Impacted teeth may remain in bone or mucosa for many years without anysymptoms and may cause pathology development as symptomatic or asymptomatic.At present, there is no consensus about the surgical extraction of asymptomaticimpacted third molar teeth yet. The reason for reporting different opinions on thattopic is that these teeth can lead to pain, infection, temporomandibular jointcomplaints, root resorption in adjacent teeth, as well as the potential to transformcystic or neoplastic pathologies from dental follicles surrounding impacted teeth. Butthey may not be cause any pathology and complaints for years. Dental folliclesurrounds enamel organ and dental papillae during tooth development, and staysattached to the crown when the tooh is impacted. This tissue is composed of fibrousconnective tissue and odontogenesis epithelial debris which it is reported to be thecausative agent of pathology. Radiographically, a dental follicle has a thin,semicircular appearance emerging from the enamel-cement junction, which is seen asa radiolucent area surrounded by radiopaque borders around the impacted teeth. Themain purpose of the histological evaluation of the follicle is to establish theproliferative activity of the follicle which has a a radiographically normal appearanceand also to get a pathological criterion to determine the incidence of cystic changes.It is difficult to detect pathologic developmental or early stage pathologies only byradiographic examination. For this reason, it is important to performhistopathological examination of the dental follicle routinely after the extraction fordefinite diagnosis.Ki-67 is an antigen associated with cell proliferation that can be seen in the S,G2 and M phases of the cell cycle other than G0. It is a cheap and useful marker thatcan be observed in the late G1 phase not in early G1, for the detection of tumor cellproliferation in a normal or neoplastic tissue .It has been reported that CD44 is a multifunctional surface molecule involvedin processes cell proliferation, cell differentiation, cell migration, angiogenesis,presentation of cytokines, lymphocyte distribution, T cell activation, chemokine and growth factor presentation at relevant receptors, introduction of proteases into cellmembranes. CD44 is essential in all these physiological activities in normal cells,also plays a role in the pathological activities of precancer and cancer cells.Smoking a global problem that has not yet been fully controlled, is important fordentistry World because the area first exposed to cigarette smoke in the body is theoral cavity. It has been reported that when the cells exposed to nicotine, the dynamicbalance between cell death and proliferation required for normal cell function isimpaired and leads to an increase in favor of proliferation.This study was carried out to investigate whether cigarette smoking causespathological degenerations in the dental follicle and the changes in expression of Ki-67, previously detected in various cancers, odontogenic cysts, and tumors, andincreased with epithelial proliferation, and also distribution and intensity of CD44which was altered in benign, premalign and malign lesions of oral cavity and alsoassociated with prognosis in previous studies.In our study, dental follicles and oral mucosa samples obtained from 52patients with clinical and radiographic asymptomatic fully impacted lower thirdmolar teeth, 26 smokers and 26 non-smokers were evaluated.Statistical analyzes were performed using the SPSS 21.0 (IBM Corp.Released 2012. IBM SPSS Statistics for Windows, Version 21.0, Armonk, NY: IBMCorp.) program.Our study concluded that cigarette use and gender did not affect the dentalfollicle and oral mucosa Ki-67 density in fully impacted in the lower third molar, thecystic and neoplastic change in the dental follicle of the asymptomatic implantedteeth was not greater than the oral mucosa and the prophylactic extraction of theseteeth is not necessary. It has been found that cigarette smoking can inducecarcinogenesis, because of the reduced CD44 score as the cigarette pack yearincreases and when considering the decreased CD44 expressions in squamous cellcarcinoma cases. Also gender and smoking influence CD44 distribution and densityin fully impacted lower third molars.Nevertheless, no exact findings were found that cigarette smoking increasesthe risk of developing pathology, or that prophylactic extraction should beconsidered in patients with cigarette smoking. For the approach to asymptomatically impacted third molar teeth, the last word has not been said yet. In order to have aconsensus on both topics, there is need for more researches that the number ofsample is higher and the follow-ups are longer.

