Anesthesia records should be correct and reliable for better perioperative patient care, better outcomes and further epidemiologic studies. Physical status of patient before the procedure, vital signs, medications and interventions during anesthesia are recorded to the anesthesia record form. Anesthesia record form is important not only for assessment of the intraoperative condition but also for medical and legal issues as a document which may be referred for further procedures.While comfort, safety of the patient and suitable surgical conditions are targets of surgical anesthesia, some other important factors for the mother and the baby should also be considered in obstetric anesthesia. These factors make obstetric anesthesia unique.Due to maternal factors like physiologic changes associated with pregnancy and labor, most of the interventions being urgent, high gastric residual volume, psychological status, altered uterine tonus due to CNS depression or sympathetic blockade, aortocaval compression, direct or indirect effects of all medications on fetus, influence of maternal hemodynamic and respiratory alterations on fetus and impossibility of intrauterine resusicitation, obstetric anesthesia is different from other anesthesia types. Sufficient knowledge and skills are expected from the anesthesiologist for safety of both mother and fetus as well as to provide suitable surgical conditions.In our study data were generally obtained from preoperative and intraoperative anesthesia record forms. For access some information, we made search at patient files and computarised data system of our instutition (Nucleus Medical Data System) .We accessed and analysed the data of 14361 patients who underwent surgical procedure in our hospital because of obstetric and gynecologic diseases between 2006 and 2012 in terms of anesthesiologic factors .In our study, the most common surgical procedures were caesarean section (43,3%), total abdominal hysterectomy+bilateral salpyngo oophorectomy (TAH+BSO) (16,1%) and diagnostic laparoscopy (5,3%).58,4% of the patients were ASA I, 37% were ASA II, 4,3% were ASA III and 0,2% were ASA IV physical status.60,3% of the cases were operated under elective conditions and 39,7% were operated under urgent conditions.When we evaluated the anesthetic techniques performed in our clinic, 87,1% of the patients has been performed general anesthesia, 12,6% spinal anesthesia, 0,2% CSE, 0,1% local anesthesia.In terms of the most common systemic disease, 17.7% of patients had cardiovasculary problems. In terms of systemic disease by age, for patients who were older than 46 years old , systemic disease rates than other age groups were found to be significantly higher (p<0.01).For Mallampati Score, at obstetrical and gynecological surgery Mallampaty Score I, at gyneco-oncologic surgery Mallampaty Score II rates was significantly higher than other groups (p<0,01).0,9% of the patients had difficult endotracheal intubation. The most common cause of difficult intubation was anterior larynx (96,0%). While 81,1% of these patients were intubated by inserting a tube stylet, 9,4% were intubated with McCoy laryngoscopy and 6,3% with fiberoptic bronchoscopy.When requirement rate for invasive monitoring was analysed (arterial and central venous pressure), we saw that requirement for invasive monitoring rates were significiantly higher for patients older than 61 years old and undergoing gyneco-oncologic surgery (p<0.05).Transfusion rates were significantly higher at gyneco-oncologic surgery .