Phototherapy is the cheapest, common and noninvasive method known in the treatment of indirect hyperbilirubinemia. The photodynamic effect of phototherapy is considered to cause diseases, such as pulmonary oxygen damage, intraventricular hemorrhage, retinopathy of the premature, and necrotizing enterocolitis. The study aimed to investigate the cytotoxic effect by assessing apoptosis in liver and kidney in rats which underwent phototherapy. Fifteen Wistar albino rats weighing 7±2 grams of sexes were included in the study. All rats were in the first 7 days of life and were accommodated under standardized conditions of light and temperature. The rats were randomized into three groups (n=5) as follows: group 1, exposure to conventional phototherapy device; group 2, exposure to LED device; and group 3, control (without phototherapy). After exposure to light, the liver and kidney tissues were examined for apoptosis. Apoptotic cells were detected by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) assay, by immunohistochemistry for caspase-3, and by light microscopy. Tissue sections detected for TUNEL and caspase activity were analyzed with BAB Bs200Pro image analysis system. We detected in our study that phototherapy significantly increased apoptosis in the liver and kidney tissues of newborn rats. Therefore, it should be taken into consideration that there may be side effects while administering phototherapy.Key words : Phototherapy, photooxidation, apoptosis.

Fototerapi, indirekt hiperbiluribinemi tedavisinde bilinen en ucuz; yaygın ve noninvaziv yöntemdir. Fototerapi sırasında oluşan fotodinamik etkinin pulmoner oksijen hasarı, intraventriküler kanama, prematüre retinopatisi, iskemi/reperfüzyon hasarı ve nektrotizan enterokolit gibi birçok hastalığa neden olabileceği düşünülmektedir. Bu çalışmada fototerapi uygulanan ratlarda karaciğer ve böbrekte apopitozu değerlendirerek sitotoksik etkinin araştırılması amaçlanmıştır. Çalışmaya ağırlıkları 7 ± 2 gram, 15 rat alındı. Rastgele 3 gruba ayrılan ratlardan birinci grup kontrol grubuydu. İkinci gruba LED, üçüncü gruba konvansiyonel fototerapi uygulandıktan sonra dokular TUNEL ve kaspaz aktivitesi BAB Bs200Pro image analysis system ile değerlendirildi. İstatiksel değerlendirmede SPSS 16 Paket program kullanıldı; apopitoz skorları yüzde olarak değerlendirildi, sonuçlar (median±SD) olarak belirtildi. Kaspaz boyama metoduyla karaciğer ve böbrek için en yüksek apopitoz skorları konvansiyonel fototerapi alan grupta bulundu. Bu grupta karaciğer ve böbrekte apopitoz skorları sırasıyla % 15,6±3,72, ve % 10,2±1,51 LED fototerapi alan grupta % 3,63±1,84, ve % 3.7±2,39 ve kontrol grubunda % 1,27±0,52, ve % 1,35±0,29 idi. Gruplar arasındaki fark anlamlıydı. Tunel boyamada en yüksek apopitoz skoru konvansiyonel fototerapi alan grupta bulundu (sırasıyla % 1,73±0,82 ve % 1,2 ±0,16). Ancak gruplar arasındaki fark anlamlı değildi. Fototerapinin doku üzerine olan sitotoksik etkilerini araştıran az sayıda çalışma bulunmaktadır. Çalışmamızda fototerapinin yenidoğan rat karaciğer ve böbrek dokusunda apopitozu anlamlı oranda artırdığını saptadık. Bu etki göz önünde bulundurularak indirekt hiperbilirubinemi tedavisinde kullanılan fototerapi uygulamasının hassasiyetle uygulanması gereği açıktır.Anahtar kelimeler: Fototerapi, fotooksidasyon, apopitozis.
