The aim of this study is to determine the factors affecting the preferences of specialization in field of medicine. Within the scope of the research, the following were studied: factors that affect preferences of field of specialization by physicains, the reasons for such preferences, significant changes in decision-making in recent years and also important were deductions made in order make suggestions decision-making and development of human resources for health in Turkey.Mix̄ed research methods and exploratory sequential research design were used. The research was designed in three stages: explorative, measurement tool development and application of measurement tool. In the ex̄plorative phase of the study, qualitative data were collected from 14 specialist physicians using semi-structured questionnare through interviews that lasted on avarage for 45 minutes. The findings were analyzed with descriptive and content analyses methods. In the second stage of the study, the measurement tool was developed from the data obtained from the qualitative analyses during the first stage. The developed measurement tool was applied to medical school students and the physicians prepared for the specialization exam. A total number of 502 participants were recruited for the second stage. Qualitative findings from the second stage were structured under 4 themes. These themes are: the factors affecting the specialization into various field of medicine, the reasons for changing preferences of the physicians in recent years, future challenges related to changing preferences and suggestions made to prevent potential problems identified.According to the results of the research, the factors affecting the physicians' preference of specialty in medicine were classified as individual, occupational and about systemic factors. Although there are many factors that affect the preferences of medical field, it is concluded that the personality traits of individuals and mortality rates associated with the branch or field of medicine are the most significan professional factors, while the risks and the exposure to violence and the application of the additional payment based on the performance of candidates are the systemic factors that affect selections and preferences.

Bu araştırmanın amacı tıpta uzmanlık alan tercihlerine etki eden faktörleri tespit etmektir. Araştırma kapsamında uzmanlık alan tercihine etki eden faktörler, bu tercihleri etkileyen nedenler ve son yıllarda hekim tercihlerindeki önemli değişiklikler incelenmiş, ayrıca Türkiye'de insan kaynaklarının geliştirilmesine katkıda bulunmak hedeflenmiştir. Araştırma karma araştırma yöntemi ile yürütülmüş, keşfedici sıralı araştırma deseni kullanılmıştır. Araştırma üç aşama olarak tasarlanmıştır. Bu aşamalar: keşfedici, ölçme aracı geliştirme ve ölçme aracını uygulamadır. Araştırmanın keşfedici aşamasında 14 uzman hekim ile ortalama 43 dakika süren yarı yapılandırılmış görüşme tekniği ile nitel veriler toplanmış, bulgular betimsel ve içerik analizi yöntemi ile analiz edilmiştir. Araştırmanın ikinci aşamasında nitel analiz sonucunda elde edilen verilerden yola çıkılarak ölçme aracı geliştirilmiştir. Geliştirilen ölçme aracı sonuçların bütüncül bir şekilde yansıtılması amacıyla tıp fakültesi öğrencileri ile uzmanlık sınavına hazırlanan toplam 502 kişi üzerinde uygulanmıştır.Araştırmada nitel bulgular 4 tema altında yapılandırılmıştır. Bu temalar; tıpta uzmanlık branşlarına etki eden faktörler, son yıllarda hekimlerin değişen tercihlerine ilişkin nedenler, değişen tercihlerin ileriki yıllarda oluşturabileceği problemlere ilişkin sorunlar ve sorunların önüne geçebilmek için yapılan öneriler olarak gruplandırılmıştır.Araştırma sonucunda katılımcıların görüşlerine göre hekimlerin tıpta uzmanlık alan tercihine etki eden faktörler bireysel, mesleki ve sistemsel faktörler olarak sınıflandırılmıştır. Tıpta uzmanlık alan tercihlerini etkileyen birçok faktör olsa da araştırma sonucunda bireysel faktörlerden kişilik özelliklerinin uzmanlık alan tercihinde önemli olduğu, mesleki faktörlerden branşın mortalite oranı, taşıdığı risk ve şiddete maruz kalma durumunun ve sistemsel faktörlerden performansa dayalı ek ödeme uygulamasının bu seçimi etkilediği ve yön verdiği sonucuna ulaşılmıştır. Ayrıca son yıllarda tercih eğilimlerinin değiştiği, hekimlerin tercihlerinin klinik branşlardan preklinik branşlara doğru kaydığı bu nedenle ileride çok tercih edilen alanlara yığılma olacağı, önlem alınmazsa bu durumun hastalar açısından nitelikli sağlık hizmeti alamama, sağlık sistemi açısından ise hizmette aksamalara neden olacağı sonucuna ulaşılmıştır. Araştırma kapsamında nitel verilere dayalı olarak 42 madde ve 7 boyuttan oluşan `Hekimlerin Branş Tercih Eğilimleri Ölçeği` geliştirilmiş, bu ölçek ile nitel verilerin nicel veriler ile desteklendiği sonucuna ulaşılmıştır.
