INTRODUCTION: Retinopathy of Prematurity (ROP); is a vascular disease caused by abnormal proliferation of retinal vessels in preterm and low birth weight infants.AIM: The aim of this study is to determine the frequency of ROP and treatment-requiring ROP in premature infants followed up in our clinic and to determine the risk factors for the development of ROP. Also we aim to determine the frequency and risk factors of PR that change in our clinic over the years.METHOD: The files of 670 babies born <37 weeks of gestation and followed up in the Neonatal Intensive Care Unit of Cerrahpaşa Medical Faculty between 2006-2011 and 2014-2019 years were evaluated retrospectively for the presence of risk factors and ROP examination results were recorded. SPSS version 22.0 was applied.RESULTS: 33.6% of babies developed ROP at various stages and 7% of them developed disease requiring treatment. The mean gestational age and mean birth weight of infants with ROP were significantly lower than those without ROP. The mean gestational age and mean birth weight were significantly lower in infants who developed ROP requiring treatment. In our study, premature birth, low birth weight, apnea, hypoxia and hypercapnia were found to be independent risk factors for ROP.Patients were divided into two different 5-year periods; In Group B (2014-2019), the frequency of ROP and ROP requiring treatment was significantly higher than in Group A (2006-2011). The mean gestational age in Group B was lower than the mean gestational age in Group A. In Group B, the rates of premature rupture of membranes (PROM), mechanical ventilation, duration of ventilation, duration of oxygen treatment, hypercapnia, sepsis, necrotizing enterocolitis (NEC), intraventricular hemorrhage (IVH), patent ductus arteriosus (PDA), phototherapy, packed RBC transfusion, seizure, hypoglycemia and hypothyroidism were increased compared to Group A.CONCLUSION: ROP is a multifactorial disease, main risk factors for it being premature birth and low birth weight. Identification of risk factors for ROP and discovery of pathophysiological mechanisms causing the disease is of utmost importance in development of ROP screening and follow up guidelines.KEYWORDS: Retinopathy of prematurity, risk factors.

GİRİŞ: Prematüre Retinopatisi (PR); erken doğan ve düşük doğum ağırlıklı bebeklerde görülen, retinal damarlarının anormal proliferasyonuna bağlı oluşan vasküler bir hastalıktır.AMAÇ: Çalışmamızın amacı; kliniğimizde izlenen prematürelerdeki PR ve tedavi gerektiren PR sıklığını saptamak, PR gelişimindeki riski faktörlerini belirlemektir. Yıllar içinde değişen PR sıklığı ve risk faktörlerini belirlemek hedeflenmiştir.YÖNTEM VE GEREÇLER: 2006-2011 ve 2014-2019 tarihleri arasında, Cerrahpaşa Tıp Fakültesi Yenidoğan Yoğun Bakım Ünitesi'nde izlenen, <37 gestasyon haftasında (GH) doğmuş ve PR taraması yapılmış 670 bebeğin dosyaları incelenerek risk faktörleri ve PR muayene sonuçları kaydedildi. SPSS versiyon 22.0 kullanıldı.BULGULAR: Hastaların %33.6'sında PR, %7'sinde tedavi gerektiren hastalık saptandı. PR gelişen bebeklerin, gelişmeyenlere göre ortalama GH ve doğum ağırlığı (DA) belirgin düşüktü. Tedavi gerektiren PR gelişen bebeklerin, tedavi edilmeyenlere göre ortalama GH ve DA anlamlı olarak düşüktü. Erken doğum, düşük doğum ağırlığı, apne, hipoksi ve hiperkapni PR için bağımsız risk faktörleri olarak bulundu. İki farklı 5 yıllık döneme ayrılan hasta gruplarından; Grup B'de (2014-2019) PR ve tedavi gerektiren PR sıklığı, Grup A'dakinden (2006-2011) belirgin yüksekti. Grup B'deki ortalama GH, Grup A'daki ortalama GH'dan düşüktü. Grup B'deki erken membran rüptürü, mekanik ventilasyon, ventilasyon süresi, oksijen tedavi süresi, hiperkapni, sepsis, nekrotizan enterokolit, intraventriküler kanama, patent duktus arteriozus, fototerapi, eritrosit transfüzyonu, konvülziyon, hipoglisemi ve hipotiroidi oranları Grup A'ya göre artmıştı.SONUÇLAR: PR; en önemli risk faktörleri erken doğum ve düşük doğum ağırlığı olan multifaktöriyel bir hastalıktır. PR'nin risk faktörlerinin saptanması ve bunların hastalık gelişimindeki rolünün aydınlatılması PR tarama ve takibi için yol gösterici olacaktır.ANAHTAR SÖZCÜKLER: Prematüre retinopatisi, risk faktörleri.
