Objective: The aim of this study was to investigate the role of preoperative tumor markers and computed tomography in predicting 5-year survival in patients with endometrial cancer.Material and Methods: This study is a retrospective clinical laboratory study and early stage endometrial carcinoma patients who were operated between January 1, 2009 - January 01, 2014 at Karadeniz Technical University Faculty of Medicine, Department of Obstetrics and Gynecology and postoperative follow-up were evaluated. Demographic data of the patients (age, body mass index, gravida, parity, comorbid diseases etc.), preoperative tumor markers (CA 125, CA 15-3, CA 19-9, CEA, AFP), computed tomography results and histopathological type of pathology specimen, cervical involvement, degree of myometrial invasion, lymph node involvement, surgical stage, clinical and pathological features such as operation characteristics were evaluated retrospectively. All data were analyzed by using statistical methods with SPSS program such as descriptive statistics, chi-square test, Student-T test and Cox Regression for determining the effect of these factors on predicting 5-year survival.Results: The patients whom endometrial cancer were 108 patients and 87 patients (80.6%) alive, 21 (19.4%) patients were died. The mean time of life was 76 months and the mean time of death was 28 months. The mean age of the patients was 66.2 years ± 9.9 years. When the cases were evaluated according to body-mass index (BMI), the mean BMI was 31.7 kg / m2 ± 5.2 kg / m2. 101 (93.5%) of the patients presented with postmenopausal bleeding. Myometrial invasion did not affect prognosis in early stage endometrial cancer. We found that myometrial thickness was a significant prognostic marker when evaluated with preoperative CT 125 and preoperative CT. Uterine serosa involvement (p <0,001), cervical invasion (p = 0,032), lymph node metastasis (p <0,001), adnexial involvement (p <0,011), peritoneal cytology positivity (p <0,001) are also effective factors in prognosis. Histological grade didn't affected on survival. Histological type and nuclear grade affect on survival. We observed that the patients with advanced stage had less life and this condition was statistically significant (p <0.001). Age (p = 0.228) and lymphovascular involvement (p=0,153) related to assessment in predicting survival was no significant correlation. Chemotherapy was found to be significant in predicting survival (p <0.001). Radiotherapy and vaginal brachytherapy were not effective on survival. CA 15.3 was found to be a prognostic marker in predicting survival, and other cancer markers were not significant in early satage endometrial carcinoma. We found that CA 125 was a significant prognostic indicator compared to age and BMI (p = 0.044). We found that preoperative CT could not be a prognostic indicator in early stage tumors (p = 0.125) and could be significant in advanced stage (p <0.001).Conclusion: According to the results of our study, we conclude that tumor markers and preoperative computed tomography are important in predicting survival in patients with advanced stage endometrial cancer. CA 15-3 elevation is a prognostic marker in early stage endometrial cancer. We suggest that preoperative tomography, CA 125, CA 19-9, CEA and AFP are not effective on prognosis in patients with early stage endometrial cancer.

Amaç: Bu çalışmada amaç, endometrium kanserli olgularda preoperatif bakılan tümör belirteçlerinin ve bilgisayarlı tomografinin, olguların 5 yıllık sağkalımını öngörmedeki yerinin araştırılmasıdır.Materyal ve Yöntem: Bu çalışma retrospektif klinik laboratuvar bir araştırma niteliğindedir ve 01 Ocak 2009 – 01 Ocak 2014 tarihleri arasında, Karadeniz Teknik Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum Kliniğinde ameliyat edilen ve cerrahi sonrası takibi tarafımızca yapılan, erken evre endometrium kanserli olgular değerlendirmeye alınmıştır. Hastaların demografik verileri (yaş, vücut kitle indeksi, gravida, parite, ek hastalıklar vb), preoperatif tümör belirteçleri(CA 125, CA 15-3, CA 19-9, CEA, AFP), bilgisayarlı tomografi sonuçları ve patoloji spesmeninin histopatolojik tipi, servikal tutulumu, myometrial invazyon derecesi, lenf nodu tutulumu, cerrahi evresi, operasyon özellikleri gibi klinik ve patolojik özellikleri retrospektif olarak değerlendirilmiştir. Tüm veriler SPSS ortamına girilip, bu faktörlerin beş yıllık sağ kalımı öngörmede etkilerinin olup olmadığını belirlemede tanımlayıcı istatistikler, Ki-kare testi, Student-T testi ve Cox Regresyonu gibi uygun istatistiksel yöntemler ile analiz edilmiştir.Bulgular: Endometrium kanseri tanısı 108 olgudan, 87 (%80,6) hastanın yaşadığı, 21 (%19,4) hasta ise eksitus olmuştur. Ortalama yaşam süresi 76 ay olup, ölen vakalarda ortalama ölüm süresi 28 aydır. Myometrial invazyonun erken evre endometrium kanserinde prognozu etkilemediği gözlendi. Uterin seroza tutulumu (p<0,001), servikal invazyon (p=0,032), lenf nodu metastazı (p<0,001), adneksiyal tutulumun (p<0,011), peritoneal sitoloji pozitifliği (p<0,001) de istatiksel olarak prognozda etkili faktörlerdendir. Histolojik tiplendirmede ve nükleer grade sağkalım üzerine etkilir. Histolojik grade sağkalıma etkisi yoktu. İleri evre de olan hastaların daha az yaşadığı ve aradaki farkın istatiksel olarak anlamlı olduğu görülmüştür (p<0,001). Yaş (p=0,228) ve lenfovasküler tutulum (0,153) ile ilgili değerlendirmede sağkalımı öngörmede anlamlı korelasyon saptanmadı. Kemoterapi sağkalımı öngörmede anlamlı olduğu tespit edilmiştir (p<0,001). Radyoterapi ve vajinal brakiterapinin sağkalım üzerine etkili olmadığı tespit edildi. CA 15-3 sağkalımı öngörmede prognostik belirteç olarak saptandı, diğer bakılan kanser markerları anlamlı sonuç vermedi. CA 125 değeri yaş ve VKİ ile karşılaştırıldığında anlamlı prognostik göstergeç olduğunu saptadık (p=0,044). Preoperatif BT nin erken evre tümörlerde prognostik gösterge olamayacağı(p=0,125), ileri evrede anlamlı olabileceğini belirledik (p<0,001).Sonuç: Çalışmamızın sonuçlarına göre tümör belirteçleri ve preoperatif bilgisayarlı tomografi kullanımı endometrium kanserli ileri evre olgularda cerrahi öncesi değerlendirirken sağkalımı öngörmede önemli olduğu çıkarımına vardık. CA 15-3 yüksekliği, erken evre endometrium kanserinde prognostik göstergedir. Erken evre endometrium kanserli olgularda preoperatif tomografi, CA 125, CA 19-9, CEA ve AFP'nin prognoz üzerine çok da etkili olmadığını belirtmek isteriz.
