Non-alcoholic fatty liver disease is a condition, which is characterized by widespread trigliserit vacuolar accumulated in liver cells as a result of steatosis process in patients not using alcohol. The term non-alcoholic fatty liver disease is used to imply two distinct diseases together. One is merely fatty liver without inflammation and fibrosis, and the other is nonalcoholic steatohepatits with necroinflammatory activity as well as steatosis. It has been reported that non-alcoholic fatty liver disease is detected in 10-24 % of the general population in various countries. Non-alcoholic fatty liver disease is a condition related to public health because of its epidemiologic load. The aim of the present study was to investigate the effects of symbiotic (probiotic + prebiotic) treatment on the fat accumulation in the liver and to evaluate the importance of Magnetic Resonance Spectroscopy in the diagnosis and the follow-up of the patients with fatty liver by means of pre/post-treatment Magnetic Resonance Spectroscopy imaging.In the present study, symbiotic treatment was given in the form of daily 1 capsule for a three-month period to the patients with non-alcoholic fatty liver disease in grades I, II, III, which was diagnosed by abdominal ultrasonography and liver biopsy. Magnetic Resonance Spectroscopy was taken before and after the treatment. In these patients, fasting blood glucose, trigliserit, total cholesterol, low-intensity lipoprotein, high intensity lipoprotein, alanine aminotransferase, aspartate aminotransferase, gammaglutamine transferase, alkaline phosphates, lactat dehydrogenase, white globe, hemoglobin and thrombocyte values were evaluated before and after the treatment. Additionally, body mass indexes were also evaluated before and after the treatment. In the present study, when intrahepatic trigliserit values measured by magnetic resonance spectroscopy technique before the treatment were compared with the one obtained by the same technique after the treatment, a statistically meaningful decrease was detected (p<0.05). When the low intensity lipoprotein alanineaminotransferase, aspartateaminotransferase values measured before and after the treatment were compared, a statistically meaningful decrease was detected (p<0.05). When the thrombocytes values measured before and after the treatment were compared, a statistically meaningful increase was detected (p<0.05). In the present study, when fasting blood glucose, trigliserit, total cholesterol, high intensity lipoprotein, gamma glutaminetransferase, alkalinephosphates, lactatedehydrogenase, white globe, hemoglobin values measured before and after the treatment were evaluated, a statistically meaningful difference was not detected (p>0.05). A statistically meaningful difference was not detected when the body mass indexes of the patients before and after the treatment were compared (p>0.05).In conclusion, probiotic treatment may decrease the amount of intrahepatic liver trigliserit, the level of liver transaminase and serum lipid profile in patients with non-alcoholic fatty liver disease. The therapeutic potential of probiotics for NASH should be evaluated on wider populations. Safer and wider human trials should be encouraged for these studies on probiotics, which are generally conducted on animals. When the different doses and treatment periods are taken into consideration, the evaluation of the available data is difficult. Before and after the treatment, liver biopsy should be simultaneously conducted along with MRS, which was the evaluation method of the liver fat accumulation in the present study, by which means the sensitivity, precision and reliability of MRS, which is a non-invasive method, should be explored.

Non-alkolik yağlı karaciğer hastalığı, alkol kullanmayan hastalarda steatoz proçesinin bir sonucu olarak karaciğer hücrelerinde biriken geniş trigliserit vakuolleri ile karakterize bir durumdur. Non-alkolik yağlı karaciğer hastalığı (NAYKH), iki ayrı hastalığı bir arada ifade etmek icin kullanılır. Birincisi inflamasyon ve fibrozisin eşlik etmediği sadece yağlı karaciğer, diğeri ise steatoz ile birlikte, nekroinflamatuar aktivitenin olduğu nonalkolik steatohepatittir (NASH). NAYKH'nın farklı ülkelerde genel popülasyonun %10-24'ünde görüldüğü bildirilmiştir. NAYKH epidemiyolojik yükü nedeniyle halk sağlığı ile ilgili bir konudur. Çalışmamızda non alkolik yağlı karaciğer hastalarında sinbiyotik (probiyotik + prebiyotik) tedavisinin karaciğer yağlanması üzerindeki etkilerini, aynı zamanda da tedavi öncesi ve tedavi sonrası Manyetik Rezonans Spektroskopi çekilerek, yağlı karaciğer hastalarının tanı ve takibinde Manyetik Rezonans Spektroskopinin yerini de araştırmayı amaçladık.Bu çalışmada abdominal ultrasonografide (USG) veya karaciğer biyopsisi ile tespit edilmiş grade I,II,III non-alkolik yağlı karaciğer hastalığı olan hastalara 3 ay boyunca günde 1 kapsül sinbiyotik tedavisi verildi. Tedavi öncesi ve tedavi sonrası Manyetik Rezonans Spektroskopi yapıldı. Bu hastalara tedavi öncesi ve tedavi sonrası açlık kan şekeri, trigliserit, total kolesterol, düşük yoğunluklu lipoprotein, yüksek yoğunluklu lipoprotein, kolestrol, alanin aminotransferaz (ALT), aspartat aminotransferaz (AST), gama glutamil transferaz (GGT), alkalen fosfotaz (ALP), laktat dehidrogenaz (LDH), beyaz küre, hemoglobin ve trombosit değerlerine bakıldı. Ayrıca tedavi öncesi ve tedavi sonrası vücut kitle indeksinede bakıldı.Çalışmamızda Manyetik rezonans spektroskopi yöntemiyle ölçülen tedavi öncesi ve tedavi sonrası intrahepatik trigliserit değerleri karşılaştırıldığında istatistiksel olarak anlamlı derecede azaldı (p<0,05). Tedavi öncesi ve tedavi sonrası ölçülen düşük yoğunluklu lipoprotein alanin aminotransferaz, aspartat aminotransferaz değerleri karşılaştırıldığında istatistiksel olarak anlamlı derecede azaldı (p<0.05). Tedavi öncesi ve tedavi sonrası ölçülen trombosit değerleri karşılaştırıldığında istatistiksel olarak anlamlı derecede artmış bulundu (p<0.05). Çalışmamızda açlık kan şekeri, trigliserit, total kolesterol, yüksek yoğunluklu lipoprotein, gama glutamil transferaz, alkalen fosfotaz, laktat dehidrogenaz, beyaz küre ve hemoglobin değerleri tedavi öncesi ve tedavi sonrası ölçümler karşılaştırıldığında istatistiksel olarak anlamlı fark bulunamadı (p>0.05). Bireylerin tedavi öncesi ve tedavi sonrası VKİ değerleri karşılaştırıldığında istatistiksel olarak anlamlı fark bulunamadı (p>0.05).Sonuç olarak probiyotik tedavisi, NAYKH hastalarında intrahepatik karaciğer trigliserit miktarını ve karaciğer transaminaz düzeyini azaltabilir, serum lipid profilini düşürebilir. Probiyotiklerin NASH'daki terapötik potansiyeli daha geniş gruplarda test edilmelidir. Daha çok hayvan deneyleri üzerinden yapılan bu çalışmaların insanlar üzerinde güvenli ve geniş çaplı yapılması da teşvik edilmelidir. Farklı suşlar, dozlar ve tedavi süreleri göz önüne alındığında mevcut verilerin uzlaştırılması zordur. Çalışmamızda kc yağlanma derecesine baktığımız yöntem olan MRS ile eş zamanlı tedavi öncesi ve tedavi sonrası karaciğer biyopsisi yapılmalı ve non invaziv bir yöntem olan MRS'nin sensitivite, spesifite ve güvenirliliği ortaya konmalıdır.
