There have been some studies in the literature regarding the incidence and relation of mixed linage leukemia (ML) and acute leukemias with the aberrant antigens expression with prognosis. However since they concluded contraversial results, no concensus has been provided yet on this subject.The aim of the this study was to evaluate the incidence of precursor B ALL, T ALL, AML, ML and acute leukemias with the aberrant antigens expression and clinical importance of aberrant antigen expression in acute leukemias and prognosis of ML and acute leukemia with aberrant antigen coexpression in our patients with acute leukemia.A retrospective study was conducted of 124 children with acute leukemia who had been diagnosed and treated between Jaunuary 2000-June 2007. Patients who were classified as mature B ALL were not included in the study.Eightytwo of total of 124 patients were found to be ALL (66.13 %), 26 (20.97 %) were AML and 16 (12.90 %) were ML.No statisticaly significant difference was found in comparison of prognostic factors, outcome (rate of relapse or death) and survival analise between the patients with My (-) and My (+) ALL as well as My (-) and My (+) precursor B ALL. Whereas frequency of CD15 positivity was found to be higher in patients younger than 12 months. Survival analises were significantly lower in CD15 (+) ALL. After exculusion of infants, OAS was still significantly lower in CD15 (+) children.Although lymphoid antigen coexpression was found to be frequently in chilhood AML they had no prognostic valueThe survival analises were lower than ALL and higher than AML. However the differences were found to be statisticaly significant only in the comparison of ML and ALL groupsIn conclusion this study showed that coexpressin of CD15 in ALL was associated with poor prognosis and it was frequently found in infantile ages and ML patients showed worse prognosis than ALL. We have concluded that these patients are included in high risk leukemia who need more intensive therapy even they should be considered as candidates for BMT.KEY WORDS: Acute leukemia, aberrant antigen, prognostic factors, prognos.

Literatürde olağandışı belirteç taşıyan lösemiler ve miks lösemilerin (ML) sıklığı ve, olağandışı belirteç taşıyan lösemilerin bilinen prognostik faktörlerle ilişkisi ve prognozu hakkında bir görüş birliği sağlanamamıştır.Biz de ALL, AML, ML ve olağandışı belirteç taşıyan lösemilerin sıklığını belirlemek, olağandışı belirteç bulundurmanın prognostik faktörler ve prognozla ilişkisini görmek amacı ile bu çalışmayı yaptık.Çalışma, Ocak 2000-Haziran 2007 tarihleri arasında 124 hastada retrospektif olarak yürütülmüştür. Olgun B hücreli ALL tanısı alan hastalar çalışmaya alınmamıştır.Çalışmamızın sonucunda 124 akut lösemi tanılı hastanın 82'si ALL (% 66,13), 26'sı AML (% 20,97) ve 16'sı da ML (%12,90) olarak bulunmuştur.My (-) ile My (+) ALL grupları ve benzer şekilde My (-) ile My (+) öncül B ALL gruplarının, prognostik faktörler, nüks ve eks olma oranı ve sağkalım analiz değerleri karşılaştırıldığında bir fark bulunmadı. Ancak CD15 pozitifliği sıklığı tanı yaşı 12 ay ve altında olan hastalarda fazla bulundu. CD15 (+) ALL grubunun sağkalım analizi değerleri de düşüktü. Tanı yaşı 12 ayın üzerinde olan CD15 (+) ve CD15 (-) ALL'li hastalar karşılaştırıldığında da CD15 (+) olan grubun OAS değeri yine düşük bulundu.Çalışmamızda AML'de olağandışı lenfoid belirteç varlığının oldukça sık bulunmakla birlikte prognostik bir önemi olmadığını gördükML grubunun sağkalım analizi değerleri AML grubuna göre yüksek, ALL grubuna göre düşük bulunmakla birlikte sadece ALL grubu ile yapılan OAS değeri karşılaştırma sonucu istatistiksel olarak anlamlıydıSonuç olarak çalışmamız CD15 taşımanın ALL'de prognozu olumsuz etkilediğini ve infantil yaş grubunda daha sık görüldüğünü, ML tanısı alan hastaların prognozunun kötü olduğunu göstermiştir. ML ve CD15 (+) ALL tanısı alan hastaların yoğun kemoterapi almaları gereken lösemiler olduğunu hatta bu hastaların KİT için aday olduklarını düşünmekteyiz.ANAHTAR KELİMELER: Akut lösemi, olağandışı belirteç, prognostik faktör, prognoz.
