Cutaneous leishmaniasis (CL) is a protozoa disease with a zoonotic and anthroponotic character, showing cosmopolitan distribution throughout the world and affecting millions of people. The increase in people travelling to endemic areas and migration for political or economic reasons have increased the frequency of this infection seen in non-endemic areas. As the drugs used in the treatment of CL have toxic effects, especially on the liver and kidneys, some Leishmania species have become drug-resistant and the drugs are expensive, this has directed researchers to investigate more reliable, more effective and cheaper treatment options. The aim of this study was to investigate the effect of macrophage-specific antibody bound graphene oxide (GO) nanoparticles together with photothermal application (PTA) on a CL model created in BALB/c mice. This treatment protocol has not been previously used. BALB/c mice were infected by inoculation of Leishmania major amastigotes to the soles of the feet. At 21 days after the inoculation, the infected animals were separated into 6 study groups (GO+15 secs PTA, GO+30 secs PTA, macrophage specific antibody (MSA) modified GO +15 secs PTA, MSA modified GO+30 secs PTA, 15 secs PTA, 30 secs PTA), and 3 control groups (miltefosine, negative and positive control). GO and MSA were bound together with chemical methods and this binding was confirmed with PT-IR and element analyses. To test the binding of GO+MSA to macrophages, immunohistochemical analysis was applied by administering the synthesized molecule to the infected mice. Nanoparticles were injected subcutaneously to the wound area and the treatment procedure was applied after 24 hours. The lesions were clinically evaluated (erythema and oedema) on the 5th and 10th days of treatment and on the 10th day, in addition to clinical evaluation, the severity of pre-inflammatory cytokines (TNF-α, IL-1, IL-6 and IFN-ɤ) was evaluated histopathologically and immunohistochemically. On the 5th day of treatment, the best result was obtained in the MSA modified GO+15 secs PTA group (50% full recovery). In this group, the full recovery rate increased to 75% on the 10th day. In the histopathological evaluation made on the 10th day, intense myofibroblasts and angiogenesis were observed. This was interpreted as an indicator of the healing process. In the immunohistochemical evaluation, a significant reduction was determined in pre-inflammatory cytokines compared to the other groups. This study is the first study in world literature to show the attachment of MSA bound GO nanoparticles to the macrophage surface and the death of infected macrophages and the amastigotes within them with phototherapy and that a response to treatment can be obtained within a short period, such as 5 days. These findings indicate that this treatment protocol could be a new treatment possibility.

Kutanöz leyişmanyoz (KL), dünyada kozmopolit bir dağılım gösteren ve milyonlarca insanı etkileyen, zoonoz ve antroponoz karakterli bir protozoon hastalığıdır. İnsanların endemik bölgelere seyahatlerinin çoğalması, siyasi ve ekonomik sebeplerle yapılan göçler endemik olmayan bölgelerde de bu enfeksiyonun görülme sıklığını arttırmıştır. KL'un tedavisinde kullanılan ilaçların özellikle karaciğer ve böbrekler üzerine toksik etkilerinin olması, bazı Leismania türlerinin ilaçlara direnç kazanması, pahalı olmaları gibi nedenler araştırıcıları daha güvenilir, daha etkili ve daha ucuz bir tedavi seçeneğini aramaya yöneltmiştir. Bu çalışmadaki amacımız, Balb/c farelerde oluşturulan KL modelinde makrofajlara spesifik antikora bağlı grafen oksit (GO) nanopartikülleri ve fototermal uygulamanın (FTU) birlikte KL üzerne etkisini araştırmaktır. Çalışmada BALB/c ırkı fareler, ayak tabanlarına Leishmania major amastigotları inoküle edilerek enfekte edildi. İnokülasyondan 21 gün sonra enfekte hayvanlar 6 deney grubu (GO+15 sn FTU, GO+30 sn FTU, makrofaj spesifik antikor (MSA) ile modifiye edilen GO +15 sn FTU, MSA ile modifiye edilen GO+30 sn FTU, 15 sn FTU, 30 sn FTU), 3 kontrol grubu (miltefosin, negatif ve pozitif kontrol) olacak şekilde gruplara ayrıldı. GO ve makrofaja spesifik antikor kimyasal yöntemlerle bağlandı ve bu bağlanma FT-IR ve elementer analizlerle doğrulandı. GO+MSA'un makrofajlara bağlandığını test etmek amacıyla sentezlenen molekül enfekte farelere verilerek immünhistokimyasal analiz yapıldı. Nanopartiküller, yara bölgesi yakınına derialtı yolla enjekte edildi ve 24 saat sonra tedavi prosedürü uygulandı. Tedavinin 5. ve 10. gününde sonra lezyonlar klinik (eritem ve ödem) olarak 10. gününde ise hem klinik hem de histopatolojik ve immünhistokimyasal olarak pre-inflamatuar sitokinlerin (TNF-α, IL-1, IL-6 ve IFN-ɤ) şiddeti değerlendirildi.Tedavinin 5. gününde en iyi sonucu makrofaj spesifik antikor ile modifiye edilen GO +15 sn FTU grubunda elde edildi (%50 tam iyileşme). Bu grupta 10. günde tam iyileşme oranı %75'e yükseldi. 10. günde yapılan histopatolojik değerlendirmede yoğun bir myofibroblastlar ve anjiyogenezis görüldü. Bu durum iyileşme sürecinin bir göstergesi olarak yorumlandı. İmmünhistokimyasal değerlendirmede ise pre-inflamatuar sitokinlerde diğer gruplara göre belirgin bir azalma saptandı.Bu çalışma, GO ile makrofaj spesifik antikor bağlanarak nanopartikülün makrofaj yüzeyine tutunmasını ve fototerapi ile enfekte makrofajların ve içlerindeki amastigotların öldürülmesini sağlayan ve 5 gün gibi kısa sürede tedaviye yanıt alınabilen dünya literatüründeki ilk çalışmadır. Bulgularımız, bu tedavi protokolünün yeni bir tedavi adayı olduğunu göstermektedir.
