BACKGROUND : Asthma and hyperreactiveairway (HRA) disease are a major cause of healthresource utilization and poor quality of life worldwide;its prevalence in adults may widely vary according tothe definition used. It is mainly a childhood disease, butits natural history till adulthood is not well known. Thisis due to other confounding factors such as smoking andenvironmental factors that may lead to misdiagnoseasthma as chronic obstructive pulmonary disease. Correctand timely diagnosis of asthma and HRA is importantbecause it can be treated successfully and affectedindividuals may achieve good control of their disease.OBJECTIVE : To evaluate the prevalence of asthmaand HRA in Lebanese adults, their symptoms and predictors.METHODS : A cross-sectional study using a multistagecluster sample was carried out in Lebanon,between October 2009 and September 2010. Lebaneseresidents aged 40 and above participated to the study; apost-bronchodilator spirometry was performed to confirmdiagnosis.RESULTS : Out of 2201 individuals, 218 (9.9%) wereconsidered to have HRA. The following factors werefound associated with HRA in the Lebanese population:Northern (ORa: 3.54) and Bekaa Plain (ORa: 2.44)] versusother regions; occupational exposure to toxic gasesand fumes (ORa: 2.08); heating home with wood (ORa:1.75); having a family history of chronic respiratory disease(ORa: 2.19), a history of childhood lung problem(ORa: 5.53), and father smoking during childhood (ORa:1.47). Added to these factors, HRA was also predicted byfemale gender (ORa: 1.81); lower education (ORa: 1.20);older age (ORa: 1.28) and low birth weight (ORa: 3.14).CONCLUSION : This is the first epidemiological studyin Lebanon that determined physician diagnosed asthmaprevalence and hyperreactive airway disease andtheir associated factors among Lebanese adults. It alsoprovides useful contributions that local health organizationsmay use for national programming and a foundationfor health researchers to pursue further study inasthma research. Public awareness about the increasedrisk of asthma associated with heating homes withwood and exposure to passive smoking should beraised; Lebanese policy makers should also generatehigher efforts to implement and reinforce the smokingban law in public places.

Cependant, le diagnostic correct estimportant parce qu’il permet de traiter avec succès lesindividus affectés et assure un contrôle adéquat de leurcondition.OBJECTIF : Évaluer la prévalence de l’asthme et del’hyperréactivité bronchique chez les adultes libanais,en plus de leurs symptômes et facteurs prédictifs.MÉTHODES : Une étude transversale a été menée auLiban, entre octobre 2009 et septembre 2010 avec laparticipation de résidents libanais âgés de 40 ans etplus ; une spirométrie en post-bronchodilatation a étéutilisée pour confirmer le diagnostic.RÉSULTATS : Parmi 2201 individus, 218 (9,9%) étaientconsidérés atteints d’hyperréactivité bronchique. En plus de ces facteurs, l’hyperréactivitébronchique était aussi prédite par le sexe féminin(ORa : 1,81), un niveau d’éducation inférieur (ORa :1,20), un âge supérieur (ORa:1,28) et un poids inférieurà la naissance (ORa: 3,14).CONCLUSION : Il s’agit de la première étude au Libandéterminant la prévalence de l’asthme et de l’hyperréactivitébronchique et leurs facteurs associés parmi lesadultes. Elle nous permet de contribuer à l’organisationadéquate de programmes nationaux de santé et defonder des recherches plus poussées sur l’asthme auLiban. La sensibilisation du public concernant le risqueaccru d’asthme dû au chauffage au bois et au tabagismepassif devrait être augmentée ; les autorités concernéesdevraient aussi générer plus d’efforts pour implanter etrenforcer la loi anti-tabac dans les lieux publics.
