Background: Traumatic brain injury (TBI) still represents a leading cause of high morbidity and mortality. Objective: This study aims to determine serum level of sFas and TNF-α and its relation to primary outcome (survival/death) of patients following severe TBI and to show variation in Bcl-2 expression and apoptosis percentage after TBI. Patients and Methods: In this Prospective case control study, twenty five patients who were victims of severe isolated TBI (GCS 3–8) with no indication for surgical intervention were included in this study. Six healthy volunteers without history of TBI were included as control group. All of them were subjected to ELISA detection of serum sFas and TNF-α level. Flow cytometric analysis of Bcl2 expression upon PBMNC and apoptotic percentage was detected. Results: Serum sFas, TNF-α levels and Bcl2 expression level were significantly higher in TBI patients and in non-survivors than in control group and survivors respectively. Apoptotic cells percentage was significantly lower in TBI patients and non-survivors than in control group and survivors respectively. Conclusion: serum sFas, TNF-α levels can be used as prognostic markers for early selection of patients at risk of deterioration after severe traumatic brain injury.

لا تزال صدمة إصابات الدماغ (TBI) تمثل سبب رئيسي لارتفاع معدلات المرض والوفاة.و تهدف هذه الدراسة إلى تحديد مستوى sFas وTNF-α وعلاقته بالنتيجة الأولية (البقاء على قيد الحياة / الموت) في المرضى بعد TBI ولدراسة نسبة Bcl-2 ونسبة موت الخلايا المبرمج بعد TBI. تمت هذه الدراسة على خمسة وعشرين مريض تعرضوا ل TBI شديدة (GCS 3-8) بدون الحاجة للتدخل الجراحي. وأدرجت ستة متطوعين أصحاء دون إصابة TBI كمجموعة ضابطة. وقد تم عمل ELISA لتحديد نسبة ال sFas ومستوى TNF-α.وأيضا تم عمل Flowcytometric analysis للكشف عن نسبة Bcl2 في PBMNC ونسبة موت الخلايا المبرمج.و كان مستوى sFasو TNF-α ونسبة مستوى Bcl2 أعلى بكثير في المرضى الذين تعرضوا للإصابة الدماغية و غيرالناجين عن المجموعة الضابطة و المجموعة الناجية على التوالي وأيضا كانت نسبة موت الخلايا المبرمج أقل في المرضى الذين تعرضوا للإصابة الدماغية و غيرالناجين عن المجموعة الضابطة و المجموعة الناجية على التوالي الاستنتاج: يمكن استخدام مستويات sFas و TNF-α كعلامات منذره للاختيار المبكر للمرضى المعرضين للتدهور الشديد بعد إصابات الدماغ.