(p<0.05).When anesthesia procedure according to operation type was analysed, it has seen that general anesthesia was applied in the ratio of 76,7%, spinal anesthesia 23,1% for obstetric surgery and general anesthesia was seen to be the most commonly used anesthesia type (96,8%) for gynecologic surgery and in also gyneco-oncologic surgery (99,4%).General anesthesia was applied in the ratio of 69,4% in elective patients, the ratio of spinal anesthesia was 30,4%. In urgent caesarean section operations, ratio of general anesthesia was 75,1% and ratio of spinal anesthesia was 24,9%.54,6% of the patients were >37 weeks gestational age .In addition, mean obstetric age was found 35,37 ± 4,36 weeks for the patients who underwent caesarean section. When we analysed pregnants in terms of caesarean section indications, we determined common indications as preeclampsia (30,1%), cephalopelvic disproportion (10,3%) and repeated caesarean section (9,1%).For C/S group, 1st min mean Apgar scores were 6,83 ± 1.79, 5th min mean Apgar scores were 7,89 ± 1,71. Mean weight of C/S babies was 2571,44 ± 1006,76 gram. 53% of the babies had neonatal ICU care requirement. Mean duration of ICU stay was 14,7 ± 17,30 days. Infants' who received ICU mean duration of mechanical ventilation was 14,58 ± 15,15 days. While 84,5 % of the C/S babies discharged from hospital with cure, 13,4% has died.Intrauterine exitus ratio was 2,1%. Mean duration of discharge from hospital was 4,2 ± 2,55 days. Mean exitus time was 13,5 ± 13,56 days.The most common types of intraoperative complications were cardiac complications (16,8%) followed by allergic complications (1,1%). We found the ratio of intraoperative cardiopulmonary arrest was 0,06%.1,9% of the pregnants had postoperative ICU requirement. When ICU requirement according to surgery condition was analysed, 1,8% of the elective patients and 2,1% of the urgent patients were found to require ICU care and there was no significiant difference between groups(p>0,05). Mortality rate was found as 0,2% among the total operated patients.When we evaluated rates of hypotension in patients undergoing cesarean section, we saw that hypotension during spinal anesthesia(32,5%) was significantly higher than general anesthesia (p<0.01).At cesarean sectio procedures,vasopressor agent consumption rates for the treatment of low arterial blood pressure at patients undergoing cesarean sectio was statistically significantly greater in spinal anesthesia than general anesthesia. (32.5%) (p<0.01).1st and 5th minutes Apgar scores were similar between general anestesia population and spinal anestesia population (p>0.05). There was no statistically significant difference between general anesthesia and spinal anesthesia populations for mortality rates (p>0.05). We conclude that regular and systematic anesthesia records could help for new studies including large patients series.Nucleus Medical Data System used in OMU and anesthesia record forms of our clinic included most of these data. We suggest performing this type of retrospective studies with regular intervals enables to evaluate the clinical practices regularly for the detection of record errors and to compare the clinics, thereby improve the development of anesthesiology.

Peroperatif daha iyi hasta bakımı sağlanması, epidemiyolojik araştırmalar yapılabilmesi ve sonuçların sağlıklı olabilmesi için anestezi kayıtlarının düzgün ve güvenilir olması gerekmektedir. Anestezi kayıt formuna hastanın anestezi öncesi durumu, anestezi sırasında yapılan işlemler, uygulanan ilaçlar ve vital bulgular kaydedilir. Anestezi kayıt formu hastanın operasyondaki durumunun değerlendirilmesi yanında, daha sonraki uygulamalarda da gerektiğinde danışılabilecek bir belge olduğundan tıbbi ve adli açıdan önemlidir.Cerrahi anestezide düşünülmesi gerekenler hastanın rahatı, güvenliği ve elverişli ameliyat koşulları sağlamak iken, obstetrik