AİM: Urolithiasis is a complex disease resulting from an interaction between environmental and genetic factors. Recently, the relationship between vitamin D receptor (VDR) alleles and calcium homeostasis has been investigated. This study was conducted to identify the association of VDR gene polymorphism with urolithiasis. MATERİAL & METHODS: We studied the VDR gene polymorphisms, ApaI, Fok and TaqI, serum electrolytes, PTHe (PTH), and 25-hydroxyvitamin D and urinary metabolic risk factors in 128 children (60 males, 68 girls) with urolithiasis and in 60 healthy children (27 males, 33 girls) without urolithiasis and positive family history. RESULTS: A significant difference in the ApaI genotype was observed between the urolithiasis and the control group ( p=0.01). It is faound that In the urolithiasis group the frequency of AA+ Aa genotype was 5.23 times more when compared with the Aa+aa genotype (95% confidence interval 1.5-18.2). TheFok and TaqI polymorphisms did not show any significant association with urolithiasis. Serum vitamin D3 levels were significantly higher in the group with urolithiasis when compared controls (P<0.001).The results indicate that the ApaI AA+ Aa genotype of the VDR gene is closely associated with urolithiasis. Besides TheFok and TaqI polymorphisms did not show any significant association with urolithiasis. However, f allele and ff+Ff genotype of Fok were seem to be related with an increase of urolithiasis risk. TaqI also presented with increased urolithiasis risk with t allele and tt+Tt and tt genotype. Our study indicated VDR polymorphisms could be potential biomarkers for urolithiasis susceptibility.Key words: Urolithiasis, vitamin D, VDR gene polymorphism.

GENEL BİLGİ: Üriner sistem taş hastalığı çevresel ve genetik faktörlerin yakın ilişkisi sonucu oluşan kompleks bir hastalıktır. Son zamanlarda, vitamin D reseptör (VDR) allelleri ve Ca hemostazı ilişkisi araştırılmaktadır. Bu çalışma VDR gen polimorfizmleri ile taş ilişkisini belirlemek amacı ile kurgulandı.MATERYAL VE METOD: Taşı olan 12 çocuk (60 erkek, 6 kız) ile taş hastalığı ve pozitif aile öyküsü olmayan 60 sağlıklı çocukta (27 erkek, 33 kız) VDR gen polimorfizmleri, ApaI, Fok ve TaqI, serum elektrolit, PTH (PTH) ve 25 hidroksivitamin D ve üriner metabolik risk faktörleri çalışıldı. BULGULAR: ApaI genotipi için ÜST ve kontrol grubunda önemli fark olduğunu gözlemledik ( P=0.01). AA+ Aa genotipi Aa+aa genotipi ile karşılaştırıldığında taş olgularında 5.23 kat daha fazla olduğu bulundu (95% güvenlik aralığı: 1,5-18,2 ). Fok ve TaqI polimorfizmleri ile taş hastalığı arasında önemli bir ilişki saptamadık. Serum vitamin D3 seviyelerinin taş grubunda kontrole göre anlamlı yüksekti (p<0,001).TARTIŞMA VE SONUÇ: VDR geni ApaI AA+Aa genotipinin taş hastalığı ile yakın ilişkili olduğunu gösterdi. Bunun yanında, Fok ve TaqI polimorfizmleri ile taş hastalığı arasında önemli bir ilişki saptamadık. Fakat, Fok f alleli ve ff+Ff genotipi ÜST riskini artırıyor gibi gözükmektedir. TaqI t allel vet tt+Tt vet t genotipleri urolityazis riskini artırmaktadır. Bizim çalışmamız VDR polimorfizmlerinin taş hastalığına hassasiyeti artırabilecek potansiyel biyomarkır olabileceğini göstermektedir.
