Araç kullanırken, yolun pürüzlülüğünden ve aracın motorundan kaynaklanan titreşimler başta koltuk olmak üzere araç ile temas edilen her bölge aracılığıyla insan vücuduna aktarılır. Tüm vücudu etkileyen bu tip titreşimlere tüm vücut titreşimi (whole body vibration) denir. Tüm vücut titreşimine maruz kalan insanların konforu ve sağlığı olumsuz biçimde etkilenebilir. Bu nedenle titreşimin insan vücudu üzerindeki etkisinin geniş kapsamlı olarak araştırılması önemli bir konudur. Titreşime maruz kalındığında omurga kasları aktifleşerek vücut stabilitesinin bozulmaması için çalışırlar. Kasların ani aktifleşmesi ya da yüksek yüke maruz kalması kaslarda yorulmaya ve omurgada dejenerasyona sebep olabilir. Tüm vücut titreşimi omurlarası disklerde baskının artmasına ve bu yüzden özellikle lumbar omurga bölgesinde ağrılara neden olabilir. Deneysel ölçümlerden elde edilen ivme, koltuk temas kuvveti ve kas aktivasyonu ile hesaplanan rezonans frekansı, geçirgenlik ve görünür kütle gibi fiziksel değerler titreşimin insan vücuduna etkisini araştırmak için kullanılmaktadır. İnsan vücudunda oluşabilecek dejenerasyonun biyomekanik açıdan değerlendirilmesi omurgada oluşan yüklerin tespiti ile yapılabilir. Fakat doğrudan ölçüm ile kaslarda kas kuvvetlerinin veya omurga yapılarındaki gerilmelerin ölçülmesi teknik ve etik açıdan çoğu zaman zorlayıcıdır. Omurga bölgesinde oluşabilecek dejenerasyon tespitinin zorluğundan ötürü matematik modellere başvurulmaktadır. Bu tez kapsamında titreşimin insan omurgası üzerindeki etkisinin biyomekanik açıdan değerlendirilmesi amaçlanmıştır. Bunu gerçekleştirmek için 12 sağlıklı gönüllüden titreşim ve elektromyografi (EMG) ölçümleri yapılmıştır. Matematik model girdilerinin oluşturulması ve analiz çıktılarının değerlendirilmesi için uygulanan deneysel çalışmada i) omurgayı saran lumbar multifidus, iliocostalis lumborum ve longissimus thoracic kaslarından yüzeyel EMG cihazı ile kas aktivasyonu, ii) pelvis, lumbar ve torakal omurlarına ve koltuk yüzeyine yerleştirilen ivmeölçerler ile ivme ve iii) kuvvet platformu ile gönüllünün koltuğa uyguladığı kuvvet ölçülmüştür. Dik oturma pozisyonu sırasında rijit koltuk aracılığı ile uygulanan düşey yöndeki titreşimlerin genlikleri 0.5, 1.0 ve 2.0 ms-2 RMS'dir. Titreşim sinyalinin frekansı 0.5 - 20 Hz arasında değişmektedir. Biyodinamik inceleme için EMG RMS, görünür kütle ve geçirgenlik değerleri hesaplanmıştır. OpenSim kas-iskelet sistemi modelinde statik optimizasyon yaklaşımı kullanılarak omurga bölgesi kaslarının titreşim esnasındaki aktivasyonu ve ürettiği kuvvet tahmin edilmiştir. Analiz sonucu elde edilen kas aktivasyonları, kas kuvvetleri ve eklem açısal hareketleri deneysel yolla elde edilen EMG verisi ve omur açısal hareketleri ile karşılaştırılmıştır ve modelin doğrulaması yapılmıştır. Sonrasında, bir gönüllüye ait bilgisayarlı tomografi verisi kullanılarak lumbar bölge ve pelvis-sakrumun sonlu elemanlar modeli oluşturulmuştur. OpenSim modeli kullanılarak hesaplanan kas kuvvetleri sonlu elemanlar modeline yükleme koşulları olarak uygulanmış ve omurlar ile omurlararası disklerdeki gerilme-genleme dağılımı hesaplanmıştır. Bu modelin doğrulaması, modelden elde edilen omurların açısal hareket sonuçları ile deneysel ölçümlerle elde edilen sonuçların karşılaştırılması ile yapılmıştır. Deneysel çalışma sonucunda gönüllülerin 0.5 ms-2 RMS genliği için ortalama 5.21±0.47 Hz, 1 ms-2 RMS genliği için 4.96±0.45 Hz ve 2 ms-2 RMS genliği için 4.65±0.38 Hz rezonans frekansına sahip olduğu bulunmuştur. Bu frekanslarda gönüllülerin özellikle lumbar multifidus kasında EMG kas aktivasyonu istatistiksel olarak anlamlı artış göstermiştir. Deneysel veriye dayalı çoklu-cisim modelinde yer alan kasların kuvvetleri statik optimizasyon yöntemi ile hesaplanmıştır. Optimizasyon sonuçlarına göre, 2 ms-2 RMS genliğinde ve 4 Hz frekansındaki titreşimin özellikle lumbar multifidus kasında aktivasyon artışına neden olduğu belirlenmiştir. 2 ms-2 RMS genliğinde ve 4 Hz frekansı için hesaplanan kas kuvvetleri ve ölçülen koltuk tepki kuvvetinin yükleme şartı olarak kullanıldığı sonlu elemanlar analizi sonuçlarına göre, omurlararası disk gerilmesinin L5-Sakrum omurga seviyesinde 4.1 MPa ile deformasyona neden olabilecek şekilde akma gerilmesine eriştiği görülmüştür. Omurlararası disklerde görülen en yüksek genleme ise L5-Sakrum seviyesinde ve %0.24 ile akma genlemesinin altında hesaplanmıştır. Omurlarda görülen en yüksek gerilme L4 omurunda kortikal kemikte 29.2 MPa ve gözenekli kemikte 0.4 MPa değerine ulaşmaktadır ve bu değerler akma gerilmesi altında kalmaktadır. Omurlarda görülen en yüksek genleme değeri ise L1 omurunda kortikal kemikte %0.10 ve gözenekli kemikte %0.29 değerine ulaşmakta ve akma genlemesi altında yer almaktadır. Sonuç olarak tez kapsamında, insan vücudunun 2 ms-2 RMS genliğindeki ve 4 Hz frekansındaki düşey titreşimlere maruz kalmasının, L5-Sakrum