anestezide bunlara ek olarak anne ile fetus/yenidoğan açısından önemli diğer etkenler de söz konusudur. Bunlar obstetrik anesteziye kendine has özellikler kazandırır.Gebelik ve eylemin yaptığı fizyolojik değişiklikler, ilaçlara yanıttaki farklılıklar, girişimlerin çoğunun acil nitelikte oluşu, midenin dolu olabilmesi, psikolojik durum, SSS depresyonu veya sempatik blokaj etkisi ile uteroplasental kan akımının değişmesi, aortokaval bası gibi anne ilgili nedenler ve bütün ilaçların fetusta direkt veya dolaylı etkisinin söz konusu olabilmesi, annenin kardiorespiratuar ve metabolik durumundaki değişikliklerin fetusa yansıması, intrauterin resüsitasyonun olanaksızlığı gibi fetusla ilgili etkenler nedeniyle obstetrik anestezi diğer anestezilerden farklıdır. Obstetrik anestezi sırasında anestezistten hem annenin ve fetusun güvenliğini hem de yeterli cerrahi koşulları sağlayabilmesi için yeterli bilgi ve beceri beklenir.Araştırmamızda elde edilen verilere ağırlıklı olarak preoperatif ve intraoperatif anestezi kayıt formlarında bulunan bilgilerden ulaşıldı. Bazı verilere ulaşmak için ise hasta dosyalarındaki kayıtlar ve `Nukleus Medikal Bilgi Sistemi` olarak adlandırılan hasta kayıtlarının ve bilgilerinin tutulduğu hastane bilgisayar sistemi kullanıldı.2006-2012 yılları arasından merkezimizde obstetrik ve jinekolojik hastalıklar nedeniyle opere edilen 14361 hastanın verilerine sağlıklı bir şekilde ulaşıldı ve bu verilerin anesteziyolojik açıdan retrospektif analizi yapıldı.Çalışmamızda kliniğimizde en sık gerçekleştirilen üç operasyonun; %43,3 oranında sezaryen, %16,1 oranında TAH+BSO, %5,3 oranında da tanısal laparoskopi olduğu bulundu.Çalışmamızda hastaların %58,4'ünün ASA I, %37'sinin ASA II, %4,3'ünün ASA III, %0,2'sinin ASA IV olduğu belirlendi. Çalışmamızda hastaların %60,3'ünün elektif, %39,7'sinin acil olarak cerrahiye alındığı bulundu. Kliniğimizde uygulanan anestezi yöntemleri incelendiğinde hastaların %87,1'ine genel anestezi, %12,6'sına spinal anestezi, %0,2'sine KSE, %0,1'ine lokal anestezi uygulandığı belirlendi.Opere edilen hastalar ek sistemik hastalık açısından değerlendirildiğinde en sık görülen ek sistemik hastalığın %17,7 oranında kardiovasküler sistem hastalıkları olduğu bulundu. Yaşa göre ek sistemik hastalık açısından değerlendirildiğinde 46 yaş üstü hastalarda ek sistemik hastalık oranlarının diğer yaş gruplarından anlamlı oranda yüksek olduğu bulundu (p<0,01).Hastalar Mallampati Skoru açısından değerlendirildiğinde obstetrik ve jinekolojik cerrahide Mallampati Skoru I, jinekoonkolojik cerrahide ise Mallampati Skoru II olan hasta sayısının diğer gruplardan anlamlı oranda yüksek olduğu bulundu (p<0,01). Çalışmamızda genel anestezi alanlarda entübasyon güçlüğü oranları incelendiğinde; %0,9 hasta da entübasyon güçlüğü geliştiği bulundu. Entübasyon güçlüğünün en sık nedeni ise %96,0 oranla yüksek larinks olarak bulundu. Entübasyon güçlüğü gelişen hastaların %81,1 tüp içine stile yerleştirilerek entübe edilirken, %9,4'ü McCoy laringoskop, %6,3'ü de fiberoptik kullanılarak entübe edildi.Hastalara yapılan invazif monitörizasyon (arteriyel ve santral venöz basınç) oranları incelendiğinde 61 yaş üstünde ve jinekoonkolojik cerrahi geçiren hastalarda diğer gruplara göre anlamlı oranda yüksekti (p<0,05).Kan ve kan ürünü transfüzyonu açısından değerlendirildiğinde jinekoonkolojik cerrahide transfüzyon oranları diğer gruplardan anlamlı oranda yüksek bulundu (p<0.05). Yapılacak operasyon türüne göre uygulanan anestezi yöntemi incelendiğinde; obstetrik cerrahide %76,7 genel anestezi, %23,1 spinal anestezi uygulandığı, jinekolojik cerrahide en sık kullanılan anestezi şeklinin %96,8 ile genel anestezi olduğu, jinekoonkolojik cerrahide de yine %99,4 oranla en fazla genel anestezinin tercih edildiği belirlendi.