ABSTRACTMany treatment methods are used alone or in combination for nonspecific back pain,. The superiority of these treatment methods has not been fully demonstrated. In our study, we investigated the acute effects of dry needle and High Velocity Low Amplitude (HVLA) manipulation after one-time administration in these patients.The volunteers included in the HVLA group were all volunteers with a mean body mass index of 23.20 ± 5.160 years and 19.9242 ± 2.01428. The volunteers included in the dry needling group had an average body mass index of 23.00 ± 4.546 years old and 22.2696 ± 3.69481. 50% of the individuals in both the HVLA and dry needling groups reported that they did not do sports. Individuals in the HVLA group performed 20% 2 days / week, 30% 3 days / week; dry needling group 10% 1 day / week, 30% 3 days / error and 10% 6 days / week reported that the sport. 20 women were randomized into two groups. The HVLA maneuver performed during the study was completed in the rotational and postero-anterior direction with bilateral hypothenar regions of the practitioner on the upper thoracic T1-T2, mid-thoracic T5-T6 and lower thoracic T9-T10 transverse processes. Application height was performed on a manipulation table which was the same in each patient.No sham application was planned. In the dry needle group, 0.25-25 mm standard acupuncture needles were used. These needles were applied vertically to the paravertebral muscles in the T1-T2, T5-T6 and T9-T10 segments by 1.5 cm perpendicular to the Gunn method. A total of 12 needle injections were performed in each segment. After waiting 15 minutes, the needles were rotated counterclockwise. No side effects were observed after either treatment.In the statistical analysis of the data collected before and after the study, there were no significant differences (P> 0.05) between the VAS scores for the rest, activity and night pain of the groups and the scores of Quebec and SF 36 before and after the study. In the group changes in the HVLA group; Resting and night pain and Quebec scores decreased significantly (P <0.05) while SF36 scores increased significantly (P <0.05). In the dry needle group, activity pain and SF36 score decreased significantly (P <0.05).As a result, dry needle and HVLA manipulation methods in nonspecific back pain are effective methods in acute period. HVLA has a more meaningful effect on quality of life. This may be that manual therapies increase the parasympathetic response on patients and a corresponding placebo effect. Nonspesifik sırt ağrılarında bir çok tedavi yöntemi tek başına veya kombine olarak kullanılmaktadır. Bu tedavi yöntemlerinin birbirlerine üstünlüğü tam olarak gösterilememiştir. Bizim çalışmamızda da bu hastalarda uygulanan kuru iğne ve High Velocity Low Amplitude (HVLA) manipülasyonun tek seferlik uygulama sonrası akut etkileri araştırılmıştır. Çalışmaya katılan tümü kadın olan gönüllülerden HVLA grubuna dahil olan gönüllüler ortalama 23.20±5.160 yaşında ve 19.9242±2.01428 beden kitle endeksine sahipti. Kuru İğneleme grubuna dahil olan gönüllüler ise ortalama 23.00±4.546 yaşında ve 22.2696±3,69481 beden kitle endeksine sahipti. Hem HVLA hem de kuru iğneleme grubundaki bireylerin %50'si spor yapmadığını bildirmiştir. HVLA grubundaki bireyler %20'si 2 gün/hafta, %30'u 3 gün/hafta spor yaptığını; kuru iğneleme grubundaki bireyler %10'u 1 gün/hafta, %30 u 3 gün/hata ve %10'u 6 gün/hafta spor yaptığını bildirmiş.Çalışmaya katılan 20 kadın hasta kura usulüyle iki gruba randomize edildi. Çalışma sırasında uygulanan HVLA manevrası üst torakal T1-T2, orta torakal T5-T6 ve alt torakal T9-T10 transvers prosesler üzerinde uygulayıcının bilateral hipotenar bölgeleriyle rotasyonel ve postero-anterior yönde tamamlandı. Uygulama yüksekliği her hastada aynı olan bir manipülasyon sedyesinde yapıldı. Her hangi bir sham uygulama planlanmadı. Kuru iğne grubunda ise 0.25-25 mm'lik standart akupunktur iğneleri kullanıldı. Bu iğneler Gunn metodu ile dik bir şekilde T1-T2, T5-T6 ve T9-T10 segmentlerindeki paravertebral kaslara 1,5 cm kadar ilerletilerek uygulandı. Her segmentte bilateral 4 iğne toplam 12 iğne uygulama yapılıktan sonra saat yönünde çevrildi. 15 dakika beklenildikten sonra iğneler saat yönünün tersine çevrilerek çıkartıldı. Her iki tedavi sonrasında da herhangi bir yan etki izlenmedi. Çalışma öncesi ve çalışma sonrası toplanan verilerin istatistik analizinde grupların çalışma öncesine ve sonrasına ait istirahat, aktivite, gece ağrılarını sorgulayan VAS skorları ile Quebec ve SF 36 skorları arasında anlamlı farklılıklar (P>0.05) görülmemiştir. Grup içi değişimlere bakıldığında ise HVLA grubunda; istirahat ve gece ağrısı ile Quebec skorları anlamlı olarak(P<0.05) azalırken SF36 skorları anlamlı olarak(P<0.05) artmıştır. Kuru iğne grubunda ise aktivite ağrısı ve SF36 skoru anlamlı olarak(P<0.05) azalmıştır.Sonuç olarak nonspesifik sırt ağrılarında kuru iğne ve HVLA manipülasyon metodları akut dönemde etkinliği olan metodlardır. HVLA yaşam kalitesine daha anlamlı etki göstermektedir. Bunun sebebi manuel uygulamaların hastalar üzerindeki parasempatik cevabı artırma ve buna bağlı plasebo etkisi olabilir. Bu konuda daha çok hasta sayılı çalışmalara ihtiyaç bulunmaktadır.Anahtar Kelimeler: Sırt ağrısı, manipülasyon, kuru iğne.
