography by the Cardiology Clinic for the treatment of ACS. Blood samples taken for routine biochemistry tests in the emergency clinic were placed in a gel seperator biochemistry tube and after taking the amount required for the tests for the patient from the blood sample, the remaining part in the tube was centrifuged at 3000xg for 10 min and the serum was separated and stored at -800C until analysis. ELISA kit was used for the determination of SCUBE-2 levels. Coronary angiographies were evaluated and SYNTAX scoring was performed to determine the severity of CAD. The SYNTAX score group with a score of 0-22 was named as a low SYNTAX score group, and the score group with a score of ≥23 was named as a high SYNTAX score group. Rentrop classification was used to determine CCC. Patients with Rentrop grade 0 were grouped as no collateral flow group, and the patients with Rentrop grade 1-2-3 were grouped as collateral flow available group.Results: Serum SCUBE-2 levels were found to be significantly higher in the group with higher SYNTAX scores than the group with lower SYNTAX scores. (1092.31±1088.84 ng/L and 408.37±337.39 ng/L respectively p = 0.001). Similarly, serum SCUBE-2 levels were significantly higher in patients with collateral flow available group than the group with no collateral flow. (597,21±618,38 ng/L and 106,99±122,14 ng/L respectively p<0,001). In the ROC (Receiver operating characteristic Curve) analysis, the SCUBE-2 cut-off value for CAD severity was found to be ˃472,9185 ng/L. The sensitivity of this value in predicting the high SYNTAX score was 83.3% and the specificity was 61.8%. (AUC 0,801 (95% Confidence Interval (CI): 0,669-0,932). For the presence of CCC, the SCUBE-2 cut-off value was found to be ˃72,0265 ng/L. The sensitivity of this value in predicting the presence of CCC was 87.9% and the specificity was 66.7%. (AUC 0.875 (95% CI: 0.789-0.967).Conclusion: The relationship between serum SCUBE-2 levels and CAD severity and CCC has been investigated for the first time in the literature and it has been determined in this study that it can play a role in predicting both the severity of CAD and the presence of CCC. We believe that serum SCUBE-2 levels may be used as a biomarker in the future to determine the severity of CAD or to detect the presence of CCC. In addition, SCUBE-2, which has the potential to induce coronary collateral circulation, is also promising to be a potential treatment method.

Acil serviste akut koroner sendrom ön tanısıyla tetkik edilen hastalarda ölçülen scube-2düzeyinin koroner arter hastalığı ciddiyeti ve koroner kollateral damar gelişimi ile ilişkisiAmaç: SCUBE-2 [signal peptide-CUB (complement C1r/C1s, Uegf, and Bmp1)-EGF(epidermal growth factor)-like domain-containing protein-2] EGF süper ailesine ait birhücre yüzey proteinidir ve gelişme, organogenez ve dallanan morfogenezde önemli roloynar. İnsan koroner arterlerinde ateroskleroz gelişiminde, progresyonunda veanjiyogenezde rol oynadığını gösteren çalışmalar mevcuttur. Literatürde koroner arterhastalığı(KAH) ve koroner kollateral dolaşım(KKD) ile SCUBE-2 ilişkisi hakkında çalışmabulunmamaktadır. Bu çalışmada SCUBE-2'nin ateroskleroz ve anjiyogenezde roloynadığından yola çıkılarak KAH ciddiyeti ve KKD ile SCUBE-2 düzeyinin ilişkisininaraştırılması amaçlanmıştır.Gereç ve Yöntem: Çalışmamız 01.11.2018–01.05.2019 tarihleri arasında Acil TıpKliniği'nde AKS nedeniyle tetkik edilerek AKS ön tanısıyla Kardiyoloji Kliniği'ncekoroner anjiyografileri yapılan 18 yaş üzeri 67 hastayla yapılmıştır. Hastaların acilkliniğinde rutin biyokimya tetkikleri için jel seperatörlü biyokimya tüpüne alınan kanınhastanın istenen tetkikleri için gereken miktarı alındıktan sonra kalan kısım 3000xg'de 10dk santrifüje edilip serum ayrıldı ve çalışma gününe kadar -800C'de saklandı. SCUBE-2düzeyleri tayini için ELİSA kiti kullanıldı. Hastaların koroner anjiyografileri yorumlanarakKAH ciddiyetini belirlemek için SYNTAX skorlaması yapıldı. 0-22 puan arası düşükSYNTAX skor grubu, ≥23 puan ise yüksek SYNTAX skor grubu olarak gruplandı. KKDiçin Rentrop sınıflandırması kullanıldı. Rentrop grade 0 olan hastalar kollateral akım yok,Rentrop grade 1-2-3 olan hastalar kollateral akım var olarak gruplandı.Bulgular: Serum SCUBE-2 düzeyleri yüksek SYNTAX skorlu grupta düşük SYNTAXskorlu gruba göre istatistiksel olarak anlamlı şekilde yüksek bulundu. (Sırasıyla1092,31±1088,84 ng/L ve 408,37±337,39 ng/L p=0,001). Benzer şekilde serum SCUBE-2düzeyleri kollateral akım var olan grupta yok olan gruba göre anlamlı şekilde yüksekbulundu. Serum SCUBE-2 düzeylerinin KAH ciddiyetini tespit etmek ya da KKD varlığını saptamak amacıyla ileride bir biyomarker olarak kullanılabileceği düşüncesindeyiz. Bunun yanı sıra koroner kollateral dolaşımı indükleme potansiyeli olan SCUBE-2 potansiyel tedavi yöntemi olma konusunda da umut vermektedir.