Çalışmamızda sezaryen yapılan hastalarda cerrahiye kabul şekline göre uygulanan anestezi yöntemi incelendiğinde elektif hastalarda %69,4 oranında genel anestezi, % 30,4 oranında da spinal anestezi uygulandığı belirlendi. Acil sezaryen operasyonlarında uygulanan anestezi oranları ise %75,1 ile genel anestezi ve %24,9 ile spinal anestezi idi.Çalışmamızda gebe hastalar obstetrik haftaya göre sınıflandırıldığında %54,6'sının 37 hafta ve üstünde olduğu bulundu. Ayrıca sezaryen operasyonu geçiren hastalarda ortalama obstetrik hafta 35,37 ± 4,36 olarak hesaplandı. Gebe hastaları sezaryen endikasyonu açısından incelendiğimizde %30,1'inin preeklampsi, %10,3'ünün sefalopelvik uyumsuzluk, %9,1'inin ise mükerrer sezaryen endikasyonuyla sezaryene alındığı belirlendi.Çalışmamızda sezaryenle doğan bebeklerin 1.dk ortalama Apgar skoru 6,83 ± 1,79, 5.dk ortalama apgar skoru 7,89 ± 1,71 olarak bulundu. Bebeklerin ortalama ağırlığı 2571,44 ± 1006,76 olarak hesaplandı. Bebeklerin %53'ünün neonatal YBÜ gereksinimi olduğu bulundu. Bebeklerin YBÜ'sinde ortalama kalış süreleri 14,7 ± 17,30 olarak hesaplandı. YBÜ'sine alınan bebeklerin ortalama ventilatöre bağlı kalış süreleri 14,58 ± 15,15 gündü. Sezaryenle doğan bebeklerin %84,5 şifayla taburcu edilirken %13,4'ü eksitus oldu. İntrauterin eksitus oranları %2,1 olarak bulundu. Sezaryenle doğan bebeklerin ortalama taburcu zamanı 4,2 ± 2,55 gün olarak hesaplandı. Eks olan bebeklerin ortalama eksitus zamanları ise 13,5 ± 13,56 gün olarak bulundu. Çalışmamızda en sık karşılan intraoperatif komplikasyon grubunun %16,8 ile kardiak komplikasyonlar olduğu, bunları %1.1 ile alerjik komplikasyonların izlediği bulundu. İntraoperatif arrest oranlarını incelendiğimizde hastaların % 0,06'inde intraoperatif arrest geliştiği bulundu. Çalışmamızda hastaların %1,9'unda postoperatif yoğun bakım ünitesi gereksinimi olduğu belirlendi. Cerrahiye kabul şekline göre YBÜ gereksinimi incelendiğinde; elektif hastaların %1,8'inin, acil hastaların %2,1'inin YBÜ gereksinimi olduğu bulundu ve iki grup arasında istatistiksel olarak fark olmadığı belirlendi (p>0,05). Opere edilen toplam hastaların mortalite oranlarının ise %0,2 olduğu bulundu. Çalışmamızda sezaryen yapılan hastalarda hipotansiyon oranları değerlendirildiğinde, spinal anestezide hipotansiyon oranı (%32,5) genel anesteziden istatistiksel açıdan anlamlı olarak yüksek bulundu (p<0,01). Ayrıca sezaryen yapılan hastalarda hipotansiyon tedavisi için vazopressör ajan kullanım oranları karşılaştırıldığında, spinal anestezide vazopressör ajan kullanım oranı (%32,5) istatistiksel açıdan anlamlı olarak yüksek bulundu (p<0,01). Çalışmamızda spinal anestezi ve genel anestezi uygulanan sezaryenlerin bebeklerinin 1.dk ve 5.dk Apgar skorları açısından karşılaştırıldığında iki grup arasında istatistiksel fark olmadığı belirlendi (p>0,05). Sezaryen olan hastalara uygulanan anestezi yöntemine göre bebeklerin mortalite oranları incelendiğinde, genel anestezi ve spinal anestezi arasında bebek mortalite oranları açısından istatistiksel olarak anlamlı fark bulunmadı (p>0,05).Sonuç olarak; düzenli ve sistematik bir şekilde tutulan anestezi kayıtlarından geniş hasta serilerini kapsayan bir çok çalışmanın sağlıklı bir biçimde yapılabileceği, OMÜ'nde kullanılan Nükleus Medikal Bilgi Sistemi ve kliniğimiz anestezi kayıt formlarının hastalarla ilgili bilgilerin birçoğunu içerdiği ve bu tarz retrospektif çalışmaların belli aralıklarla tekrarlanmasının hem aynı klinikteki uygulamaların sürekli olarak değerlendirilmesine, hem kayıtlardaki eksikliklerin saptanmasına, hem de klinikler arası karşılaştırma yapılmasına olanak vererek anesteziyolojinin gelişimine katkı sağlayacağı kanısına varıldı.