omurga seviyesinde deformasyona neden olabileceği saptanmıştır. During cruising in a vehicle, vibrations originating from the roughness of the road and the engine of the vehicle are transmitted to the human body through its each contact region to the vehicle, especially through the passenger seat. The vibration affecting the entire of body mentioned above is called `whole body vibration`. Whole body vibration (WBV) may adversely affects comfort and health of people exposed to vibration. Hence, the effect of vibration on the human body needs to be investigated comprehensively. Once exposed to vibration, spinal muscles are activated to maintain the body stability. Instantaneous muscle activation or high exposure against muscle forces can lead to muscle fatigue and spinal degeneration. WBV can cause a stress increase and pain in the intervertebral discs, especially in the lumbar spine. The effects of vibration on the human body can be investigated by taking into consideration some physical parameters such as resonance frequency, transmissibility and apparent mass which are calculated by using experimental measurements such as acceleration, seat reaction force and muscle activation. Biomechanical evaluation of the degeneration on the human body can be carried out by quantifying the loads occurred in the spine. However, it is technically and ethically challenging to measure in vivo muscle forces or stresses in the spinal structures. Due to the difficulty in degeneration detection of the spinal region, mathematical models are needed to use. In this thesis, we aimed to biomechanically evaluate the effect of vibration on the human spine. To do so, the vibration and electromyography (EMG) data were measured from 12 healthy volunteers. In the experiments, which were performed for generating input data for the established mathematical models and evaluating the output of the analysis, muscle activation was measured from the lumbar multifidus, iliocostalis lumborum, and longissimus thoracic spinal muscles by a surface EMG sensor. The acceleration data from the pelvis, lumbar and thoracic vertabrae and seat surface and the volunteer's seat reaction force were acquired by accelerometers and a force platform, respectively. Measurements were acquired from volunteers during seating in a straight position, who exposed vibrations with 0.5, 1.0 and 2.0 ms-2 root mean square (RMS) amplitudes and frequencies ranged between 0.5 and 20 Hz. EMG RMS, apparent mass and transmissibility values were calculated for each volunteer. The activation and force of spinal muscles during vibration were predicted by using the static optimization approach available in OpenSim. Then the muscle activation, muscle force and joint angular motion as results of the analysis were compared to both experimental EMG and angular motion data and finally, the model was verified. Then, the finite element model of a volunteer's lumbar region and the pelvis-sacrum unit was acquired based on computed tomography data. The calculated muscle forces via the OpenSim model were introduced to the finite element model as loading conditions and the stress-strain distribution in the vertebrae and intervertebral discs were calculated. The finite element model was verified by comparing the angular motions of the vertebrae obtained from the model and the experimental measurement. The experimental study pointed out that the volunteers had average resonance frequencies of 5.21±0.47 Hz, 4.96±0.45 Hz and 4.65±0.38 Hz for RMS amplitudes of 0.5, 1 and 2 ms-2, respectively. EMG activation in the lumbar multifidus muscle indicated a significant statistical increase for the relevant resonance frequencies. We found from the experimental study that the highest risk of spinal degeneration occurred at the vibration level with an amplitude of 2 ms-2 RMS and frequency of 4 Hz. We conducted OpenSim and finite element simulations under this defined vibration characteristics. In the finite element analysis, we observed that the peak stress over L5-Sacrum vertebral level (4.1 MPa) exceeded the yielding stress. The maximum strain on the intervertebral discs at L5-Sacrum intervertebral disc level was %0.24, which is under the yielding strain value. The maximum vertebrae stress was calculated as 29.2 MPa at the cortical bone and as 0.4 MPa at the spongious bone of the L4 vertebrae. All of the stresses on the bone tissue were under the yielding stress limit. The maximum vertebrae strain was calculated as %0.1 MPa at L1 vertebrae cortical bone and as %0.29 at the spongious bone. All of the strain results on the bone were under the yielding stress value. In conclusion, according to the results of intervertebral disc stress, the WBV with an amplitude of 2 ms-2 RMS and frequency of 4 Hz in vertical direction could cause deformation at L5-Sacrum intervertebral disc level.
