Objective: Being exposed to cigarette smoke is important both for its extensity and for its being preventable in terms of public health. It is needed to have biochemical data of passive smoking to define the risks and to count the benefits of anti-smoking responses. The cotinine is the most important and reliable biomarker of nicotine in the passive smokers and active smokers. During the smoking, a lot of free radical and reactive oxygen products come out. While there happens an increase in the serum level of the lung pathology inflammation of the surfactant protein A (SP-A) and surfactant protein D (SP-D) from the collectines which have an important role in lungs' natural immune system, their levels in the lungs decrease. It is regarded that SP-A and SP-D are specific indicators of the lungs for the arrangement of the therapy schema and the follow-up of the disease. It is proclaimed that smoking cigarette or exposure to cigarette smoke have negative effects on the levels of SP-A and SP-D in the lungs.In this search, the aim is to evaluate the effect of smoke exposure on the surfactant protein A and D, which are effective on lung natural immune system, by measuring the cotinine level in the lungs of the children who are exposed to cigarette smoke.Method: 79 children in total were used for this search. A survey was applied to evaluate the smoke exposure and urinary cotinine levels were measured. Cotinine level was measured by chemiluminescence method (children's urines are used) and serum SP-A and SP-D levels were measured by ELISA method (peripherial venous blood is used.). Statistical data was analysed by using SPSS 18 for Windows. P value being <0, 05, was accepted as significant.Findings: The average urinary cotinine level of the children who were exposed to smoking was 622, 27±600, 66 ng/ml and 4, 25±7, 50 ng/ml of the children who were not exposed to smoking. The serum SP-A level in children exposed to smoking were rated as high statistically (p<0,001) and was measured as 2, 64±0, 78 U/L and that was 2, 2±0, 76 U/L in children who were not exposed to smoking. The level were rated as high statistically (p=0, 03) in children exposed to smoking. The serum SP-D level was high in children who were exposed to smoking but it was not significant statistically. It was verified that there was a correlation between the average urinary cotinine level and serum SP-A level(r=0,257, p=0, 02). It was not verified for SP-D level.Conclusion: In our research, it is pointed out that Serum SP-A level, which has a big role on lungs' natural immune system, is higher in the children who are exposed to smoking when compared to the children who are not exposed to. This data shows us that cigarette's inflammatory effect increases as a response to its anti-inflammatory effect in the serum level.Key words: Exposure to smoke, child, cotinine, SP-A, SP-D.

Amaç: Sigara dumanına maruz kalma; hem yaygınlığı hem de önlenebilir olması halk sağlığı bakımından oldukça önemlidir. Risklerini tanımlamak ve sigara karşıt müdahalelerin yararlarını saymak için sigara dumanına maruziyetin kesin biyokimyasal ölçümlerine gerek vardır. Sigara dumanına maruz kalmış sigara içmeyen kişilerde ve aktif sigara içicilerinde maruziyeti göstermede kotinin nikotinin en önemli ve en güvenilen biyomarkeridir. Sigara içimi sırasında çok sayıda serbest radikal ve reaktif oksijen ürünleri açığa çıkmaktadır. Akciğerin doğal immün sisteminde önemli rolü olan kollektinlerden sürfaktan protein A (SP-A) ve sürfaktan protein D'nin (SP-D) akciğer patolojilerinde enflamasyona sekonder olarak serum düzeylerinde artış olurken, akciğerdeki düzeylerinde azalma olmaktadır. SP-A ve SP-D nin hastalığın takibinde, tedavi şemasının düzenlenmesinde akciğere spesifik bir belirteç olduğu düşünülmektedir. Sigara içimi veya maruziyetinin akciğerdeki surfaktan protein A ve D düzeyini olumsuz yünde etkilediği bildirilmiştir.Bu çalışmada amacımız kotinin düzeyi ölçülerek sigara dumanına maruziyeti saptanmış çocuklarda, bu maruziyetin akciğerin doğal savunma sisteminde etkili surfaktan protein A ve D düzeyi üzerindeki etkisini değerlendirmektir.Yöntem: Çalışmaya toplam 79 çocuk alındı. Sigara dumanına maruziyetini değerlendirmek için anket uygulandı ve idrar kotinin seviyeleri ölçüldü. Çocuklardan alınan idrardan kemilüminesan yöntemiyle kotinin, periferik venöz kandan ise ELISA yöntemi ile serum SP-A ve SP-D düzeyleri ölçüldü. SPSS 18 kullanılarak istatistiksel analizler yapıldı. P değerinin <0,05 olması anlamlı olarak kabul edildi. Bulgular: Ortalama idrar kotinin düzeyi sigaraya maruz kalan grupta 622,27±600,66 ng/ml olarak saptanırken, kalmayan grupta 4,25±7,50 ng/ml olarak saptandı. Sigara dumanına maruz kalanlarda idrar kotinin düzeyi istatistiksel olarak anlamlı derecede yüksek saptandı (p<0,001). Serum SP-A düzeyleri sigara dumanına maruz kalan grupta 2,64±0,78 U/L olarak saptanırken kalmayan grupta 2,2±0,76 U/L olarak ölçüldü. Sigara dumanına maruz kalanlarda istatistiksel olarak anlamlı derecede yüksek bulundu (p=0,03). Serum SP-D düzeyi sigaraya maruz kalan grupta yüksek saptanmakla birlikte istatistiksel olarak anlamlı değildi. Sigara dumanına maruz kalan çocuklarda idrar kotinin düzeyi ile serum SP-A düzeyi arasında pozitif korelasyon saptandı (r=0,257, p=0,02). SP-D düzeyi ile kotinin arasında korelasyon saptanmadı. Sonuç: Çalışmamızda sigara dumanına maruz kalan pasif içici durumundaki çocuklarda akciğer doğal savunma sisteminde etkili serum SP-A düzeyinin sigara dumanına maruz kalmayan çocuklara göre yüksek olması sigara dumanının inflamatuar etkisine antiiflamatuar yanıt olarak serumda yükseldiğine işaret etmektedir.