Gömülü dişler semptom vermeksizin uzun yıllar boyunca kemik veya mukozaiçinde kalabilir, semptomatik ya da asemptomatik olarak patoloji gelişmesine nedenolabilirler. Günümüzde asemptomatik gömülü yirmi yaş dişlerinin cerrahi çekimikonusunda bir görüş birliği mevcut değildir. Farklı görüşlerin bildirilmesinin sebebiise, gömülü dişleri çevreleyen dental folikülden kistik veya neoplazik patolojileredönüşüm potansiyelinin bulunması, ağ rı, enfeksiyon, temporomandibuler eklemş ikayeti, komş u diş lerde kök rezorpsiyonlarına sebep olabildikleri gibi, hiçbirpatoloji ve şikayet oluşturmaksızın da gömülü vaziyette kalabilmeleridir. Dentalfolikül diş gelişimi esnasında mine organı ve dental papillayı çevreler, gömülüdişlerde ise kurona yapışık olarak bulunur. Bu doku fibröz bağ dokusu ileodontogenezisin epitel artıklarından oluşur ve patolojiye neden olan etkenin bu epitelartıkları olduğu bildirilmiştir. Radyografik olarak dental folikül, gömülü diş lerinetrafında radyoopak sınırla çevrilmiş radyolusent alan olarak izlenen, mine-sementbirleş im bölgesinden çıkan, ince, yarım daire ş eklinde görünümüne sahiptir.Folikülün histolojik değ erlendirilmesindeki temel amaç ise, radyografik olaraknormal görünümde izlenen foliküllerin proliferatif aktivitesini ve kistikdeğ iş ikliklerin insidansını belirlemek için patolojik bir kriterin oluş turulmasıdır.Gelişmesi muhtemel veya başlangıç aşamasındaki patolojilerin yalnızca radyografikincelemeyle tespit edilmesi zordur. Bu nedenle kesin tanı için dental folikülüngömülü diş çekimleri sonrasında rutin olarak histopatolojik incelenmesinin yapılmasıönemlidir. Nikotine maruz kalan hücrelerde, normalhücre fonksiyonu için gerekli olan hücre ölümü ile proliferasyonu arasındaki dinamikdengenin bozulduğ u ve proliferasyon lehine artış a neden olduğ u belirtilmiş tir.Bu çalış mada, sigara kullanımının dental folikülde patolojik dejenerasyonlaraneden olup olmadığ ını araş tırmak amacıyla, daha önce çeş itli kanser, odontojenikkist ve tümörlerde tespit edilen ve epitelyal proliferasyon ile arttığı tespit edilen Ki-67 ile önceki çalışmalarda oral kavitenin benign, premalign ve malign lezyonlarındaekspresyonunun değiştiği rapor edilen ve prognoz ile ilişkilendirilen CD44'ünyoğ unluğ u ve dağ ılımının immünohistokimyasal yöntemle araş tırılmasıamaçlanmış tır.Çalışmamızda 18-59 yaş aralığında, klinik ve radyografik olarakasemptomatik tam gömülü alt yirmi yaş dişi bulunan, 26 sigara kullanan ve 26 sigarakullanmayan olmak üzere 52 hastadan elde edilen dental folikül ve oral mukozaörneği değ erlendirilmiş tir.İstatistiksel incelemeler, SPSS 21.0 (IBM Corp. Released 2012. IBM SPSSStatistics for Windows, Version 21.0. Armonk, NY: IBM Corp.) paket programıkullanılarak yapılmış tır.Çalışmamızda, sigara kullanımının ve cinsiyetin tam gömülü alt yirmi yaş dişdental folikül ve oral mukoza Ki-67 yoğunluğunu etkilemediği, asemptomatikgömülü dişlere ait dental folikülün kistik ve neoplazik değişim riskinin oralmukozadan daha fazla olmadığı ve bu dişlerin profilaktik çekiminin gerekli olmadığısonucuna varılmıştır. Sigara paket yılı arttıkça CD44 skorunun azalması, SHKvvakalarında CD44 ekspresyonlarının azalmış olduğu göz önüne alınırsa, sigarakullanımının karsinogenezi tetikleyebileceği, cinsiyetin ve sigara kullanımının tamgömülü alt yirmi yaş dişi dental folikülünde ve oral mukozada CD44 dağ ılımı veyoğunluğ unu etkilediği tespit edilmiştir.Bununla birlikte, sigara kullanımının patoloji gelişim riskini arttırdığ ına ya dasigara kullanan hastalarda profilaktik çekimin düşünülmesi gerektiğ ine dair bulgulararastlanmamıştır. Asemptomatik gömülü yirmi yaş diş lerine yaklaşım konusunda iseson söz henüz söylenebilmiş değ ildir. Her iki konuda da konsensus olabilmesi içinörneklem sayısının daha çok ve takibin daha uzun olduğu araştırmalara ihtiyaçvardır.
