ABSTRACTThe incidence of invasive aspergillosis (IA) which is associated with extremely high mortality rates, has increased over the last years especially in immuncompromised patients. Neutropenia is an important risc factor for IA. Because of difficulties about diagnosing the infection early in the course of the disease and rapidly progresive clinical outcome; serological( Galactomannan antigen test, GM) and molecular diagnostic assays (Polimerase chain reaction) developed in recent years. This study was conducted with 358 sera samples from 99 patients febril neutropenic episodes who were followed in haematology and bone marrow tranplantation units of ESOGU faculty of medicine hospital with suspicious IA and 29 samples from 29 control patients between January 2011-January 2012. Patients clinical and laboratory findings prospectively observed. They were classified by European Organisation for the Research and Treatment of Cancer/Mycoses Study Group criteria. GM antigen test and two different commercial real-time PCR test; one of them is standardised and the other one is in house real time PCR and conventional PCR performed. All these assays showed different sensitivity rates but highest rate of sensitivity obtained from standardised real-time PCR test. Also sensitivity is increased by using combination of diagnostic methods. We evaluated impact of chemoterapy regimens, antifungal and antibacterial treatment, underlying disease, degree of neutropenia on diagnostic tests performance. We detected that antifungal treatment and the degree of neutropenia may influence of diagnostic assays performances. In conclusion; we thought that together with assessment of clinical signs and symptoms, GM antigen test and standardised real-time PCR tests could be beneficial for early diagnosis and treatment of ?A. ÖZET?nvaziv aspergilloz (?A) özellikle bağı?ıklığı baskılanmı? hasta grubunda son yıllarda insidansında artı? görülen ve yüksek mortalite nedeni olan bir mantar enfeksiyonudur. Bu hasta grubunda nötropeni ?A geli?imi için önemli bir risk faktörüdür. Mevcut yöntemlerin erken tanıda yetersiz kalması ve hastalığın hızla ilerlemesi nedeniyle serolojik yöntemler (Galaktomannan antijen testi) ve moleküler yöntemler (polimeraz zincir reaksiyonu) gündeme gelmi?tir. Bu çalı?mada, Ocak 2011-Ocak 2012 tarihleri arasında ESOGÜ Tıp Fakültesi Hematoloji Servisi ve Kemik ?liği Transplantasyon Ünitesinde takip edilen hematolojik maligniteli 99 hastadan febril nötropenik oldukları dönemde toplam 358 ve kontrol grubuna ait 29 serum örneği elde edilmi?tir. Prospektif olarak hastaların klinik bulguları ve laboratuvar verileri izlenmi?tir. Sınıflandırma Avrupa Kanser ve Mikoz Ara?tırma ve Tedavi Çalı?ma Grubu (EORTC-MSG) kriterlerine göre yapılmı?tır. Galaktomannan antijen testi ve biri standardize diğeri in house olmak üzere iki farklı real time PCR testi ve geleneksel PCR i?lemleri uygulanmı?tır. Bu yöntemlerin herbiri farklı duyarlılık oranları sergilemi? olmakla birlikte, en iyi performans standardize real time PCR testi ile elde edilmi?tir. Ayrıca yöntemlerin kombine kullanımı ile duyarlılık oranlarında anlamlı yükselmeler gözlenmi?tir. Hastalara uygulanan kemoterapi rejimleri, kullandıkları antifungal ve antibakteriyel tedavi, nötropeni düzeyi, altta yatan hastalığı gibi faktörlerin kullanılan yöntemler üzerine etkisi değerlendirilmi? olup nötropeni düzeyi ve antifungal kullanımının test performansını etkileyebileceği saptanmı?tır. Sonuç olarak; ?A?un erken tanı ve tedavisinde, klinik bulgularla birlikte GM ve standardize real time PCR testlerinin birlikte kullanımının yararlı olacağını, PCR?ın rutin kullanıma girmesi için daha geni? çaplı çalı?malara ihtiyaç olduğunu dü?ünmekteyiz.Anahtar Kelimeler: PCR, invaziv aspergilloz, febril nötropeni, galaktomannan.
