In today's dentistry, implant-supported restorations can be applied in quite broad indications from a single missing tooth to the totally edentulous cases. Dental implant applications are are long-term treatments, including surgical and prosthetic phases. The treatment not only prevents alveolar bone resorption in the edentulous area, but also offers comfort of a fixed restoration to the patient.Providing satisfaction in totally edentulous patients is not always possible with complete dentures. In addition, as the age of the patient increases, alveolar ridge resorption, changes in balancing of the muscles, reduced masticator efficiency and vertical height, inadequate phonetic and esthetics are observed due to tooth loss. As a result, problems are occured in retention and stabilisation of protheses. To overcome these problems, implant supported removable partial dentures are used in totally edentulous patients in the recent years. However; due to such disadvantages like the rejection of the use of removable prostheses by the patients, the lack of retention and stabilization, implant-supported fixed prostheses are preffered by both patients and clinicians.According to anatomical conditions, 6-8 implants were placed in both maxilla and mandibula of the totally edentulous patients. Following the osteointegration, implant supported fixed protheses are constructed. After 1 month use of prostheses, patients were asked to chew prepared silicone chewing molds and sieving analysis method was used for evaluating the chewing performances. During that chewing process, surface EMG was applied to the right and left masseter muscles of the patiens. These tests were not only applied to the full arch implant-supported fixed prostheses patients but also patients with full arch natural tooth-supported fixed prostheses and full arch natural teeth.When the results are evaluated, a significant statistical difference between the EMG results obtained from patients during the chewing cycle was not found. However, although statistically significant difference does not exist; more amount of mold was observed in the collecting container in the patients with full arch implant supported fixed protheses. It is thought, this consequence is a result of limited chewing control mechanism in full arch implant supported fixed prostheses rather than groups with natural teeth. No statistical difference was found among groups in the amount of material loss. Although there is no statistically significant difference, the highest losses seen in patients was full arch implant-supported fixed prostheses group for both the right and left directions.

İmplant destekli restorasyonlar, günümüz diş hekimliğinde tek diş eksikliğinden tam dişsizlik olgularına kadar oldukça geniş endikasyonlarda uygulanabilmektedir. İmplant uygulamaları cerrahi ve protetik fazı içeren uzun süreli tedavilerdir. Söz konusu tedavi, diş eksikliğinin bulunduğu bölgelerde alveolar kemik rezorbsiyonu engellendiği gibi, hastaya sabit restorasyon konforu sunar.Dişsiz hastaların protetik rehabilitasyonunda tam protez uygulamalarıyla hasta memnuniyetini sağlamak her zaman mümkün değildir. Ayrıca hastaların yaşı ilerledikçe, diş kaybının bir sonucu olarak, kret rezorbsiyonu, çiğneme etkinliğinde azalma, kas dengesinde değişim, dikey boyutun düşmesi, estetik ve fonetik yetersizlik gözlenmektedir. Bunun sonucunda da yapılan protezlerde tutuculuk ve stabilite sağlarken problemler ortaya çıkmaktadır. Bu problemlerin üstesinden gelebilmek için tam dişsiz bireylerde implant destekli hareketli protezlerin uygulanması son yıllarda oldukça kullanılır. Ancak; hareketli protezlerin kullanılmasının hastalar tarafından reddedilmesi, tutuculuğunun ve stabilizasyonunun yetersiz olması gibi dezavantajları sebebiyle implant destekli sabit protezler hastalar ve hekimler tarafından tercih edilmektedir. Anatomik koşullar göz önüne alınarak, tam dişsiz hastaların her iki çenesine de 6-8 adet implant yerleştirilmiştir. İmplantların osseointegrasyonunun ardından sabit protezleri uygulanmıştır. Protezlerin uyum süreci olan 1 aylık kullanımdan sonra hazırlanan silikon çiğneme materyallerin, hastalar tarafından çiğnemeleri istenmiş ve hastaların çiğneme performanslarının değerlendirilmesi amacıyla, elek analizi yöntemi kullanılmıştır. Bu işlem sırasında hastaların sağ ve sol masseter kaslarına yüzeyel EMG uygulanmıştır. Bu testler, tam ark implant destekli sabit protez hastalarının dışında, tam dişli ve doğal diş destekli tam ark sabit protez kullanan hastalara da uygulanmıştır. Elde edilen sonuçlar değerlendirildiğinde, hasta gruplarından çiğneme siklüsleri esnasında alınan EMG sonuçları arasında istatistiksel açıdan anlamlı bir fark bulunamamıştır. Ancak, hasta grupları arasında hem sağ hem de soldan alınan tüm EMG ölçümlerinde tam ark implant destekli sabit protez hastalarının değerleri daha yüksektir. Ayrıca hastaların sağ taraflarından elde edilen potansiyellerin EMG değerleri ile sol taraflarından elde edilen potansiyellerin EMG değerleri arasında anlamlı bir ilişki bulunmuştur. Buna göre hastalar hem sağ hem de sol tarafıyla benzer ısırma kuvvetleri uygulamaktadır. Hasta gruplarının elek üzerinde kalan materyal miktarlarının ortalamaları arasındaki farklılıklar değerlendirildiğinde, istatistiksel olarak anlamlı sonuçlar elde edilmiştir. Ayrıca, sonuçlarında istatistiksel fark olmasa da, implant destekli tam ark sabit protez hastalarının toplama kabında daha fazla materyal miktarı olduğu görülmektedir. Bu sonucun, implant desteklerindeki çiğneme kontrolü mekanizmasının doğal dişli gruplara göre daha kısıtlı olmasından kaynaklandığı düşünülmektedir. Hastaların madde kaybı miktarları arasında istatistiksel olarak bir fark bulunamamıştır. İstatistiksel olarak fark bulunamamasına karşın, en yüksek kayıplar hem sağ hem de sol yönler için tam ark implant destekli sabit protez hastalarında görülmektedir.
